Laceration to left thumb while operating a power tool

Caught in, under or between a moving and a stationary object

Laborer was hooking up lift cable to lift a crusher head to a loader bucket tooth and pinched EE's right thumb between the cable and tooth. This cut EE's finger and ripped off EE's thumb nail. Hospital took X-Rays and pulled the nail and glue up the cut.

Cleaning snow off of # 9 conveyor belt slip on a patch of ice on grated cat walk started to fall and caught the hand rail under the left arm pit and damaged left shoulder.

Individual was breaking up frozen material and ice while sitting in a bin when a piece broke free and landed across EE's knee bruising the knee and ligaments.

Climbing down front Ladder of Haul Truck #64 at coffee break and missed the bottom step Fell to the ground on left side Hip. Could not put pressure on it was taken by ambulance to the Hospital to be examined.

Replacing screen on Horizontal screen deck had climbed inside to position a screen when EE felt a discomfort in left shoulder. That evening the pain became intolerable so EE went to a medical facility.

A piece of angle was on cat walk and was moved when person came down Ladder and went to step on the deck EE stepped on the angle which rolled under EE and twisted the Right ankle causing a severe Sprain

A conveyor belt needed to be trimmed with a razor knife. So an assistant went over to trim the belt and drew the knife toward his hand

Employee was tossing scrap metal into steel dumpster when the wind blew back towards him and a speck of rust got around his glasses and entered into his right eye.

Replacing a screen on a screen deck tightening bolt with breaker bar. Felt right shoulder pop. Had no strength in arm after.

Operator pulled up to office trailer to unload his personnel objects & bin controllers when he climb out to ladder and started down was probably about 4 feet from ground he lost his grip when shoe got stuck on step tread-and fell backwards, he stated that he was only using a couple of finger to hold on with items in hand.

Recycling concrete, a piece of rebar had stuck on side of conveyor opposite side of cat walk. He climbed up to remove two pieces of rebar. Removed last one when he went to grab 2nd one, work glove pinky finger touched roller and pulled his hand in squeezing left ring finger between squirt board bolt.

Installing a skirt board rubber on #2 feeder, had to trim to fit using a utility knife, slipped and hit left leg on inner calf of leg. 1"" cut, 1"" deep, 5 stitches.

He was helping another person to remove a clamping cylinder from 7' symous crusher, when he was disconnecting cylinder, he felt his back go out.

Clearing a block shoot with pry bar. Poked it several times. When bar went through he hit finger on super structure and cut left hand ring finger requiring 4 stitches.
