Ladder twisted & fell; Low back & left hand sprain

Pry bar contacted right forearm of employee.

EE slipped on ice, falling backwards and hitting the back of his head on a metal I-beam, required stitches.

Climbing down steps, foot slipped & sprained right ankle.

Employee was coming down ladder and foot slipped causing his toe to get hung up on next rung causing him to fall back. hanging by his foot.

He was standing on the ground, prying frozen chunks out of the box, when one chunk of frozen dirt hit him on his left ankle.
