The employee thought ee was on the bottom rung of the ladder and stepped down, causing ee to fall and strike knee on the bottom rung of the ladder
Employee was removing a conveyor when he struck his left shoulder on the side of it.
Walking on uneven terrain, tripped over rock in path. Lacerated and fractured left hand index finger.
Employee was dismounting his articulating haul truck at the end of the day. While proceeding down the steps his foot caught awkwardly in one the steps causing his hand to slip off of the railing and fell about 5 feet to the ground. Refused treatment initially, but the next morning wanted to get checked out and was diagnosed with 2 broken ribs on his right side.
Employee was hooking up clevises to a lifting hook, which he gained access by climbing up a ladder on the inside. After hooking up clevises he decided to jump off an 8' high tank to the ground. He strained his thigh on his right leg.

EE WAS SHOVELING SAND OUT OF SAND STORAGE TANK WHEN HE SLIPPED AND FELL BACK AGAINST THE EDGE OF THE TANK AND BRUISED HIS LOWER BACK.
EMPLOYEE WAS WALKING ALONG PIT CONVEYOR LINE STARTING UP LINE AND SLIPPED IN MUD AND FELL.
OPERATOR WAS PUTTING ANTI FREEZE IN RADIATOR OF LOADER. WHEN FINISHED HE WAS STEPPING DOWN ON STEP TO EXIT MACHINE. HIS FOOT SLIPPED OFF OF STEP CAUSING HIM TO FALL AND HIT HIS TAIL BONE ON MA CHINE.
OPERATOR SLIPED OFF STEP EXITING CAB OF MACHINE FALLING ONTO A HYDRAULIC CYLINDER CONTACTING LOWER LEFT SIDE OF BACK.
OPERATOR STEPPED OUT OF EXCAVATOR CAB ONTO TRACK STARTED TO STEP OFF OF TRACK ONTO THE NEX STEP EXITING THE MACHINE. HIS FOOT SLIPPED OFF OF STEP AND HE FELL TO THE GROUND LANDING ON BUTTOCKS ABOUT 3 FEET. WENT TO DOCTOR 4/15/02. FIRST DAY OFF 4/16/02.
