Caught in, under or between a moving and a stationary object

Miner was inside Stedman crusher preparing to weld on reel. As they stood up, they placed a foot on the reel, dislodging wedge and rotating reel upward catching their leg and pushing it up against the apron.

Fall to the walkway or working surface

A contractor without a contractor ID installing arc flash labels at the mine walked down a slight incline off a small cement pad, lost EE's balance and broke EE's right leg.

Employees were trying to remove a crusher breaker plate by welding a lifting eye on the plate and attaching a come-a-long to pull it out. They put tension on the lifting eye and after a few minutes the eye released from the plate and struck one of the employees on the left cheek.

While closing the right side window of 988G loader, finger was pinched in the closing mechanism causing laceration to the left hand pinky finger. This injury is reportable because the miner received stitches.

Employee claims hit head on a chute while pulling a hose across the floor. This claim was disputed by the employer. Ohio BWC recently ruled this injury was work related. Notification was received by employer on July 15.

The employee was walking along the catwalk along C-7 conveyor when the last section of Gripstrut catwalk slipped out from under his feet causing him to fall about 9 feet to the ground. The section of catwalk was not bolted down.

Employee was drilling a 1/2 inch hole in a conveyor frame. The drill ""caught"" as it exited the steel and the torque from the drill twisted his right hand. Injury is fractures of F-3 & F-4.

Employee was greasing a tail-pulley of a conveyor. While in a squatting position, he twisted and felt a pain in his left knee.

Pinched his thumb between two wear-plates while repairing chutes. The employee did not seek medical attention until July 8, 2008 after pain developed where the thumbnail is growing.

Employee was cutting a piece of HDPE with a circular saw. He reached under the HDPE to see how close to the sawhorse he was with the saw and his finger came in contact with the rotating saw blade.

Employee was changing hammer on drill. The hammer started to fall and he tried to catch it, pinching his finger between the hammer and drill frame.

Employee was removing derailers from storage shelf when he struck his upper arm on shelf, resulting in a strain of the upper arm. This accident became reportable on 7/12/05, when lost time occurred.
