Through an industrial audiogram exam, it was determined the employee sustained reportable hearing loss due to industrial noise exposure.

Worker was sitting on the side step of the loader waiting for fuel when worker slipped off and injured shoulder.

Employees were changing 8'x 24'screen. The counter weight was being heated and employee stepped forward to give instruction when the counter weight came loose and struck employee in the right leg.

Employee was drilling a hole in conveyor belts to be used on a screen. The drill bit bound up while drilling and caused the drill to twist suddenly injuring the employee's wrist. Employee did not report the injury until 07/16/2019.

Employee missed the last step getting down of the loader and fell back hitting his head on the ground.

A scheduled blast at the quarry sent clay material north into the air damaging two homes. A hole was made in a garage wall in one home & a window broke in a second home. It appears that a clay seam occurred in the rock to be blasted. The clay seam went undetected. The blast blew out the side of the wall because of the presence of the clay sea, sending particles into the air.

Employee was cleaning C#5 belt of 2 inch minuses rock when he twisted his lower back.

Employee strained his back due to being bent over while welding.

Pressure had not been released from line, employee was loosening four bolts on hydraulic line when oil under high pressure sprayed him in both eyes and forehead.

Employee was working on a screen chute, he slipped and fell landing with his arm extended and dislocated his left shoulder.

Employee put a second ear plug in same ear, causing the first plug to be jammed in ear. He also complained of heel pain from standing too long

Worker was cutton on an uneven surface, he slipped and the torch crossed over his gloved hand and as a result he received a burn to his hand.

OPERATING HAMMER DRILL, DRILLING ON A CONCRETE CULBERT, EE WAS IN A SIDE WAYS POSITION WHEN HE FELT A SHARP PAIN IN HIS RIGHT GROIN AREA AS WELLAS HIS BACK ON THE RIGHT SIDE.

WHILE TRYING TO LOOSEN ROCK FROM PULLY, HE INJURED HIS LEFT 5TH FINGER CUTTING THE FINGER AND NAIL.

OPERATOR HAS A BUNGY CORD ATTACHED TO CB RADIO MIKE TO KEEP IT FROM SWINGING INSIDE CAB OF LOADER. WHEN HE PULLED THE CORD TO TALK IT CAME LOOSE AND STRUCK HIM IN THE FACE BREAKING HIS GLASSES AND GETTING GLASS IN HIS EYE.
