An operator was driving a haul truck down a heading when the haul truck started to slide on a downhill grade. The truck slid for roughly three headings then overturned in the travel way. The operator was wearing a seat belt and removed from the cab. EE was diagnosed with a hairline fracture on EE's left clavicle

Showing all 35While manually assisting a forklift to place a sawhorse, the north end of the sawhorse fell and struck the top of the effected employees right foot resulting in a laceration and fracture.
Employee was adjusting roller when pallet exited conveyor and rolled over their hand. Contusion on right hand. Nothing broken.
Tramming Fletcher Scaler and the tram motor went out causing it to free wheel downhill. Miner applied manual break. Scaler traveled 10-20 ft. and hit rib. On impact, miners leg slipped off break and hit the bracket holding the fire suppression system in cab causing a laceration.
Employee was doing the daily task of using a forklift to empty a dumpster filled with off-spec material into a front end loader bucket. As employee went to empty the dumpster the full bucket immediately bounced back causing employee's left hand to be pinned against the forks and the frame of the dumpster.
On 6-26-2020, emp was backing up a loader and struck a parked haul truck. Was jarred and had soreness in neck. EE was taken to the ER to be evaluated and was told to rest for the next 2 days, missing both shifts. EE returned to work on 6-29-2020 at full duty.
Employee was cleaning product from the ground in a congested area with a skid steer loader. The loader bucket caught the cement pad jerking the equipment. EE's right index finger was pinched between the controls and a cross beam.
The employee was driving haul truck in underground mine and ran over rock on road. Reported tightness in upper back but thought EE was OK. EE later requested a medical evaluation. EE was see on 2/9, and was put on restricted duty. An investigation was completed and it was determined that hitting the rock may have aggravated an exiting condition.
Loading rock with loader...hit a hard spot and jolted the operator. The sudden stop caused shoulder soreness.
Operating mechanical scaling equipment, an enclosed can. Mechanical scaler, scaling the wall, pulled rock off the wall, shook the cab of equipment. Employee was wearing seatbelt, jerked EE's body and experienced lumbar strain. Employee was seen by doc., lumbar strain. Follow up 1 week later, prescribed physical therapy. Employee working, no work restriction.

Employee was operating a haul truck traveling down rough roadway in mine. The back end of the haul truck bounced a couple times from hitting uneven road which caused injury to the lower back.
Employee was pulling on hand brake on railcar - dislocated shoulder
Loader bucket caught a rock and threw the operator forward in the 980 G seat. When he came back in the seat, he claims to have pinched a testicle. On Thursday, 4/9/09 he went to the emergency room. they said not to work on Friday. He came back on full duty on Monday, one day lost.
Employee was standing along side of a parked fork truck when a second fork truck backed around the corner hitting the employee.
Employee was operating a Komatsu hydraulic hammer scaler. While scaling the face, the motion of the Komatsu scaler lunged forward.
Unloading hydrate lime from bulk tank trailer into lime storage silo. Silo filled and transfer hose was unhooked before all pressure was drained causing lime to gush out of hose and onto employee resulting in lime getting in eyes even with safety glasses on.
While hauling limestone fines, tractor trailer tipped over injuring driver's shoulder and rib.

Employee developed a headache & felt sick. Claims it was fumes in the cab. The carbon monoxide detector that is mounted in the cab did not go off. The employee was taken to the hospital and was found to have carbon monoxide in his system. He was told to rest for the day. No medication was given.
While scaling the face of 14E - 31N, a piece of rock came off of the rib and hit the employee on the right ankle.

Employee was helping close the hood on #4 kiln with the south trolly when the coal pipe hit the top handrail and the handrail broke off and fell about 15ft striking the employee on the right arm.
Slick road pre-wet to be ready for dust control. Tag axle in front of driver was down making it hard to steer. 70+ mph winds and too much speed. Driver required three stitches to his head.

THE EMPLOYEE WAS MOVING THE 120' JLG OUT OF 7EW BOTTOM WHEN HE RAN OVER A LARGE ROCK WITH THE FRONT WHEEL - THROWING HIM AROUND INSIDE THE BASKET. HE HIT HIS LEFT LEG AND RIGHT ARM ON THE BASK ET RAILS.
BAGS OF LIME WERE BEING SLID DOWN A FREE(NON-POWERED) ROLLER CONVEYOR AND A BAG GOT CAUGHT IN THE ROLLER. EMPLOYEE REACHED IN TO PULL OUT BAG AND CAUGHT HIS HAND BETWEEN TWO ROLLS. A CONVEYOR BELOW WAS PULLING ON THE CAUGHT BAG AND PINCHED HIS HAND.
EE WAS WORKING WITH OTHERS TO HELP PUT A NEW BELT ON THE STOCKPILE CONVEYOR, WHEN THE BELT GOT STUCK ON A TROUGHING ROLLER. EE REACHED DOWN TO SEE IF HE COULD GET IT LOOSE & IT BROKE FREE ON I TS OWN STRIKING HIM ON THE HAND & CAUSING HIM A GREAT DEAL OF PAIN UP HIS ARM TO HIS SHOULDER.
BOULDER SLID OFF SHOT PILE STRIKING BUCKET OF LOADER.
OPERATING WHEEL LOADER AT MUCK PILE, ROUGH DIGGING MAKING FLOOR. REPORTED NEXT MORNING AT BEGINNING OF SHIFT "KNOT IN MY BACK" I FELT A SNAP IN MY BACK YESTERDAY WHILE DIGGING TOE.
DRIVER OF TRUCK WAS RETURNING TO THE QUARY AT CRICKET MOUNTAIN TO PICK UP ANOTHER LOAD OF LIME ROCK. JUST PRIOR TO REACHING THE QUARRY HE FELL ASLEEP AND RAN OFF THE ROAD. THE TRUCK WAS NOT DA MAGED BUT DRIVER SUSTAINED TWO LACERATIONS ON HIS FACE AND EYE.
EMPLOYEE WAS STANDING IN SAFE ZONE, WHILE HAUL TRUCK WAS DUMPING INTO HOPPR STONES, COME OFF HAUL TRUCK, HOPPER STRICKING EMPLOYEE.
