Strain to lower back.

Employee was working on tail pulley of General Steel Screen Plant Cross Conveyor. Employee cut right hand ring finger resulting in 5 sutures.

Employee got left thumb under cone, causing laceration to left thumb. Received stitches.

Very windy day and a object flew into EE's eye, EE was wearing his safety glasses.

Employee was checking drive belt tension on jaw screen 13-367 (Plant was shut down). Employee pulled down on drive belt and pinched right middle and ring finger. Plant was locked out. **Lost tips of middle and ring fingers**

Employee tried to dislodge rock from conveyor using pry bar. The bar pinched his hand between it and the frame.

EE WAS REBOLTING BELLY PAN ON DOZER. THE PAN WAS SECURED WITH A COME ALONG. EE MOVED PAN TO ALIGN BOLTS AND PAN SLIPPED AND FELL ON FINGER.
