Employee was changing two troughing idlers. The first one was set into place. When the second was set, it caused the first to flip over striking the employee's arm. Injury was a laceration to the right wrist that required stitches.

Employee caught his right middle finger between the basket of the JLG Lift that he was operating and a conveyor frame, fracturing the tip of the finger.

Mechanic was trying to secure a tarp on an excavator. The wind was whipping and he lost his balance and fell on the ground. He landed on a previously injured knee (right). It was reported to the company immediately. Safety staff picked him up to be seen by a doctor. It was diagnosed as a fractured patella. He will see a specialist on Thursday at 9:15 a.m.

While changing screen cloth he was using a bar to position the screen panels. The screen above slid down and struck the bar bending his wrist backward. Original diagnosis was a sprain/strain of the wrist. The employee was diagnosed with a fracture on November 27, resulting in lost time.

The employee was removing a liner bed from a conveyor when it slipped. He tryed to catch it and felt a ""pop"" in his wrist and elbow.

An employee was attempting to remove a rock from the tailpulley of the C-3 Conveyor. The injured employee reached into the tailpulley just as another employee jogged the belt. Lock-Out Tag-Out was not used.
