Employee was looking at an air chuck quick connect that had broken, when EE felt something possibly dirt in the right eye. Brought to urgent care after trying to flush eye on site. Doctor didn't find anything, but prescribed eye drops due to possible scratch in eye since employee still felt irritation in eye.

Employee was lifting an air compressor off a pick up truck when employee slipped and fell. Employee's back hit the side of the truck and the compressor landed on employee's right leg.

Employee got out of truck, while walking to office stepped on a stone and twisted right knee. Employee refused treatment until 10/30/19.

Employee was descending a wet plant stairway when the employee slipped and lost footing. Employee leaned back and wrenched shoulders trying to prevent self from falling.

Struck against a moving object

EE was backing a 35 ton Terex haul truck up to the hopper at the rock plant. EE was leaning out the window looking to the rear when the left front tire exploded. The force of the blast blew EE back into the cab and lifted EE off the seat. EE injured EE's neck, arm and left side and may have hit EE's head.

Employee was climbing off a Cat D8 that was in the shop for repair. When ee stepped off the machine, ee rolled ankle and had a hard landing on the shop floor. The following day ee went to the doctor's to have it examined. EE has a broken fibula and torn ligaments.

Employee was on a walkway on the crushing plant. EE ducked down to walk under a conveyor, and when ee stood up ee felt a ""pop"" in knee. It didn't bother ee until ee got home, when ee felt pain when trying to bend leg.

Mechanic had just completed brake job on company 3/4 ton GM van. He had removed the jack stands from the rear axle. He had the hydraulic jack under the center of the axle. He was lying on a creeper and turned the handle to release the jack. The jack came down fast and the handle smashed his wrist onto the shop floor.

The employee was pulling on a lever to release the fifth wheel and lost his footing. He slipped and bashed his chest on the fender of the tractor, knocking the wind out of himself and tearing cartilage from a rib.
