The EE was walking down from the tower to drain the screw and missed the 3rd step from the bottom. The EE was carrying a crescent and during the fall smashed their right pinky between the wrench and the ground.

While exciting drill truck stepped onto a rock & stumbled twisting left knee felt some pain but not intolerable finished shift drove home pain level about the same when woke up next morning knee was swollen and pain was to the point that they couldn't bend or twist their knee.

EE was working with another employee picking up the 92lbs shoes for the VSI crusher. The injured EE was picking up the shoe from the crate and handing it to the 2nd employee to put in place. As the injured EE bent over to pick up the shoe, they had intense pain in their groin and abdomen.

The accident occurred at 3:35PM. Employees were replacing the #2 primary conveyor lower bend pulley. The injured EE was pulling the bearing on the shaft into position when the pulley rolled and came forward, pinning EE's left hand between the bearing and the conveyor frame.

The injured employee and another employee were spinning the rotor. The injured employee bent down to pick up a dropped tape measure. While bent down, IE pulled on the hammer as part of the process. As this happened, the other EE was spinning the rotor. This caused the injured employee's right arm to be pinched between the hammer and the cheek plate.

The EE was asked to unplug the crusher. During this process, the EE LOTO the crusher and began to dig out the stone that was causing the issue. While performing this task a piece of stone came loose and struck the EE's right hand. The EE was then taken to urgent care and then referred to the ER. At the ER the EE was sent to Ortho to address the issue.

While washing the pad under the crusher, the employee did not realize that they were close to the edge and slipped off. The edge of the pad is approximately 8 inches tall. When the employee fell they landed on their right elbow causing a dislocation.

The employee strained their back in February while lifting a tire. The employee received no medical treatment for the injury. While receiving a bone scan from a non-occ issue it was discovered that the employee had a fracture to a vertebra. The employee believes this is from the strain they suffered in February.

According to the electrician, the wire came out of the lug and landed on the fuse holder causing the arc when the employee went to install the new fuse. The employee was not able to see the loose wire due to the plastic guard that keeps individuals from making contact with the live top. During the investigation it was discovered that the box was properly locked out.

While changing an elevated belt that was on the ground in below freezing temperatures, EE stepped on the belt to manually pull the belt which caused the EE to slip. EE complained of left shoulder pain from the fall. EE was told prior to the fall to use a skid stir for the particular task.

The injured employee fell through an open bin door that was being monitored by another employee as they were pulling down material from the bin. The injured employee was aware of the door being open. The door had an employee kneeling beside it in order to see the bin level. That employee could not stop the injured employee before falling.

EE was cutting an old belt to use as skirting boards for the tail end of the number 5 belt. The EE was using a box cutter to perform this task. The EE was cutting towards themself when the knife slipped and cut the EEs right mid-thigh (approximately 8 inches above the knee) leaving a 2 cm laceration.

The employee was performing ground fault checks on a ladder. While ascending the ladder EE slipped on a rung and lost their balance. As EE was trying to stop their fall they caught themself with their right arm. This led to immediate discomfort in the EEs right shoulder.

EE was moving a 2x3x.75 steel plate off of a pallet onto a work table. The steel plate was positioned vertically on the table and started to slip. EE attempted to stop the fall of the plate and injured EE's Left Middle Finger. EE was wearing PPE at the time of the incident.

EE WAS USING IMPACT TO TAKE NUT OFF A SQUARE LEAD BOLT IN WHICH THE BRACKET HAD A PIECE ON IT TO KEEP THE BOLT FROM SPINNING, BUT THE BOLT SPUN OUT AS EE WAS PUTTING PRESSURE ON THE BOLT WITH HIS FINGER. THE BOLT CAUGHT EE'S FINGER, CAUSING A LACERATION.

HE COULDN'T HEAR OUT OF THAT EAR THE NEXT MORNING, BUT WAITED TO SEE IF IT GOT BETTER. HE WENT TO FAMILY DR.ON 5/13/04. THEY FOUND THE METAL ON HIS EAR DRUM, & REPORTED THAT IT WAS INFECTED. PRESCRIBED ANTIBIOTICS & REFERRED HIM TO SPECIALIST TO REMOVE METAL.

EE WAS MOVING A TOGGLE BLOCK FOR THE JAW CRUSHER. HE AND ANOTHER EE TIPPED THE BLOCK BACK TO MOVE SOME WOOD CRIBBING AND WHEN THEY LET THE BLOCK DOWN EE'S RIGHT THUMB WAS UNDER IT. THE WEIGHT OF THE BLOCK WAS SUCH THAT THEY HAD TO TIP IT BACK TO RELEASE HIS THUMB.
