On 6/17/25 employee provided a modified duty work status report from their personal doctor dated 6/12/25. On 4/25/25 employee reported a first aid from pain to their right arm while placing a wear liner into position. Employee self procured medical treatment, was diagnosed with a ruptured bicep, and restricted duty is being accommodated.

Employee was getting on the loader and missed a step, falling backwards on their right arm/wrist.

Worker was cleaning plant when they pinched their right 4th and 5th fingers in between bobcat bucket and hydraulic ram causing laceration of the 4th finger and fracture of 5th finger.

Employee fell off ladder and injured finger

Worker was sitting on the side step of the loader waiting for fuel when worker slipped off and injured shoulder.

Employee slipped and fell climbing from ladder to the cone crusher, injuring lower back.

Employees were changing 8'x 24'screen. The counter weight was being heated and employee stepped forward to give instruction when the counter weight came loose and struck employee in the right leg.

Employee was drilling a hole in conveyor belts to be used on a screen. The drill bit bound up while drilling and caused the drill to twist suddenly injuring the employee's wrist. Employee did not report the injury until 07/16/2019.

Employee went to step up on ladder on 769 Cat Rock Truck with right foot and strained left calf.

Employee was being trained on a Rock Drill when one of the steel drill rods on the rotating carousel became loose from the top part of the carousel star. While employee was applying pressure to the drill rod trying to push it back in place, EE smashed finger.

Employee missed the last step getting down of the loader and fell back hitting his head on the ground.

A scheduled blast at the quarry sent clay material north into the air damaging two homes. A hole was made in a garage wall in one home & a window broke in a second home. It appears that a clay seam occurred in the rock to be blasted. The clay seam went undetected. The blast blew out the side of the wall because of the presence of the clay sea, sending particles into the air.

Employee was walking at the plant close to the 3" minus rock pile when he stepped on a rock and twisted his left knee.

Employee was cleaning C#5 belt of 2 inch minuses rock when he twisted his lower back.

While standing on a 4 x 12 board between two metal conveyor supports employee was guiding forklift operator to raise the gravity take-up on the belt to install a new belt. A fork got caught on a 3 piece of angle iron and the pressure on the fork became too great causing the angle iron to bend, and the fork to strike employee in the neck, face, and side of head.

Employee strained his back due to being bent over while welding.

The employee was changing screens on the shaker when his glove got hung up on one of the screen hooks, it pulled his hand down on a metal plate and cut his left fourth finger.

Employee was lying on his back changing screens on a shaker and the vibration of the movement caused a toolbelt and small hammer to fall and strike the employee in the head.

Employee was changing the skid steer from loader bucket to forks and was using a cheater pipe to assist him. The pipe slipped and employee smashed his fifth finger on his right hand.

While doing repair work on a conveyor, the cutting torch slipped from the worker's hand. The torch landed on top of his work boot and burned him.

Pressure had not been released from line, employee was loosening four bolts on hydraulic line when oil under high pressure sprayed him in both eyes and forehead.

Rather than walk down steps, employee jumped off landing about 2 feet onto ground. When he landed he broke his leg.

Employee was kneeling down cutting a conveyor belt with a knife. THe knife slipped and the employee cut his left thigh 2" deep above the knee.

Employee was removing electrical box from wall and it slipped striking the employee in the right knee.

Employee was working on a screen chute, he slipped and fell landing with his arm extended and dislocated his left shoulder.

Employee put a second ear plug in same ear, causing the first plug to be jammed in ear. He also complained of heel pain from standing too long

Worker was cutton on an uneven surface, he slipped and the torch crossed over his gloved hand and as a result he received a burn to his hand.

Employee was lifing and moving a water pump by hand up a steep grade approximately 100 feet to the service truck.

OPERATING HAMMER DRILL, DRILLING ON A CONCRETE CULBERT, EE WAS IN A SIDE WAYS POSITION WHEN HE FELT A SHARP PAIN IN HIS RIGHT GROIN AREA AS WELLAS HIS BACK ON THE RIGHT SIDE.

HE WAS CLIMBING LADDER AND HIT KNEE ON STEP.

THE WORKER WAS CLEARING G-CONE FROM A LOADER TOOTH BY STRIKING IT WITH A HAMMER. A PIECE OF METAL BROKE OFF AND ENTERED HIS THIGH.

RASH ON RT. AND LT. ARMS CAUSED BY CONTACT WITH POISON OAK WHILE CLEARING BRUSH.

REOCCURING FLAIR UP - ORIG. REPORTED ON 12-30-88, MSHA ID 0404443 - MOBILE SAND.

WHILE INSTALLING EXCAVATOR BUCKET, PIN SLIPPED AND THE EE TWISTED HIS BACK.

WHILE DOING PRODUCTION THE CRUSHER BECAME PLUGGED. IN THE PROCESS OF CLEANING THE CRUSHER EE SLIPPED OFF CRUSHER AND FELL TO THE DECK. THE DISTANCE IS APPROX. 40". WHILE FALLING HIS LEG WAS NO T IN THE PROPER POSITION WHEN HE LANDED ON HIS FEET.

REMOVING BASKET OFF CLEMRO CRUSHER, EMPLOYEE'S HAND GOT CAUGHT IN A PINCH POINT BETWEEN BASKET AND MOTOR GUARD.

WHILE TRYING TO LOOSEN ROCK FROM PULLY, HE INJURED HIS LEFT 5TH FINGER CUTTING THE FINGER AND NAIL.

CONVEYER BELT UNDER BAXTER JAW BECAME OVERLOADED, WHILE EMPLOYEE WAS CLEARING OFF THE BELT FROM ROCK, A DISLODGED AND CRUSHED TWO FINGERS BETWEEN ROCK AND STEEL BAR.

OPERATOR HAS A BUNGY CORD ATTACHED TO CB RADIO MIKE TO KEEP IT FROM SWINGING INSIDE CAB OF LOADER. WHEN HE PULLED THE CORD TO TALK IT CAME LOOSE AND STRUCK HIM IN THE FACE BREAKING HIS GLASSES AND GETTING GLASS IN HIS EYE.

THE EE WAS ATTEMPTING TO LOOSEN SANDSTONE FROM A CONVEYOR WHEN HIS KNEE BUCKLED.

THE OPERATOR WAS WATERING HAUL ROAD, STOPPED AT TOP OF HILL & STARTED TO BACK UP; IT WAS TOO WET AND HE LOST AIR PRESSURE, GOT OUT OF GEAR & DROVE TRUCK INTO BANK. WATER TRUCK STOPPED, THEN W ATER IN TANK SHIFTED FORWARD, CAUSING THE TRUCK TO ROLL ON SIDE.

SMASHED BIG TOE BETWEEN ROTOR AND CHUTE OF TORGESON.
