EE was stepping down from conveyor, when left foot stepped on top of a rock. As EE stepped backwards, the rock pushed from underneath the left foot. This resulted in the EE's left knee to twist and causing the the EE to fall to the ground.

Hurt back.

Employee putting up Spray bar when rock bounced off conveyor and landed on left hand, pushing it against the hopper.

Worker was climbing down ladder and slipped. Fell and landed on scrotum. Ruptured testicle.

Employee bent down to check cord. When attempting to stand employee could not fully extend leg or walk on leg.

Removing oil drain plug with wrench. Heard pop in elbow. Didn't learn employee would need surgery until later. Surgery occurred 08/10/2016.

Removing concrete and rebar. Tripped and fell on shoulder.

Employee and coworker attempting to pry lock ring out of cone to replace liner. Employee got finger caught in between cone and lock ring.

Employee fell while unloading and was injured.

Rolled ankle walking to plant.

Undermining high wall in pit. Overhanging material broke loose.

Workers attempting to remove bearing. Came off pinching one workers left pinky finger between bearing and frame of chain feeder.

Crusher stopped working. Employee ran to stop plant. Ran and ducked under conveyor, lost balance, hard hat was knocked off and he cut head on inside of frame of conveyor belt. Stitches received.
