Employee was working under machine while servicing a loader CO UNIT# L-27 when dust fell into their right eye. Employee went to urgent care after flushing eye and still feeling like something was in it, employee was told nothing is in there and there were some scratches. Employee returned to work next day.

Employee was lifting scale parts off a pallet when EE's left thumb got stuck between two parts. EE went to the ER and hand specialist. EE has a break in EE's thumb.
Employee was driving company unit L-27 mini loader when the right window swung open, then swung back and shattered causing glass to go all over employee causing minor cuts on right arm and hand. On 8/7/25 EE noticed EE's right hand was swelling up. EE reported the accident on 8/8/25, and went to urgent care. They put EE on antibiotics and gave no restrictions to return to work.
Employee was walking up steps to get to 5-1/2' cone crusher when EE tripped on the 5th step and fell hurting EE's left leg above EE's ankle. EE was brought to the emergency room by EE's supervisor and put in a walking boot.
Employee was trying to turn a butterfly valve when they lost footing and fell. They felt pain in their upper back.

Employee was using water wash hose to clean under crusher when the water sprayed back at the employee, causing something to go into right eye. Employee was wearing safety glasses.
Employee was working on the #1 conveyor removing a pulley with a pry bar and the pry bar hit them on the left side of the face causing a facial contusion.
Employee was walking with a shovel to clear snow from a storm and slipped on ice. EE hit their head on the ground when they fell.

Employee was exiting Hyundai HX300 Company #E-8 excavator. When EE went to come down off the track, EE felt the ball of EE's left foot become sore. EE went to the emergency room to get checked, and they said EE has a possible fracture and needs to see an orthopedic doctor.
Employee was using cutting torch and grinder to repair a rock crusher main frame liner, when a piece of metal went up under the face shield and safety glasses into EE's eye.
Employee was climbing down off of loader when EE slipped and fell off the loader steps and felt pain in lower back, hip and hamstring.

The miner was working on replacing a tramp release cylinder on a cone crusher, and went to use a hammer to remove a pin. The hammer slipped and hit EE's middle finger causing a cut.
While shoveling felt a pop in right shoulder.

While working with another employee to install a return roller, the roller slipped out of ee hands and hit him in the nose.
Employee noticed eye irritation but thought it was an eyelash in eye. Never reported this to anyone at the time. Called supervisor at home at 7:00 pm to tell him he thought he had metal in eye and was going to the emergency room.
One employee was on a ladder (wearing fall protection gear) when the ladder slipped out from underneath him, due to ice. The injured employee attempting to help, put the ladder back and proceeded to climb the ladder to get the hanging employee back on. The ladder slipped out a second time, and he was injured when he fell.

Employee began to feel back pain while driving haul truck around 10 a.m. He continued to drive and completed his shift leaving at 5 p.m. He called in at 6 a.m. the following day, said ""he could not move and was going to see his doctor."" This man has not returned to work as of this date. Will update when more information is available.
