Laceration to left thumb while operating a power tool

Laborer was hooking up lift cable to lift a crusher head to a loader bucket tooth and pinched EE's right thumb between the cable and tooth. This cut EE's finger and ripped off EE's thumb nail. Hospital took X-Rays and pulled the nail and glue up the cut.

Hearing test exceeded the 25 Theshold

Coveralls got caught and tripped the person involved causing the person to fall and puncture hand when falling to the ground.

Employee was cutting a hose with a utility knife and it ran across EE's left leg above the knee and required 4 stitches.

Climbing up on machine to check Oils when getting down felt pain in left Buttocks and hip and upper leg.

Employee was descending down into the Jaw Crusher to stand on a platform placed to disassemble locking wedge and jaw die when EE twisted Right ankle.

Injured person was cleaning/Grinding Particle entered left eye behind safety glasses

Cleaning snow off of # 9 conveyor belt slip on a patch of ice on grated cat walk started to fall and caught the hand rail under the left arm pit and damaged left shoulder.

person was trying to remove large piece of wood from the # 1 conveyor where they had to reach and turned to throw it off the belt and catwalk when they injured their back.

Individual was breaking up frozen material and ice while sitting in a bin when a piece broke free and landed across EE's knee bruising the knee and ligaments.

Climbing down front Ladder of Haul Truck #64 at coffee break and missed the bottom step Fell to the ground on left side Hip. Could not put pressure on it was taken by ambulance to the Hospital to be examined.

Replacing screen on Horizontal screen deck had climbed inside to position a screen when EE felt a discomfort in left shoulder. That evening the pain became intolerable so EE went to a medical facility.

Was grinding with a portable grinder and had a piece of debris enter eye under safety glasses.

A piece of angle was on cat walk and was moved when person came down Ladder and went to step on the deck EE stepped on the angle which rolled under EE and twisted the Right ankle causing a severe Sprain

Retest of hearing after initial test showed: Failed.

A conveyor belt needed to be trimmed with a razor knife. So an assistant went over to trim the belt and drew the knife toward his hand

Employee was tossing scrap metal into steel dumpster when the wind blew back towards him and a speck of rust got around his glasses and entered into his right eye.

Was assisting the foreman service equipment. Climbing down from machine lost grip and landed on right knee.

Replacing a screen on a screen deck tightening bolt with breaker bar. Felt right shoulder pop. Had no strength in arm after.

Operator pulled up to office trailer to unload his personnel objects & bin controllers when he climb out to ladder and started down was probably about 4 feet from ground he lost his grip when shoe got stuck on step tread-and fell backwards, he stated that he was only using a couple of finger to hold on with items in hand.

Recycling concrete, a piece of rebar had stuck on side of conveyor opposite side of cat walk. He climbed up to remove two pieces of rebar. Removed last one when he went to grab 2nd one, work glove pinky finger touched roller and pulled his hand in squeezing left ring finger between squirt board bolt.

Removing a steel bar from rack. Felt a pain in groin.

Trying to remove a piece of concrete off the conveyor belt, sprained back.

Welding on a feeder in tight location, did not pull welding shield down fast enough. Received a welding flash.

Employee was removing engine from haul truck, removing various components, when he felt a pull in left shoulder. Has appointment to see doctor on 12-4-09.

Installing a skirt board rubber on #2 feeder, had to trim to fit using a utility knife, slipped and hit left leg on inner calf of leg. 1"" cut, 1"" deep, 5 stitches.

He was helping another person to remove a clamping cylinder from 7' symous crusher, when he was disconnecting cylinder, he felt his back go out.

Clearing a block shoot with pry bar. Poked it several times. When bar went through he hit finger on super structure and cut left hand ring finger requiring 4 stitches.

Was attempting to discard a piece of steel into a scrap dumpster. Started to slip and twisted shoulder. Did not fall. Walk had been previously sanded but was raining that morning so there was a glaze on service again.

The parking area was icy and employee slipped when getting out of his car.

Employee slipped on stairs from office trailer. He fell onto his right arm and tore his rotator cuff.

Cleaning under jaw on portable plant, hit elbow on plant, hand became swollen after (2) hours. Had x-ray, cracked bone in elbow.

SLipped and fell from loader.

CHANGING STEEL ON 7' CRUSHER, PULLING ON CROW BAR, WHEN HE FELT A POP & PULL AT SAME TIME IN STOMACH AREA.

GETTING A PIECE OF STEEL TO CUT UP WHEN A PIECE (250 LBS) FELL OFF OF LOWER SHELF INJURING TOP OF LEFT FOOT.

DIGGING A TRENCH TO LAY PIPE, CLIMBING OUT OF TRENCH WHEN HE SLIPPED AND FELL AGAINST TRENCH WALL AND HURT ARM/SHOULDER (LEFT).

EMPLOYEE RUNNING FRONT END LOADER, HIT BUMP IN ROAD CAUSING A LOWER BACK ACHE.

WORKING WITH HP400 BLOCKING WHEN SPLINTER FROM WOOD ENTERED RIGHT HAND.

ROLLING OUT BELT RUBBER (WHILE IN WET BIN) BELT WAS ROLLED IN. EE PU LEFT HAND UP TO BLOCK AND CAUGHT HIS LEFT THUMB, BENT IT BACK.
