Employee was exiting a tool trailer onto the top of a 4ft ladder. EE lost EE's balance, jumped down and twisted EE's knee. EE sought medical treatment and on 7/11/25 EE was Dx with a ACL tear.

Showing all 26Employee was installing equipment with crane assist. While walking employee tripped over a dunnage block and fell down. Employee sought medical treatment and was Dx with a rotator cuff tear. Reporting within 10 days.
Employee was performing equipment setup on water tank. While walking to hook up a hose, EE slipped on limestone slurry and fell down. EE sought medical treatment and was DX with a right shoulder strain. On 11/18/24 we were notified that EE was referred to physical therapy. Reporting within 10 days.
Employee was walking while moving a conveyor, tripped and fell, rolled EE's left ankle. Employee sought medical treatment, was diagnosed with a sprain. Was taken off work for a couple of days.
Employee removed seatbelt to exit the broom and got tangled in the seatbelt. Employee grabbed the side handle and injured right shoulder. Employee first sought treatment on 7-14-2021. Employee had an MRI on 7-20-2021. We received the MRI report on 7-26-2021 which showed a partial tear.
Employee was clearing off catwalk to mobilize the plant and caught a toe on wood blocking. Employee grabbed the handrails to stabilize self and in doing so strained left shoulder. Employee initially did not seek medical treatment. On 4-10-20, employee sought treatment with a chiropractor.
EE was performing maintenance while standing on the counterweight of a loader. EE misstepped and fell. EE broke collarbone.

Employee was climbing off loader using the attached ladder & while on the last rung employee's foot got caught & employee fell on their side. Employee sought medical treatment & is currently off work.
Employee was pulling a silt barrier across a wash pad & strained knee. EE sought medical treatment & was recently referred to physical therapy. Due to this recent treatment this is now MSHA reportable.
Employee was walking to personal vehicle at the end of shift & fell on ice in parking lot that had dislodged during work shift. Employee did not seek medical treatment immediately. Employee has now sought medical treatment with chiropractor.
An employee was getting into a skid steer on 5-28-14 and slipped causing him to fall and land on the skid steer floor. He did not seek medical treatment until 12-6-14 when he saw a chiropractor.
Passing motorist intentionally drove thru locked quarry fence & vehicle got stuck. He walked to nearby top edge of high wall. Waukesha County Sheriff responded at 8:03 PM. 2 Sheriff Deputies tried to "talk him down," but he jumped at 8:24 PM landing 80 feet below on quarry floor. We were notified at approx. 10:35 PM & we reported the incident to MSHA at 10:45 PM CST.
Employee was tightening bolts on machine. He bumped his shoulder & lost his balance & fell, lending on both feet. He was diagnosed with a heel fracture.
Employee was stepping from concrete onto uneven ground & rolled his left ankle. He sought treatment & was given an ankle brace.

RUNNING TO SHUT DOWN CONVEYOR TO CONE CRUSHER HESLIPPED & FELL ON DEBRIS THAT HAD BEEN SHOT FROM
STEPPING DOWN OFF OF WATER TRUCK, SLIPPED & CUT KNEE CAP, STITCHES AND ONE DAY OFF.
STEPPING UP ONTO METAL PLATE NEAR CONVEYER WHEELS AND THE PLATE BROKE. EMPLOYEE FELL SCRATCHING AND BRUISING LEG, LATER IN THE WEEK THE WOUND BECAME INFECTED AND REQUIRED TREATMENT.
WENT TO PICK UP LADDER & SLIPPED IN MUD TWISTIN G KNEE. MEDICAL ONLY, NO LOST TIME.
TEARING DOWN THE PRIMARY JAW ON CRUSHER AND THE SUPPORT PINS HAD TO BE REMOVED. HE WAS WALKING ON SNOWY CONVEYOR AND SLIPPED AND CAUGHT HIMSELF W/ARM AND PULLED SHOULDERS.
