Employee was working on tail pulley of General Steel Screen Plant Cross Conveyor. Employee cut right hand ring finger resulting in 5 sutures.

Employee was checking drive belt tension on jaw screen 13-367 (Plant was shut down). Employee pulled down on drive belt and pinched right middle and ring finger. Plant was locked out. **Lost tips of middle and ring fingers**

EE WAS REBOLTING BELLY PAN ON DOZER. THE PAN WAS SECURED WITH A COME ALONG. EE MOVED PAN TO ALIGN BOLTS AND PAN SLIPPED AND FELL ON FINGER.
