Fall onto or against objects

An employee lost their footing while installing an access door on a chute. While attempting to prevent a fall, the employee used their left arm to brace themselves while still holding an impact wrench. The resulting force caused a fracture to the employee's left elbow.

While walking to a portable restroom in an outside parking area, the employee slipped and hit their head on the restroom door, causing neck pain and stiffness.

While attempting to break free a can on a C-17 conveyor, the employee's left middle finger was stuck by a hammer.

While removing a bolt with a 3/8"" ratchet, to remove the transmission on an F-750 service truck, the employee felt a sharp pain in the left wrist.

ON 1-6-17 AROUND 7:00 AM, IT WAS SNOWING AND THE ROADS GOT SLICK AND 2 DUMP TRUCKS SLID OFF THE ROAD INTO THE DITCH AND HAD ROAD BLOCKED AT FRONT ENTRANCE, EMPLOYEE WAS GOING DOWN THE ENTRANCE ROAD, APPLIED BRAKES AND STARTED SLIDING AND COULDN'T STOP. LEFT FRONT TIRE HIT THE REAR TIRE ON DUMP TRUCK, THEN LEFT MIRROR HIT THE BED AND BROKE IT OFF. STEERING DAMAGED.

was pulling up the guard with his right hand and reaching across his right arm to put a bolt in, something popped in his right shoulder, started to hurt. went to doctor to get checked out, DR. said looked like strained tendon or tear in the tendon. DR.wants to see patient again. scripts Norco for the pain at night and Etodolac for inflammatory.

Employee was cleaning out a clogged chute with a water hose when material dislodged from chute wall striking him in the hard hat which resulted in his face impacting the chute access door.

Employee was shoveling material from under the surge tunnel conveyor when he felt pain in his arm and hand. *The reason for the late reporting is the doctor has been evaluating this injury to see if it was new or a reoccurence of an injury to the same area in the previous year(2009).

Employee is not sure how he hurt his wrist. He did not come in contact with anything nor did he strain it in anyway. He thinks he may have hurt it during March 2009 while washing and cleaning the chutes at the primary. According to the management team, no one was aware of this incident until July 2009.

EE ALLEGES INJ OCCURRED ON 12/29/04 BUT DID NOT REPORT TO MGMT UNTIL 1/1/05. EE STATED HE STRAINED RIGHT ELBOW WHILE BEATING ON A CHUTE WITH A HAMMER TO DISLODGE SOME MATERIAL.

EE did not report this accident until 10-6-04 that he injured his lower back while washing out under pugmill scales. EE stated he needed medical attention. It was determined that he had strained a muscle in his lower back. He was released on restricted duties to return to work.
