Employee was attempting to remove snow when EE slipped and landed on their right hip.

Employee was walking catwalk on Primary 2 access the ladder at the bottom of the conveyor. As EE crossed a section of catwalk under head pulley of the return belt, EE stepped on a weak section of the catwalk. The weak section of catwalk gave way causing EE's right foot to break through the catwalk. Sprained right ankle; working restricted duty.

Employee was walking and rolled ankle on uneven ground.

Employee slipped on ice and fell while fueling a piece of equipment.

An employee was descending the cab access ladder of a haul truck. The light illuminating the ladder timed out and went dark. The employee continued to descend the ladder in the darkness. The employee thought ee was on the bottom rung of the ladder and stepped down, causing ee to fall and strike knee on the bottom rung of the ladder

Employee was removing a conveyor when he struck his left shoulder on the side of it.

Walking on uneven terrain, tripped over rock in path. Lacerated and fractured left hand index finger.

Employee was dismounting his articulating haul truck at the end of the day. While proceeding down the steps his foot caught awkwardly in one the steps causing his hand to slip off of the railing and fell about 5 feet to the ground. Refused treatment initially, but the next morning wanted to get checked out and was diagnosed with 2 broken ribs on his right side.

Employee was hooking up clevises to a lifting hook, which he gained access by climbing up a ladder on the inside. After hooking up clevises he decided to jump off an 8' high tank to the ground. He strained his thigh on his right leg.

Employee was walking on a blacktop surface, when he went to turn his leg turned but his ankle didn't, causing an ankle sprain to his right ankle.

Employee was walking down steps at the secondary plant, when he got to the bottom of the steps, he slipped and fell and landed on his left hand. Diagnosed with a fractured thumb.

Employee was unjamming the primary crusher, while doing this, he picked up some oil on the soles of his shoes from a leaky hydraulic line. When he went to walk down a set of stairs, he slipped and overturned his left ankle. Diagnosed with a sprained ankle.

Employee was walking down steps at the sand tanks when he slipped on the ice covered steps. He went to catch himself with the hand rail and that was also ice covered which caused his hand to slip on rail. He bent his right index finger backward and fractured his finger.

Stepped into a water puddle that was deeper than orginally thought, fell forward on knee and brusied knee.

Employee was refueling his dozer when he slipped and came down on his left knee. Contusion to knee at this point but has to follow up with a specialist.

EE WAS OBSERVING THE #3 PIT CONVEYOR LINE. WHILE OBSERVING HE WAS WALKING DOWN HILL & SLIPPED IN MUD, HE SAID HE HEARD SOMETHING POP IN HIS LEFT FOOT. DIAGNOSED WITH A SPRAINED LEFT FOOT/ANKLE .

EE WAS SHOVELING SAND OUT OF SAND STORAGE TANK WHEN HE SLIPPED AND FELL BACK AGAINST THE EDGE OF THE TANK AND BRUISED HIS LOWER BACK.

EMPLOYEE WAS WALKING ALONG PIT CONVEYOR LINE STARTING UP LINE AND SLIPPED IN MUD AND FELL.

OPERATOR WAS PUTTING ANTI FREEZE IN RADIATOR OF LOADER. WHEN FINISHED HE WAS STEPPING DOWN ON STEP TO EXIT MACHINE. HIS FOOT SLIPPED OFF OF STEP CAUSING HIM TO FALL AND HIT HIS TAIL BONE ON MA CHINE.

OPERATOR SLIPED OFF STEP EXITING CAB OF MACHINE FALLING ONTO A HYDRAULIC CYLINDER CONTACTING LOWER LEFT SIDE OF BACK.

OPERATOR STEPPED OUT OF EXCAVATOR CAB ONTO TRACK STARTED TO STEP OFF OF TRACK ONTO THE NEX STEP EXITING THE MACHINE. HIS FOOT SLIPPED OFF OF STEP AND HE FELL TO THE GROUND LANDING ON BUTTOCKS ABOUT 3 FEET. WENT TO DOCTOR 4/15/02. FIRST DAY OFF 4/16/02.
