The employee was changing screens on the shaker when his glove got hung up on one of the screen hooks, it pulled his hand down on a metal plate and cut his left fourth finger.

EMPLOYEE WAS ASSITING FOREMAN IN PLACING A BEARING ON PLANT WHILE USING A CHISEL, SMALL METAL SHAVING CAME LOOSE CAUSING IT TO PENETRATE IN TO EMPLOYEE'S RIGHT CHEEK - NEAR NOSE BRIDGE.

EMPLOYEE WAS REPAIRING A CONVEYOR BELT. HE WAS PULLING ON ONE SIDE OF THE BELT WHEN IT RIPPED FROM THE OTHER END, AND HIT HIM CAUSING HIM TO FALL BACK.
