Employees were changing 8'x 24'screen. The counter weight was being heated and employee stepped forward to give instruction when the counter weight came loose and struck employee in the right leg.

Employee was drilling a hole in conveyor belts to be used on a screen. The drill bit bound up while drilling and caused the drill to twist suddenly injuring the employee's wrist. Employee did not report the injury until 07/16/2019.

A scheduled blast at the quarry sent clay material north into the air damaging two homes. A hole was made in a garage wall in one home & a window broke in a second home. It appears that a clay seam occurred in the rock to be blasted. The clay seam went undetected. The blast blew out the side of the wall because of the presence of the clay sea, sending particles into the air.

OPERATOR HAS A BUNGY CORD ATTACHED TO CB RADIO MIKE TO KEEP IT FROM SWINGING INSIDE CAB OF LOADER. WHEN HE PULLED THE CORD TO TALK IT CAME LOOSE AND STRUCK HIM IN THE FACE BREAKING HIS GLASSES AND GETTING GLASS IN HIS EYE.
