Caught in, under or between a moving and a stationary object

EE reported that EE was trying to remove rear tow pin from loader. EE was using another pin to dislodge the stuck pin and got EE's pinky finger cut/smashed in between the loader counterweight and the pin. This resulted in the tip of EE's left pinky finger getting cut.

Lifting/pulling screens or hammers and pulled muscle. Diagnosis of Right Inguinal Hernia was made on June 25, 2018. Lost time began July 3, 2018.

The employee was assisting on cutting out the S2 screen support tube and was sliding the pipe out the back and misjudged where the floor was and let go resulting in the pipe striking the fifth digit on EE's left hand. The finger received a small laceration and possible nondisplaced fracture.

Employee was helping install a crusher feed ring and pinched his right thumb between the ring and shaft. Injury was a laceration of the right thumb needing three stitches to close.

A non-employee was reportedly struck by a ricocheted bullet while deer hunting on a non-mining, agricultural area of the property. This location is over a mile from the active mining area. His injury was minor consisting of a soft-tissue injury of the shoulder. He received first aid treatment at a local hospital.

The employee was walking along a string of 5 rail cars when a second employee, operating a pick-up truck in reverse, hit and knocked down the injured employee. The victim received only minor contusions.

Was using a needle scaler to clean metal when a piece of rust got in to his eye. This was reportable because the foreign body had to be removed with tweezers.

Employee sprained his ankle while walking on rough terrain. In doing so, was exposed to poison ivy. Was diagnosed with rhus dermatitus on 9/9/05. Company was notified of diagnosis on 9/20/05.

Employee was driving a pin out of a conveyor support when the conveyor shifted, pinching his left thumb between the drift pin and conveyor frame. Employee received a laceration to his thumb requirig stitches to close.

Cleat on workboot caught on step of loader as he exited, causing knee to hyper-extend. Note: This accident was not reportable until employee had surgery on 5/3/05. Error in not reporting found during self audit.

Employee was exiting an all-terain forklift when his foot slipped off the rung. The hand that was holding on to the grab-bar was then twisted breaking a bone in his hand.

Accident type, without injuries

A leaking natural gas valve allowed gas to accumulate in the Dorr-Oliver dryer control room. The gas was apparently ignited by an electrical device inside the room. The resulting blast severely damaged the walls of the control room and caused a partial roof collapse.

HE WAS DISMOUNTING A HAUL TRUCK AND MISJUDGED THE HEIGHTH OF THE LAST STEP. HE LOST HIS BALANCE AND FELL CATCHING HIMSELF WITH HIS LEFT ARM. NO LOST TIME FROM THIS OCCURENCE UNTIL 2/19/04.
