Showing all 25EE was reversing an L350H front end loader down the ramp that leads to the primary crusher. As the EE was reversing, EE cut the turn too close to the berm that lines both sides of the ramp. EE's right tires went on top of the berm, causing the FEL to tip over on its left side. EE sustained a dislocated left shoulder, and there was heavy damage to the left side of the FEL.
While backing dust truck up ramp to unload, miner got too close to right side berm and truck rolled over onto driver's side.
Employee was riding on the side of the lead car in a string of cars when EE slipped while dismounting and fell under the car. The car ran over EE's legs severing both legs.
While closing the right side window of 988G loader, finger was pinched in the closing mechanism causing laceration to the left hand pinky finger. This injury is reportable because the miner received stitches.

On top of truck repairing drive for auger unit on truck. Employee was running drive to check alignment on sprocket and chain. The unit was stopped so he could tighten set screw on key way for sprocket. The control was bumped and auger started to run, catching his index finger on left hand.
An employee was attempting to remove a rock from the tailpulley of the C-3 Conveyor. The injured employee reached into the tailpulley just as another employee jogged the belt. Lock-Out Tag-Out was not used.
Employee was running a front end loader and felt a pain in his back. This accident was not reported as work related until 9/23/04. The case was litigated and was accepted as work related on 12/28/04.

WAS DUMPING LOAD OF MATERIAL ON STOCKPILE. EE APPARENTLY BACKED UP BERM ALONG RAMP WITH THE LEFT REAR TIRES, CAUSING THE TRUCK TO TIP OVER.
WAS TRAMMING TO CRUSHER WHEN ROCK ROLLED OUT OF BUCKET OF FRONT END LOADER. LOADER RAN OVER ROCK. EMPLOYEE FELT PAIN IN BACK AFTER BEING JOLTED INSIDE CAB OF LOADER.
REACHED AROUND BEHIND AND ABOVE TO ADJUST CB RADIO AND FELT A SHARP PAIN IN NECK AND UPPER BACK. NOT REPORTED AND NO LOST TIME UNTIL 12-20-01.
EE WAS LEAVING PLANT PROPERTY AFTER CLOCKING OUTAT END OF SHIFT. LOST CONTROL OF HIS PERSONAL VEHICLE CAUSING A ROLL-OVER. HE WAS EJECTED FROM VEHICLE.
CAUGHT FINGER IN FAN WHILE TRYING TO REPLACE VENT.
EMPLOYEE EXITING MANLIFT; DOOR CAME DOWN AND STRUCK HIS BACK.
WAS INSTALLING SKIRT RUBBER ON CONVEYOR, CAUGHT HAND, CAUSING LACERATION.
