Team member was assisting rolling pipes on the dredge. Tool became disengaged when team member went to pull down on handle, causing them to lose balance and fall backwards onto a metal box.

Miner was negotiating stairs to perform maintenance on a screen. When they turned to face screen, their foot slipped off of the stair tread causing them to trip down the remaining two steps and sit down on top of their right foot/ankle. Miner was evaluated for right ankle strain.

Team member was walking on unstable ground. Sunk into mud up to knee, went to turn to grab a stable ladder and felt a pop in the right knee.

The employee was walking across the yard and slipped and fell in the snow. Hitting the back of EE's head.

Miner was removing a rock from a conveyor. When they stepped back, they tripped over a rock and jammed their left thumb while trying to catch themselves.

WHILE CLIMBING ONTO LOADER, EMPLOYEE CAUGHT RIGHT FRONT TOE OF BOOT ON DECK CAUSING EE TO FALL FORWARD AND JAM RIGHT MIDDLE FINGER INTO CORNER OF RADIATOR GUARD.

Office worker tripped on floor mat, falling and breaking right femur.

Employee was walking by the mud pump and slipped in the mud, injuring lower back.

Prior to shift, team member fell through doorway into break room landing on left side and breaking hip. Walkway and break room are finished concrete slab which were clean and clear of identifiable obstructions. No other environmental factors were identified.

Employee was walking from personal vehicle to office and tripped over a small taper at scales, hitting forehead on ground and causing a contusion.

Employee was walking towards a conveyor on uneven ground and lost balance, causing him to fall and twist ankle.

Employee was walking out of switch van after putting harness on. He started to slip on the top step, but caught himself on hand rail, straining his back.

Employee was replacing a 10" diameter pipe that went through the catwalk (expanded metal) that he was working on. After he removed the pipe and co-workers were raising the new pipe, the employee turned to retrieve an air hose and his left leg fell into the 10" hole in the catwalk where the old pipe used to be.

Employee wandered away from job duties, past a clearly marked "Do Not Enter" sign and fell into crusher feeder. He broke his left femur and left wrist.

Slip, fall while getting out of cab of service truck. Back strain.

Slipped while stepping down from a conveyor Recieved an acute lumbard strain

Employee stepped off of truck and turned to shut the door and twisted his knee.

While getting off of D-10 Dozer, slipped and fell. Cut the base of right thumb requiring stitches. Restricted work - no use of right hand for 1 week.

Working on a Ty-rock shaker, miss-stepped on ladder coming off; slipped and landed on right knee.

STEPPING BACK TO DISMOUNT FROM LOADER AND TRIPPED ON THE BUCKET OF THE YARD LOADER. FELL BACK AND LANDED ON BACK OF LEFT SHOULDER.

WHILE EXITING FORKLIFT, TURNED ANKLE.

STEPPING OUT OF LOADER & TWISTED ANKLE

CLIMBING INTO LOADER OFF OF A CEMENT BLOCK, SLIPPED AND CUT LEFT SHIN ON THE EDGE OF THE CEMENT BLOCK.
