Employee was placing removable legs on a transfer conveyor. Conveyor was held in place by a loader. Removable leg fell and struck employee in the face, fatally injuring the employee.

EE was shoving top soil away from the highwall. He backed up over and through an 8 foot berm that had been constructed at the edge of the highwall. The dozer went over the wall rolling over and throwing him out of the cab.

Two miners working in an underground bin excavation were attempting to level the muckpile by blowing muck down a hole at the brow. They were both tied to retractable fall restraints when the muckpile slid down the repose angle unexpectedly. One employee manually pulled his line and engaged his restraint but the second man was engulfed and suffocated by the finely broken muck.

While performing a section repair in sidewall of a large earthmover tire the associate was discovered unconscious and unresponsive inside the tire by a mining company worker.

At approx 7:40pm, a haulage accident occurred on the surface area of mine. The EE was in the process of attaching a wire nip to a trolley pole of a ditch digger while apparently standing in front of the ditch digger. The ditch digger became energized and contacted the EE causing injury and subsequent amputation of the lower right extremity at the hip. EE was pronounced dead.

Co-worker was in the process of shutting down the generators, to do maintenance to the power cables. At that time employee started to unguard the power cables on the switch van out of the vision of co-worker. He refused to have the power checked by two other co-workers. Moments later he was electrocuted.

The investigation is not complete, but as of now it appears a miner was fatally injured when material that traveled through a borehole struck him after a blast was initiated. The victim was on the 1290 level ramp waiting for a blast to be initiated. The concussion from the explosion traveled through a 3-inch borehole, blowing small rock and debris onto the victim.

Drill crew was attempting to back off a stuck drill pipe. A wire rope snub line was used to secure the worm tong. The snub line failed and the tong struck the employee resulting in fatal injuries.

The employee received fatal injuries when he was struck by a rock that fell from the mine roof. He was involved with longwall shield recovery and was building wooden cribs in an area where a shield had been removed. While in the process of constructing the cribs, he was struck by falling roof material.

The victim was leaning over a base lift jack housing. The molded casing catastrophically failed striking the victim in the chest from unknown/undetectable reasons. Cause was catastrophic failure of the housing. Compliance with rules/regs, protective items not a factor, miner proficiency may be a factor, mining equip/systems may be a factor.

Victim was cleaning rocks and coal to set timbers along a rib line. The rib and brow fell crushing the miner. The brow measured approximately 100" long by 32" thick, by 37" tall.

Working on a structure 15' high & was safely tied off to structure, when he completed his work, instead of stepping onto platform from beam he was standing on while tied off - he untied and then stepped and fell 8' onto another 5 1/4" beam with his chest. He walked to the company truck and sat down ambulance arrived, got in and died in route to the hospital.

Had just finished up workplace exam and shimming the cone, done pre-shift inspection and had blown horn for start up and around 8:19 am. Co-worker saw EE lying on conveyor belt with his legs under the magnet. He immediately notified the crusher operator and he immediately shut off the operation.

EARLY MORNING AND THE SUN WASN'T COMPLETELY UP HOWEVER IT WAS LIGHT ENOUGH FOR GOOD VISIBILITY WITH OR WITHOUT LIGHTS BEING ON. BACK UP ALARM WAS FOUND TO BE IN GOOD WORKING CONDITION. The victim was struck by a road grader when it backed up. The accident occurred in the staging area where the victim and other coworkers were checking their equipment for the shift.

A scraper and a service truck collided head-on while traveling in opposite directions on a haul road. The impact ignited a fire to both equipment and resulted in fatal injuries to the service truck driver.
