Employee was pushing up a bolt using the wrench. Bolt was a 6 foot resin bolt. First bend went in the roof. Employee straightened and moved plate down. When EE applied up pressure the plate slipped on the bolt and employee did not have time to leave go of the plate. EE's thumb got pinched between the plate and roof. Resulting in a laceration and fracture of the left thumb

Showing all 50EE1 was assisting EE2 adjust head on oper. side. They were done adjusting head. EE1 told EE2 to lower head. EE1 verbally verified EE's hands are clear to EE 2. EE2 lowered head slowly about one foot when EE1 said "Whoa Stop". The long arm contacted EE1 left pinky and ring finger. EE2 started bolter up to free EE1 fingers that were pinched in canopy post and long arm.
Miner was adding second drill steel to set and a finger was pinched between the coupler and the steel in the roof. On 10/22/21, the treating specialist advised miner to stay home to prevent infection to injury.
Employee was installing a 6-foot roof bolt into the mine roof. When the bolt was being pushed up to the first bend, it rotated and caused a laceration to the palm of the employee's right hand just below the pointer finger. Employee received 4 sutures to close the wound.
Employee was trouble shooting a valve body on a joy miner. When he lowered the heads of the machine they contacted the bottom causing the rear of the machine to lower onto his foot causing three (3) broken toes and lacerations to two (2) of his toes
The roof bolter stood on the head of bolter to insert resin and bolt in a 9 ft high area. He asked another operator to raise wrench up with head to push the bolt into roof. After pushing the bolt up, the assisting operator began lowering the head. The victim's foot which was still on the drill head frame became pinched between the frame and hydraulic fitting of the machine.
The man was drilling a bolt hole into the roof near the rib to install a roof bolt. While lowering the drill head, a corner of the drill head struck the man on the upper leg.

DUE TO LOW CEILING HEIGHTS, INJURED EE HAD 2 BENDS IN 4' RESIN GROUTED ROOFBOLTER. UPON UTILIZING DRILL HEAD UP PRESSURE TO ASSIST BOLT INSTALLATION, ROOF BOLT BEARING SLIPPED OFF DRILL HEAD STRIKING EE'S GLOVED RIGHT HAND.
EMPLOYEE WAS STRUCK ON HIS FACE BY DRILL STEEL THAT WAS STUCK IN DRILL HOLE. STEEL SUDDENLY DISLODGED, STRIKING EE & CAUSING A LACERATION TO HIS LEFT CHEEK, REQUIRING 7 SUTURES FOR CLOSURE.
INJURED PERSON WAS INSTALLING A RESIN GROUTED ROOF BOLT WHEN HE INADVERTENTLY PINCHED TWO FINGERS OF HIS LEFT HAND BETWEEN THE ROOF BOLT BEARING PLATE AND THE ROOF. HE SUSTAINED OPEN FRACTURES OF THE MIDDLE AND RING FINGERS ON HIS LEFT HAND WHICH WILL REQUIRE HIM TO LOSE TIME FROM WORK.
While operating roof bolter employee dropped drill boom on right knee causing large bruise with contusion.
WHILE REACHING BEHIND ROOF BOLTING MACHINE TO MOVE TRAILING CABLE. HIS LEFT HAND WAS PINCHED BETWEEN BOLTING MACHINE AND RIB. HE RECIEVED INJURY REQUIRING 4 STITCHES
EMPLOYEE WAS LOWERING CANOPY JACK TO FACILITATE SLIDING OF METAL ROOF MAT ONTO DRILL HEAD WHEN HIS FINGER WAS PINCHED BY TELESCOPING CANOPY JACK SEGMENTS.
Continous Miner was tramming in reverse direction. Injured employee reached to move cable/h20 line from rear of machine. Finger was caught between miner and line post.
While drilling hole for roof bolt EE got piece of foreign object in eye. Had to get medical treatment with prescription eye drops.
Injured person was pushing a roof bolt (resin) into a bolt hole in the roof with a wrench in the chuck of the roof bolter. The bolt started bending and slipped out of wrench striking him on left wrist. He sustained a fractured left wrist.
THE INJURED PERSON HAD FINISHED BOLTING THE FACE OF #2 ENTRY AND WAS PREPARING TO LIFT THE FOOT JACK WHEN HE WAS STRUCK BY A PIECE OF FALLING DRAW SLATE. THIS SLATE HIT HIM ON THE LEFT SHOULDE R. HE SUSTAINED A CRACKED LEFT SCAPULA. HE WAS LOCATED IN A ROOF BOLTED AREA WHEN ACCIDENT OCCURRED.
EMPLOYEE WAS CUTTING OR GRINDING ON A SHUTTLE CAR AND A PIECE OF METAL GOT IN HIS EYE.
REACHING FOR A ROOF BOLTER STEEL, PIECE OF ROCK BROKE OFF FROM ROOF STRIKING EMPLOYEE ON LEFT HAND MIDDLE FINGER REQUIRING STITCHES TO LEFT FINGER.
WHILE OBSERVING THE PASS BEFORE THE FINAL PUSH OUT THE MINER HELPER WAS STRUCK BEHIND THE LEFT LEG BY A FALL OF RIB FROM #91 COAL BLOCK. THIS RESULTED IN A BRUISE TO LEFT CALF MUSCLE AND ACHILLES TENDON OF LEFT LEG.
