An employee arrived early to work and waited outside the closed gate. Around 5:30 AM the foreman arrived with the gate key. The employee got out and assisted the foreman on opening the gate. On the way back to their personal vehicle, the employee slipped on the gravel and fell onto the asphalt, breaking their right hip. The employee was not clocked in and not wearing work boots.

Employee stated he was okay, but was taken for medical exam. On 8-9-11 operator went to Dr. and was taken off work duty for two weeks.

The ground man was attempting to dislodge rock with a 5' pry bar. When the rock disloged, employee was unable to stop momentum of the bar and struck his left hand pinky finger. Employee was taken to emergency room for x-ray, which revealed a small-angulated fracture on the little finger of his left hand. Employee returned to work without restrictions.
