On 6/3/2010, after employee was suspended, he alledged that he was carrying his tool box and slipped on the track rail. He was seen by a doctor on 6/15/2010, but did not address when employee could work, but tests were scheduled. The company does not have any information as to whether this is a reportable event.

Employee stepped from ladder on top of tank when the tank collapsed. He grabbed the edge the tank, cutting his fingers and resulting in three stitches.
Employee was helping install a silt fence, when he stepped in a drop off and fell approximately 4 feet. He was complaining with neck pain and was transported to the hospital and kept overnight for observation as a precautionary measure.
