The EE was walking down from the tower to drain the screw and missed the 3rd step from the bottom. The EE was carrying a crescent and during the fall smashed their right pinky between the wrench and the ground.

Showing all 21While washing the pad under the crusher, the employee did not realize that they were close to the edge and slipped off. The edge of the pad is approximately 8 inches tall. When the employee fell they landed on their right elbow causing a dislocation.
While changing an elevated belt that was on the ground in below freezing temperatures, EE stepped on the belt to manually pull the belt which caused the EE to slip. EE complained of left shoulder pain from the fall. EE was told prior to the fall to use a skid stir for the particular task.
The injured employee fell through an open bin door that was being monitored by another employee as they were pulling down material from the bin. The injured employee was aware of the door being open. The door had an employee kneeling beside it in order to see the bin level. That employee could not stop the injured employee before falling.
The employee was performing ground fault checks on a ladder. While ascending the ladder EE slipped on a rung and lost their balance. As EE was trying to stop their fall they caught themself with their right arm. This led to immediate discomfort in the EEs right shoulder.
EE WAS WELDING INDIDE THE PRIMARY CRUSHER, SLIPPED AND TWISTED HIS RIGHT KNEE. EE REPORTED THE INJURY BUT DIDNT WANT TREATMENT. ON 10/15/2004 EE DECIDED HE WOULD GET TREATMENT.
During morning clean-up, ee entered a feed box to clean out material build up. As he kicked material, it broke free causing him to twist his back and fall. As he fell he stradled the inner wall.
EE WAS IN THE SHOP. AN EXTENSION CORD WAS HANGING ON A HOOK ATTACHED TO A SHELF. THE CORD WAS NOT IN USE. ONE LOOP OF THE CORD HAD FALLEN TO THE FLOOR. EE REACHED FOR A SCRAPER & STEPPED INSID E THE LOOP OF THE EXTENSION CORD WITH HIS RIGHT FOOT. HE BECAME ENTANGLED, LOST HIS BALANCE & FELL BACK, REACHING BEHIND W/HIS RIGHT HAND TO CATCH HIMSELF, HE FELL & BROKE HIS RIGHT WRIST.
WAS CUTTING A CATWALK TO BE REMOVED UNDER SAND CLASSIFIER AND HE FELL ON GROUND BELOW AND AWAY FROM WHERE HE WAS STANDING.
EE WAS CLIMBING DOWN THE LADDER OF A FRONT END LOADER. THE LADDER WAS WET FROM RAIN. HIS FOOT SLIPPED ON A WET LADDER RUNG AND HE TWISTED HIS RIGHT KNEE TRYING TO CATCH HIMSELF FROM FALLING.
EE CLIMBED UP ON THE CRUSHER TO REACH AND CLEAN A CAMERA LENSE. AS HE WAS DESENDING, HE STEPPED ON TO THE MOTOR AND JUMPED DOWN ON TO THE DECK, TWISTING HIS LEFT KNEE.
EE WAS WALKING DOWN THE INCLINE TOWARD THE STONE CRUSHER WHEN HE SLIPPED ON A PATCH OF ICE AND FELL, TWISTING HIS LEFT ANKLE.
CLIMBING 6' LADDER TO CRUSHER, FELL BACK HIT FACE ON TRUCK BUMPER.
HE INJURED HIS ANKLE HELPING WITH PIPE PROJECT, STEPPED WRONG.
EE WAS HELPING CO-WORKER READ GROUND RESISTANCE. HE WAS WALKING ACROSS THE QUARRY FLOOR, WHEN HE STEPPED ON A ROCK. THE ROCK ROLLED, CAUSING HIM TO FALL ON TO ANOTHER ROCK.
EMPLOYEE WAS HELPING ANOTHER EMPLOYEE DO SOME REPAIRS, WHEN HE STEPPED ON A HOSE TWISTING HIS RIGHT ANKLE.
EE WALKING THE PLANT TO BEGIN HIS SHIFT, STEPS ON AN UNEVEN PART OF CATWALK,W HICH CAUSE DINJURY TO HIS LEFT KNEE. EE'S KNEE BENT OUT SLIGHTLY.
EE WAS BESIDE THE STONE PLANT AT THE WASH OUT AREA, WASHING OUT THE BED OF HIS HAULING UNIT. HE SLIPPED ON THE MUD AND TRIED TO CATCH HIMSELF AS HE WAS FALLING. HOWEVER, HIS FOOT WAS CAUGHT IN THE WATER HOSE. THE WEIGHT OF THE HOSE KEPT HISFOOT FROM MOVING AS HE WAS FALLING. THE LARGE LEG BONE WAS BROKEN IN 5 PLACES, AND THE SMALL LEG BONE WAS BROKEN IN ONE PLACE.
EE WAS WALKING ON THE CONCRETE SLAB (BESIDE OUR SHOP), CARRYING A PIECE OF STEEL TO THE TRUCK. HE STEPPED ON A FOREIGN OBJECT (ROCK OR PIECE OF WOOD), CAUSING HIS RIGHT ANKLE TO TWIST OUT FROM UNDER HIM. HE IMMEDIATELY CALLED THE QUARRY OFFICE ON HIS CELL PHONE, STATING THAT IT FELT LIKE HIS ANKLE WAS BROKEN. THE PLANT SUPERINTENDANT IMMEDIATELY TOOK HIM TO THE MED CENTER.
EE WAS CLIMBING BACK DOWN OFF GROVE CRANE AFTER STARTING IT. HE SLIPPED ON STEP OF CRANE LOOSING HIS FOOTING. HE FELL OFF CRANE BACKWARDS AND STUCK HIS LEFT HAND OUT TO BREAK FALL.
ICE & SNOW ON GROUND WAS VERY SLICK EE SLIPPED AND FELL AND BROKE HIS FINGER.
