EE was using a 3lb hammer to connect equipment. Set hammer on conveyor belt and place hand on structure. Hammer fell, striking the left thumb of EE.

Victim was riding with another EE to the pit. When they exited the plant truck, the victim fell to the ground almost immediately. The driver called out, thinking that EE had tripped. Upon walking around to the back of the truck, the driver noted that the victim was on the ground, not breathing and had no pulse.

The employee stepped off of a concrete pad, lost his balance and fell. The fall resulted in a broken ankle.

Standard Threshod Shift on Hearing test, noted at ""reportable hearing loss"" as required by Part 50 for reporting occupational injuries and illnesses. Amended original edoc to include name, social and job title per call to Company on 4/1/04.
