The injured employee and another employee were spinning the rotor. The injured employee bent down to pick up a dropped tape measure. While bent down, IE pulled on the hammer as part of the process. As this happened, the other EE was spinning the rotor. This caused the injured employee's right arm to be pinched between the hammer and the cheek plate.

The employee strained their back in February while lifting a tire. The employee received no medical treatment for the injury. While receiving a bone scan from a non-occ issue it was discovered that the employee had a fracture to a vertebra. The employee believes this is from the strain they suffered in February.

While changing an elevated belt that was on the ground in below freezing temperatures, EE stepped on the belt to manually pull the belt which caused the EE to slip. EE complained of left shoulder pain from the fall. EE was told prior to the fall to use a skid stir for the particular task.

EE was changing broken rear step on a 980G loader, When EE was taking off the bolts that hold the step on with a impact wrench, the bracket spun while EE was holding the rubber mount and the Rusty Bracket cut EE's left hand.

Employee was washing a 988H loader with a 2 inch fire hose while moving hose and stepping backwards stepped on hose and twisted left ankle.
Miner was pulling electric lines to the primary crusher. While pulling the ground line tight the strap he was using slipped. When the line slipped he grabbed the wire with his left hand and a sharp piece on line cut his hand.

The miner was welding hammers on the primary crusher. He took welding helmet off and using his hands brushed the dust off of himself. When he brushed the dust off the fan he was using to blow welding fumes out of crusher, blew a small piece of metal in his eye. The miner had on all of his safety equipment, including his safety glasses.
Employee was servicing the lime plant while stepping off the machine ladder, he stepped on a frozen piece of lime on the ground and twisted his ankle.

When finished welding he went to get up and experienced a sharp pain in his lower back. May have strained or pulled a muscle in his back.

Employee was cleaning off HP300 crusher platform with high pressure water hose. It was also raining; finishing up the employee started down the stairway leading from HP300 platform. His foot slipped on the wet step and he jammed his heel down on the step.
Employee was stockpiling 53's, wind blown particle got in right eye, employee was wearing hard hat and safety glasses/when bin door was opened to empty bin strong wind from east was blowing fines over to cat walk where stockpiler stands.

HE WAS KNELT DOWN GRINDING METAL, WHILE GRINDING HE HIT A BURR ON THE METAL AND THE GRINDER DEFLECTED OFF AND HIT HIS LEFT KNEE.
NO UNSAFE CONDITIONS EXISTED, EMPLOYEE SAID HE STRAINED HIS BACK REPLACING WIPERS ON WEST PLANT THOR CONVEYOR, EMPLOYEE WAS LOOSENING BOLTS THAT HOLD RUBBER WIPER IN PLACE MOVED AND STRAINED BACK.

EMPLOYEE OPENED DOOR ON WELDING TRUCK AND GOT IN. AS HE REACHED OUT TO CLOSE DOOR, THE WIND CAUGHT THE DOOR SWINGING IT SHUT. THE PICK UP DOOR HIT ON THE END OF HIS FINGER JAMMING THE FIRST JO INT, SEVERED THE TENDON.
