Caught in, under or between a moving and a stationary object

Miner entered HP400 cone crusher after performing lock out/tag out to clear a large rock stuck between cone and mantel. Miner failed to recognize the stored energy in the cone being suspended by the rock. When dislodging the rock, the weight of the cone came down trapping both feet between the cone and mantel.

Struck by falling object

TEAM MEMBER WAS LIFTING INSPECTION DOOR TO VSI. WHILE MOVING TO GET A BETTER LEVERAGE ON THE DOOR TO USE TWO HANDS, THE DOOR SLIPPED AND LANDED ON TEAM MEMBER'S LEFT THUMB. THE EVALUATION BY THE HAND SPECIALIST INDICATES NERVE DAMAGE AND BONE IS OFFSET AND WILL NEED SURGERY.

Team Member removed the mantel nut on an HP-400 Cone Crusher while performing regular maintenance. Team member lost grip of the mantel nut while setting it to the side causing there right hand / wrist to be pinch between the cone liner and mantel nut.

Employee wandered away from job duties, past a clearly marked ""Do Not Enter"" sign and fell into crusher feeder. He broke his left femur and left wrist.

Employee was operating a manlift. A loader backed into the manlift at the operator's basket. The employee received broken bones to his left foot.

While getting off of D-10 Dozer, slipped and fell. Cut the base of right thumb requiring stitches. Restricted work - no use of right hand for 1 week.
