Miner was negotiating stairs to perform maintenance on a screen. When they turned to face screen, their foot slipped off of the stair tread causing them to trip down the remaining two steps and sit down on top of their right foot/ankle. Miner was evaluated for right ankle strain.

MINER WAS STRUCK IN THE LOWER BACK BY THE OVERFLOW CHUTE OF A PORTABLE CRUSHER AFTER IT SHIFTED WHILE BEING STOWED ON THE TRAILER FOR TRANSPORT. THE IMPACT KNOCKED MINER TO THE GROUND.

Employee was taking down safety rails on screen plant while two other employees were holding rail. Rail slipped and hit employees hand, fracturing right hand.

Fall from machine
The worker was climbing down in a proper 3 pt contact from the flat bet trailer. The heel of his boot caught on a short stake, in a stake pocket, he lost his balance and fell to the ground. He landed on his right elbow, chipping the bone. The trailer is 58"" off the ground. The worker had been wearing fall restraint just prior to the fall.
