On April 13, 2026, employee experienced a right elbow strain disconnecting a loader shroud. Employee attributed it to an awkward position while using an impact gun. Reported on April 16, 2026, received an initial first aid assessment . Due to ongoing pain, a medical evaluation on April 28, 2026, diagnosed medial epicondylitis, leading to modified duty.

ON 9-22-25 EMPLOYEE REPORTED HAVING A FOREIGN BODY IN EE'S (R) EYE FROM TORCHING HOLES ON A NEW CLAD INSTALL ON 9-19-25. THE FOREIGN BODY WAS REMOVED IN THE EMERGENCY ROOM ON 9-22-2025. EE WAS PRESCRIBED EYE DROPS AND WAS RELEASED TO FULL DUTY SAME DAY.

During a routine maintenance procedure on an air conditioning unit, site mechanic was injured when EE's left leg became entrapped by the equipment's bucket. Employee sustained a left ankle and foot fracture.

Employee was climbing out of a chute with a lit torch in left hand when they swiped their right hand on the torch. Employee sustained a third degree burn to the right hand.

Employee was working on the jaw crusher. When EE cut the bolt, it ricocheted hitting the employee's face. Employee was treated at the emergency room and diagnosed with a fractured nose and fractured eye socket.

While training another person, employee looked back at the drill rig, the operator motioned the drill pipe. Employee noticed the drill pipe fell out of gripper arm. When employee touched the pipe to assist it back into the gripper arm. The pipe dropped and pinched the tips of fingers 3 & 4 on right hand

An employee was helping another mag drill holes. The plate was cleaned and drill clear, another employee turned it on at the same time IE reached back in to move a rag. It became entangled in the bit and around IE's glove. This caused lacerations to ring finger and lacerations/fractures to the tip of middle finger, resulting in the loss of 1/2 inch of the tip of middle finger.

EE was removing the segments on 988G loader bucket. EE was using 3/4" air impact gun to remove nuts from plow bolts. The bolt loosened, descended allowing it to spin. EE used palm to push the bolt back to lock it in place to allow impact to remove the nut from top. The plow bolt engaged EE's glove pulling EE's hand into it and caused a severe cut across the right palm/thumb.

EE was welding in the primary feeder, while welding EE suffered flash burn not knowing until the next day. Cause was possible flash reflecting off the feeder coming through the back of EE's welding hood.

On August 4, 2020 the employee struck a welding rod on the top side of EE's right hand in the thumb area causing an arching burn. The employee was returned to work without restrictions. On November 9, 2020, the employee was prescribed work restrictions following surgery to repair nerve damage.

On 5/4/19 employee was taken to the local hospital where EE received surgery for a puncture wound. While greasing the left lift cylinder of the 775E haul truck, and holding the grease lines together, the grease gun came loose of the grease zirk resulting in a puncture to the left thumb.

On 9/14/15 the employee was taken to the Company clinic, received stitches for a left eye orbit laceration and was released to regular work on 9/15/15. The employee was struck in the orbit of his left eye by a drag cable during an installation and sustained a laceration.

On 7/13/15, the employee was treated at an emergency room, was diagnosed with a 2nd degree burn to the left wrist and was placed on restricted duty which is being accommodated. The employee lost control of a torch and subsequently burned his left wrist.

Newly hired employee requested medical treatment on 11-10-14 and was placed on restricted duty which is being accommodated. The employee alleged that while operating a dozer pushing material, he backed to the top of the slope and when the dozer broke over to even ground, he felt pain in his lower back and leg. He was diagnosed with a lumbar strain and disc degeneration.

On 6/13/14, the employee requested medical treatment, was taken to the company clinic and was prescribed an antibiotic for left eye conjunctivitis. The employee felt something enter his L-eye while using an air impact wrench to tight bolts on a pipe. He was released to full duty.

On 11/6/13 the employee was taken to the hospital and received stitches for a laceration. While grinding on a crusher rotor, the grinder kicked back and struck the employee in the mouth resulting in a laceration to the upper lip. The employee was released to full duty on 11/7/13.

On 11-14-13 the employee requested medical treatment, was taken to the Company clinic and was placed on restricted duty which is being accommodated. The employee alleged that operating the excavator and possibly the bobcat caused pain in his lower back.

Mechanic was not looking up at air blower. One of four air diffusers had fallen out and as the machanic moved his hand across the diffusers his finger came in contact with the fan blade where the diffuser was missing.

The employee was taken to the company clinic on 7/18/08 and was placed on restricted duty. The employee was using a cutting torch and as he re-positioned himself, he turned the torch towards his R-wrist resulting in a 1st - 2nd degree burn.

The employee was taken to the hospital on 5/10/07 and was taken off work. The employee was struck on the head and chest by a tree limb while it was being cut down by another employee.

Employee was chipping a weld and a piece of hot slag entered his left eye. He was wearing clear safety glasses and a welding hood

The employee went to see the company doctor on 1/28/2006 and was placed on restricted duty. The employee was grinding on the crusher bowl threads when a foreign object became embedded in his L-eye.

The employee went to see the company doctor on 11/9/2005 and was placed on restricted duty. While cutting a chute liner with a welding torch, the flame burned through the glove and burned the employee on the right hand.

The employee went to see the company doctor on 9/6/2005 and was placed on restricted duty. While cutting wear plates with a welding torch, the employee burned his L-thumb due to a hole in the welding gloves.

