Miner was experiencing shortness of breath and chest pains while operating loader. After notifying tower operator of symptoms, miner drove themselves to hospital where they were admitted and treated for a heart attack resulting from 95% blockage in one of the main arteries related to personal medical condition.

WORKER WAS TRAVELING TOO FAST DOWN AN INCLINE. HE ATTEMPTED TO MAKE AN ""S"" TURN AND ROLLED THE BOX OF HIS TRUCK 360' AND LAY THE CAB ON ITS SIDE. HIS THIGH HIT THE STEERING WHEEL BRUISE IT.

WHILE IN THE PROCESS OF PICKING A CONTROL TOWER OFF OF A STAND, THE WORKER GOT BUMPED WHILE ON THE ROOF OF THE TOWER, LOST BALANCE AND FELL 3' TO A VAN ROOF STRIKING A RAILING ON THE WAY DOWN. LANDED ON HIS RIGHT HIP AND BRUISING IT.
