Injury became reportable on 04/02/18 when EE had surgery. Employee was digging a headline cable that was buried in the sand and did not release pressure on the winch. Employee went to step over the cable, when it released from the sand it caught EE's leg it flipped EE. EE landed on right shoulder.

Showing 50 of 261The employee was working near the raw mill when EE tripped on the catwalk but caught himself on the handrail. When EE caught self EE felt a strain in right small finger. We monitored the strain to see if it would improve but it didn't. A doctor diagnosed that the tendon in the finger had torn and needed repair. The employee had surgery on 3/22/18 making it reportable.
Employee was walking to the picnic table for lunch and twisted right ankle. X-rayed and given Ibuprofen and an ace wrap and told to keep elevated with ice. Released to regular duty.
Employee was descending a ladder with a piece of material inside raw mill. One of the ladder legs shifted causing employee to lose footing and fall down. Employee was taken to hospital and diagnosed with rib fracture and fracture of transverse process vertebra.
Employee was washing out the log washer and felt a pain in calf causing employee to slip. Hyperextended right knee. Did not report injury until 5/22/17 and did not report it as work related until 5/25/17.
Employee stepped off of one barge to another as they turned to walk down the barge they tripped over the loose dock line causing them to twist an ankle.
On 3/15/17, employee tripped, fell on right knee, and experienced minor knee swelling. EE was recovering as expected until EE started performing a lot of maintenance work on EE's knees. On 3/31/17 EE's conditioned regressed and EE sought treatment from EE's PCP. X-rays revealed no broken bones; diagnosed with deep tissue bruise on knee cap and placed on restricted duty.
Employee was walking over to lock up the gas pumps for the day and slipped and fell landing on their left shoulder.
Employee slipped and fell outside while walking in snow / ice conditions. EE fell to EE's buttocks, experiencing mild back and leg pain. EE received initial first aid and on 12/25/2016 went to the ER for increasing pain. EE received a CT scan disclosing disc bulge in the lumbar area, with 4-6 weeks of PT and an MRI ordered. No missed work to date.
Injured employee was outside and walking towards the mill building entrance. As employee approached, slipped on ice, that was on a vehicle pathway, and fell on back. Employee reported a sore lower back and was instructed to implement first aid/RICE. Four days later employee felt they needed to go to the doctor and was placed on a 20 pound lift restriction.
Employee was exiting the front end loader to the ground via ladder, stepped off onto uneven ground and rolled ankle.

Employee was walking around skid steer and tripped over the edge of the bucket and feel to the ground. In the process of falling the employee attempted to grab a handle on the equipment and received a laceration to their thumb caused by a sharp metal burr on the handle.
EE was walking down a ramp to a water pump. EE stepped on the hose and slipped and fell, twisting right leg. EE underwent surgery to repair multiple fractures.
One of our dump truck drivers injured leg as EE was getting out of truck. EE caught foot on part of the truck causing EE to lose balance. EE landed awkwardly on other leg resulting in a fracture.

The employee was attempting to get into a plant pickup truck when EE rolled ankle. The employee was fine at the time but at the beginning of April began to feel pain in ankle. On May 3rd EE's doctor prescribed an ankle support boot to help ankle heal. The injury became reportable on May 3, 2016.

On 1/19/16 the employee slipped on the ice and fell as they were leaving work at the end of their shift. On 1/25/16 the ee requested medical treatment as there was minimal improvement to their condition. The employee was seen by a medical provider and diagnosed with a fractured rib. The employee was given prescription medicine making this a reportable injury.
Injured employee (IE) was trouble-shooting controls for an overhead hoist from a coal mill walkway. IE left the area and fell/stumbled forward while transitioning from one walkway to another. IE's right ear struck the mid-rail of an adjacent guardrail resulting in a 1cm laceration requiring two sutures. This is the first time the IE worked in the area.
Employee was coming down the stairs from the break room when he slipped and strained his left knee.
Employee was exiting the track mobile when he slipped from the step and strained his right hip.
He twisted his knee stepping out of a connex box on 17 Sep. Re-aggravated the knee on 21 Oct and on 23 Oct he requested treatment. We set up an appointment for 26 Oct. He went to the doctor on the 26th and they gave him a knee brace and Ibuprofen 800 and returned him to work at full capacity.

Employee was coming down from the slurry tanks on an icy metal stairway when he slipped. His right hand was on the handrail, but when his feet slipped, his right elbow went over the rail. He felt something pop in his right shoulder, which gave him shooting pain in the right shoulder and neck. The fingers on his right hand started tingling.
EE was helping move river cables and tried stepping over a 14" plastic pipe when he slipped and his right leg folded under. He received a contusion of his right knee.
Employee was coming down the tipple steps and tripped and fell on his wrist.
The employee was working in a skid steer and finished the task he was assigned. When he was done, he stepped out of the skid steer and landed wrong on his left foot. He felt a sharp pain in the tendon above his left heel. The working area was clean and paved with no debris or abnormal working conditions.
Trainee was looking for his cell phone and said he was going to the office to see if he left it there. He instead climbed back on top of the trailer without fall protection and hit his head on the overhead shoot causing him to fall off the trailer toward the ground. He landed on the drawbar between the trailers.
While fueling a loader, staged away from the quarry, fueler slipped on ladder attached to loader, falling to ground, injuring elbow. Weather conditions were wet/rainy and fueler's gloves were wet, causing the fueler to lose his grip on the ladder.
Employee was standing on the Marland C-Conn gearbox. He was handing another employee a hammer when he lost his footing and fell about 49 inches, landing on the heel of his left foot. He sustained a fracture to his left heel bone.
