Employee was at primary crusher for plant 17 checking for leaks on a skid steer when a rock came out of the crusher causing a small laceration above their left eye.
Employee was helping install a plate in the top apron of the primary crusher when the plate they were holding slipped and came in contact with their neck. The plate had just been cut to fit in place and still was hot, welder was going to weld into place with employee holding it for them.
Employee climbed on C20 counterweight guard to remove ice from bend pulley when EE slipped and fell fracturing right wrist

Employee was working on drill, removing radiator hose when employee stepped backward into a hole for the service hatch. Employee tried to catch self with left arm and felt pain in the left shoulder.
Employee was assisting another worker with a belt splice when EE was struck in the eye by a foreign object
Employee was looking in the primary crusher to determine whether or not a rock was still wedged inside, preventing EE from closing the crusher. EE was standing on the adjacent drive belt guard at the time. EE slipped and fell, landing on a crate of tools on the catwalk below. Contusion to the right knee / Laceration to right arm resulting in 13 stitches.

Employee has been diagnosed with trigger finger of right thumb and carpel tunnel of right hand resulting from operating front end loader for years. Employee is currently working w/no restrictions pending treatment determination from medical provider.
Miner was at the garage using a pressure washer to clean off the top the skid steer. EE was trying to stand up on bucket, came back down and pivoted on right foot, slipped and fell, hurting right ankle.

EE was reversing an L350H front end loader down the ramp that leads to the primary crusher. As the EE was reversing, EE cut the turn too close to the berm that lines both sides of the ramp. EE's right tires went on top of the berm, causing the FEL to tip over on its left side. EE sustained a dislocated left shoulder, and there was heavy damage to the left side of the FEL.
While assisting install a loader bucket pin, the pin slipped and thumb was caught between pin and loader bucket.
While completing maintenance activities, miner smashed EE's middle finger between the head of a sledge hammer and the cone crusher body.

While working on FS1 changing screens, back started hurting as day went on.
Employee was helping with wash plant screen replacement. Another employee rolled a second bad screen down the stairs where it hit the employee lacerating EE's skin at the knee and requiring 10 stitches.
During installation of a gear reducer sheave, employee was using a pry bar to position the taper lock and align bolt holes. The pry bar slipped and stuck employee in the face.

Employee was climbing an extension ladder to tie off the top when his foot slipped and he fell about 5 feet, breaking three ribs and collapsing a lung.
Employee was replacing a feeder plate. He had a 3/4 electric impact gun on a shelf above were he was working. When he reached over to put a crescent wrench on a bolt he caught the cord of the impact which pulled it off the shelf. It struck his left thumb.
Was moving conveyor trusses around when the wind blew a foreign object into his right eye.

On top of truck repairing drive for auger unit on truck. Employee was running drive to check alignment on sprocket and chain. The unit was stopped so he could tighten set screw on key way for sprocket. The control was bumped and auger started to run, catching his index finger on left hand.
Employee was climbing a ladder when it slid out from under him, causing him to fall. Ladder was set up on snow and ice. Diagnosed with left hip pain.
