The employee was tied off, standing on angle iron, cutting bolts out of a wash screen with a torch. As they turned to step down, employee felt a pop in their right knee. Diagnosed with a right knee strain.

While loading a rib line hole using an anfo hose, the employee felt a sharp pain in their upper back/neck area.

Rock fell from brow. Employee sustained injuries to head, left arm and left shoulder.

On 08/19/2019 employee sustained a slightly sprained ankle during normal work maintenance. On 09/27/2019 the employee experience sharp pain in the ankle and reported to EE's supervisor. An x-ray revealed a slight fracture making the incident reportable on 09/27/2019.

Over-exertion in lifting objects

On 3/20/18, employee strained their back while lifting 40 / 50 lb. bags of ANFO. EE reported the strain on 3/21/18 but choose not to seek medical treatment until 3/28/18 at which time it was deemed reportable. This is being reported within the 10-day reporting requirement of the injury being deemed reportable.

Employee leaned over ticket printer, located on platform, and fell to the ground. Employee had a partially collapsed lung, 5 broken ribs, cracked vertebrae.

Emp allegedly reached with left hand to grasp handle to pull himself into drill cab, felt pain in left shoulder, pain continued down into left side. Upon arising Fri AM noticed bulge above naval area with pain in abdominal area & left shoulder.

EE WAS PUTTING PANEL COVER ON MOTOR COMPARTMENTWHILE STANDING ON LOADER TIRE. HE SLIPPED, FELL &INJURED HIS BACK
