Employee was lying on his back changing screens on a shaker and the vibration of the movement caused a toolbelt and small hammer to fall and strike the employee in the head.
Employee was changing the skid steer from loader bucket to forks and was using a cheater pipe to assist him. The pipe slipped and employee smashed his fifth finger on his right hand.
While doing repair work on a conveyor, the cutting torch slipped from the worker's hand. The torch landed on top of his work boot and burned him.