Running crusher plant while lime silo was being emptied with hydrated lime. Five (5) days later, employee informed office of "rash or infection" to feet. Employee stated that he thought it was from the lime.

EE WENT TO SEE COMPANY DR. ON 3/19/04, STITCHES WERE APPLIED AND HE WAS PLACED ON RESTRICTED DUTY. WHILE REMOVING NUTS AND BOLTS FROM A LOADER BUCKET, EE CUT HIS LEFT INDEX FINGER CAUSING A LACERATION.

EMPLOYEE WENT TO SEE A DOCTOR ON 1/16/04 AND WAS PLACED ON RESTRICTED DUTY UNTIL 1/19/04 DUE TO A FRACTURE TO HIS TOE. EMPLOYEE WAS TURNING OVER A SKID STEER BUCKET TO INSTALL A NEW BOTTOM PIE CE AND BUCKET DROPPED ON HIS LEFT FOOT CAUSING A CONTUSION TO LEFT FOOT AND A FRACTION TO HIS LEFT TOE.

EE WENT TO SEE COMPANY PHYSICIAN ON 2-11-02 AND WAS PLACED ON RESTRICTED DUTY. WHILE USING CUTTING TORCH TO CUT A PIECE OF STEEL, IT FLIPPED OVER FROM A TWIST IN THE HOSE CAUSING A BURN TO EE' S LEFT HAND.

EMPLOYEE WENT TO SEE COMPANY PHYSICIAN ON 12-4-02 AND WAS PLACED ON RESTRICTED DUTY. EE USING A 9" HAND GRINDER ON A 10" I BEAM, WHILE GRINDING THE BEAM SHIFTED OFF SUPPORT ONTO RIGHT FOOT WHI CH CAUSED A FRACTURE TO RIGHT FOOT.

EE WENT TO SEE CO DR ON 8/9/02 AND WAS PLACED ON RESTRICTED DUTY. EE CLAIMS THAT OPERATING MOBILE EQUIPMENT OVER THE YEARS, REAGGRAVATED AN OLD BACK INJURY. ALLEGED.

WHILE CUTTING OFF THE BEARING SUPPORT ARM WITH A TORCH, SUPPORT ARM FELL ONTO EE'S L-KNEE WHICH CAUSED A CONTUSION & LACERATION TO L-KNEE. EE WENT TO SEE COMPANY DR ON 3/12/02 & WAS PLACED ON RESTRICTED DUTY.

WHILE HE WAS OPERATING THE JAW CRUSHER, A TOOTH FROM THE DRAGPLATE SHOT OUT OF THE JAW CRUSHER, GLANCED OFF ANOTHER OBJECT AND WENT THROUGH A CRUSHER WINDOW STATION AND STRUCK HIS SHOULDER.

EMPLOYEE WENT TO CLINIC ON 9/29/01 AND WAS RETURNED TO FULL DUTY AND RECEIVED PRESCRIPTION AND MEDICATION. EMPLOYEE WAS WELDING ON EXCAVATOR BUCKET WHEN HE RECEIVED FLASH BURN TO BOTH EYES.

EMPLOYEE WENT TO SEE HIS DOCTOR ON 8/17/01 AND WAS TAKEN OFF WORK. A PIECE OF METAL IMBEDDED INTO EMPLOYEE'S R-EYE WHILE GRINDING ON A LOADER BUCKET.

EMPLOYEE WENT TO THE COMPANY DR. ON 7-31-01, STICHES WERE APPLIED AND HE WAS PLACE ON RESTRICTED DUTY. EMPLOYEE WAS WELDING WELDS ON PIPE WITH LARGE ELECTRIC GRINDER WHEN THE GRINDER KICKED BA CK AND STRUCK THE EMPLOYEE IN THE R. KNEE WHICH CAUSED A LACERATION.

EE WENT TO SEE COMPANY DOCTORE ON 5-23-01 AND WAS PLACED ON RESTRICTED DUTY. EE WAS SITTING DOWN UNDER A CRUSHER MAIN FRAME PEROFRMING NORMAL JOB DUTIES WHEN RIGHT KNEE CRAMPED UP.

EMPLOYEE WENT TO SEE A DOCTOR ON 11-8-00 AND WAS PLACED ON RESTRICTED DUTY. OBSERVING DOZER PUSH MATERIAL INTO DOZER TRAP AND AS AIR WAS DISPLACED FROM FEEDER, IT CARRIED AIRBORNE PARTICLES WH ICH ENTERED EMPLOYEE'S RIGHT EYE.

EMPLOYEE WENT TO SEE THE DOCTOR ON 8-31-00 WHO REMOVED A FOREIGN OBJECT EMBEDDED IN RIGHT EYE. WHILE WELDING, A CHIP OF HOT, DRIED PAINT ENTERED INTO EMPLOYEE'S RIGHT EYE UNDERNEATH HIS CUTTI NG GLASSES.

WHILE PUSHING SAND AND GRAVEL MATERIAL IN THE PIT FOR PRODUCTION, EMPLOYEE RAISED BLADE OF DOZER AND DOZER JUMPED UP. EMPLOYEE FELT A SHARP IMPACT ON THE LOWER BACK.

EE WENT TO SEE DOCTOR ON 3/17/00 AND WAS PLACED ON RESTRICTED DUTY. WHILE GIVING HAND SIGNALS TO EQUIPMENT OPERATORS, A PIECE OF METAL OR ROCK SHOT OUT FROM UNDER TRAC OF DRAGLINE AND HIT EE I N THE NECK.
