Employee was cutting a dried silicone caulk tube out of caulk gun with a utility knife, when the knife slipped at the end and cut EE's left ring finger requiring stitches.

EE was cleaning stone off a catwalk. While walking backwards, EE lost EE's balance and grabbed the handrail with EE's left hand for support. EE felt something pull/pop in EE's left elbow.

Employee opened a storage bin chute when fine material fell and got in EE's right eye. EE was diagnosed with a corneal abrasion.

Employee was carrying a replacement screen cloth up a conveyor catwalk when ee hit a support with the cloth injuring right shoulder. Injury was a dislocated shoulder.

Walking from shop to office, slipped fell on ice and broke leg.

Employee was cleaning around conveyors when a return idler guard fell off and struck his left hand.

Stepped off of truck loading platform and twisted his ankle (stepped on rock). Was not reportable until lost time occurred on 4/29/05.

Employee slipped on step and bruised his shinn. Note: Employee did not seek medical attention until 6/29/04. Lost time occurred from 6/30/04 to 7/9/04. Returned to work 7/12/04.

WAS SPLICING CONVEYOR USING A UTILITY KNIFE TO SQUARE BELT. IT SLIPPED AND HE CUT HIS FINGER. INJURY OCCURRED ON 1/5, BUT HE DID NOT SEEK TREATMENT UNTIL 1/6. WE WERE NOT AWARE THAT TREATMENT INCLUDED ""STERI-STRIP"" CLOSURES UNTIL MEDICAL REPORT WAS RECEIVED ON 1/12/04.

SLIPPED & FELL ON ICE, BROKE RIGHT ANKLE.

WAS DUMPING LOAD OF MATERIAL ON STOCKPILE. EE APPARENTLY BACKED UP BERM ALONG RAMP WITH THE LEFT REAR TIRES, CAUSING THE TRUCK TO TIP OVER.

EMPLOYEE WAS REMOVING TUBE ASSEMBLY FROM SCREEN WITH A COME-ALONG. TUBE CAUGTH ON FLOOR, CAUSING TO TO JUMP, STRIKING HIM IN THE ARM.

WAS HOLDING LARGE WRENCH SO ANOTHER EMPLOYEE COULD HIT IT WITH A HAMMER. PINCHED FINGER BETWEEN WRENCH AND CRUSHER.

WAS RE-SPLICING CONVEYER BELT WHEN CABLE COME-ALONG CAME APART CAUSING HIM TO FALL TO THE GROUND.

WAS INSTALLING SKIRT RUBBER ON CONVEYOR, CAUGHT HAND, CAUSING LACERATION.
