Employee went to urgent care after flushing eye and still feeling like something was in it, employee was told nothing is in there and there were some scratches. Employee returned to work next day.

Employee was lifting scale parts off a pallet when EE's left thumb got stuck between two parts. EE went to the ER and hand specialist. EE has a break in EE's thumb.
Laborer was hooking up lift cable to lift a crusher head to a loader bucket tooth and pinched EE's right thumb between the cable and tooth. This cut EE's finger and ripped off EE's thumb nail. Hospital took X-Rays and pulled the nail and glue up the cut.
Hearing test exceeded the 25 Theshold
Employee was driving company unit L-27 mini loader when the right window swung open, then swung back and shattered causing glass to go all over employee causing minor cuts on right arm and hand. On 8/7/25 EE noticed EE's right hand was swelling up. EE reported the accident on 8/8/25, and went to urgent care. They put EE on antibiotics and gave no restrictions to return to work.
Employee was walking up steps to get to 5-1/2' cone crusher when EE tripped on the 5th step and fell hurting EE's left leg above EE's ankle. EE was brought to the emergency room by EE's supervisor and put in a walking boot.
Employee was lifting buckets of aggregate when EE felt EE's knee buckle. Sought treatment and was diagnosed with a left knee MCL sprain.
The employee was using a shovel to try and get a rock out of the #1 conveyor belt scraper when EE hit hand on the conveyor. The next day the hand started swelling up so EE went to see a doctor. They told EE EE fractured the thumb and put EE on restricted duty for two weeks.
Employee was changing a screen panel on the #2 screen, while picking up the screen panel, EE was cut by a sharp piece of metal on EE's right hand middle finger.
Coveralls got caught and tripped the person involved causing the person to fall and puncture hand when falling to the ground.
Employee was trying to turn a butterfly valve when they lost footing and fell. They felt pain in their upper back.

Employee was looking at an air chuck quick connect that had broken, when EE felt something possibly dirt in the right eye. Brought to urgent care after trying to flush eye on site. Doctor didn't find anything, but prescribed eye drops due to possible scratch in eye since employee still felt irritation in eye.
Employee was working on the #1 conveyor removing a pulley with a pry bar and the pry bar hit them on the left side of the face causing a facial contusion.
Employee was walking with a shovel to clear snow from a storm and slipped on ice. EE hit their head on the ground when they fell.
Employee wasn't paying attention to where EE was going and walked into a guard with their face. EE has damage to the teeth and they're checking EE's head and shoulder for other injuries.
Employee notified yard foreman on 01/12/2024, that on 01/04/2024, EE was digging a rock out of a cone crusher, when EE hurt EE's hip or back. EE made an appointment with a doctor for 01/17/2024. The doctor prescribed EE medication, and gave EE a note on 01/18/2024 that EE was able to return to work regular duty.
Climbing up on machine to check Oils when getting down felt pain in left Buttocks and hip and upper leg.
Employee was descending down into the Jaw Crusher to stand on a platform placed to disassemble locking wedge and jaw die when EE twisted Right ankle.
Wood got stuck in the tail pulley. Employee used a piece of pipe to hit it out. Swung pipe, Clipped their finger and tore the fingernail.
Employee was using a razor knife to cut drive belts that were hanging from HP400 Crusher when employee cut self with the knife on left arm.
Employee was exiting Hyundai HX300 Company #E-8 excavator. When EE went to come down off the track, EE felt the ball of EE's left foot become sore. EE went to the emergency room to get checked, and they said EE has a possible fracture and needs to see an orthopedic doctor.
Employee was using cutting torch and grinder to repair a rock crusher main frame liner, when a piece of metal went up under the face shield and safety glasses into EE's eye.
The miner was grinding a steel plate when the grinding wheel broke apart. The piece that came off cut their right leg requiring going to the emergency room for stitches.

The employee was moving/relocating a portable LT130 Metso jaw crusher. The employee was wearing a hard hat. A rock fell off of the top of the machine around the feeder area, which they saw the shadow of and looked up and the rock struck them in the head under hard hat.
Moving rocks out of the jaw, a rock slipped and jammed EE's right index finger.
Employee was cleaning the windows on loader and slipped and fell from the machine.
The employee was climbing onto a loader to begin shift. A knee banged into the second step.

The miner was working on replacing a tramp release cylinder on a cone crusher, and went to use a hammer to remove a pin. The hammer slipped and hit EE's middle finger causing a cut.
Employee was lifting an air compressor off a pick up truck when employee slipped and fell. Employee's back hit the side of the truck and the compressor landed on employee's right leg.
While shoveling felt a pop in right shoulder.
While doing plumbing work, the employee caught left hand thumb between a pipe wrench and a jaw wrench. This caused the finger nail on EE's thumb to tear, EE went to the hospital where they gave EE stitches to repair it.
Rock fell off a conveyor striking the employee in the back
While operating an end dump, the employee started experiencing soreness in the left knee, as the day went along it got worse.
While adjusting a rubber curtain with a razor knife the employee cut EE's index finger
Excavator operator opened the door to the cab. Operator was unable to close it, due to the way the machine was facing. Operator decided to spin the machine and the door slammed shut on left elbow.
Cleaning snow off of # 9 conveyor belt slip on a patch of ice on grated cat walk started to fall and caught the hand rail under the left arm pit and damaged left shoulder.
While removing screen from screening plant, screen dropped on EE's hand.
Employee got out of truck, while walking to office stepped on a stone and twisted right knee. Employee refused treatment until 10/30/19.
I was walking through some brush to take a water sample and a branch/plant brushed my eye.
