Struck against stationary object

The employee was walking by an electrical box while wearing a hard hat and made contact with the electrical box. Injury did not become Reportable till 8/16/2024.

Fall onto or against objects

The employee was performing ground fault checks on a ladder. While ascending the ladder EE slipped on a rung and lost their balance. As EE was trying to stop their fall they caught themself with their right arm. This led to immediate discomfort in the EEs right shoulder.

Fall from machine

EE WAS EXITING CAT 216B SKID LOADER. HE STEPPED OUT OF MACHINE TO GROUND ON RIGHT LEG. BEFORE LEFT LEG REACHED GROUND HIS RIGHT KNEE CAP AREA POPPED CAUSING PAIN & FELL TO GROUND. DID NOT SEEK MEDICAL ATTENTION UNTIL FOLLOWING MORNING WHERE DOCTOR PRESCRIBED MEDICATION AND KNEE BRACE WITH LIGHT DUTY RESTRICTIONS FOR 5 DAYS.

Struck against stationary object

The men were attempting to replace a bearing on the yarder. EE was swinging a sledge hammer at a gear puller several times. On the last swing, the hammer missed its mark, causing his arm to travel further around. His left forearm struck a pipe hand rail that had been cut off with a pipe cutter prior to the beginning of the shift. This laceration required 8 staples.

Fall to the walkway or working surface

While working outside, ee walked across an icy area. His feet flew up & he fell flat on his back knocking his hard hat off. He laid very still & possibly lost consiousness, but got up & continued working to finish the day. The next day was a snow day off & he continued having headache & soreness. He sought medical treatment on 1/29/09 & will be off work until 2/2/09.

While trying to loosen a sheave, employee placed a block of wood against it. While still holding the block of wood a co-worker proceeded to swing the sledge hammer striking EE's finger & smashing it causing a fracture to his middle finger.
