While attempting to secure a concrete block used as a counter weight, the strap that was holding the block broke. The employees right foot was resting on the block as he was securing the counter weight. When the block fell so did the employee.

The accident occurred on 8/20/2005 but the employee did not seek treatment until 8/25/2005. Two maintenance employees were taking a seat out of a D6H dozer. As they were moving the seat out the the cab one employee slip and felt a pull in his back.
EE needed oil for loader-He stepped up into oil shed, had a misstep and kicked side of step twisting left knee which resulted in severe pain.
