A fatal accident occurred on the longwall face around #144 shield. A ladder line hose got pinched under the toe of the #144 shield. EE did not see hose under shield and initiated the electronic push of the pan-line. The hose that was pinched under the toe of the shield, stretched by movement of pan-line, broke apart and high pressure emulsion fluid struck victim as he traveled by.

Employee was operating a haul truck transporting material from the wash bins to a stockpile located Northwest of two settling ponds. On the return trip to the wash bins, the haul truck traveled through a berm on the left side of the haul road, went over an embankment and overturned on its left side in the settling pond.

Employee was shoveling coal into the Longwall pan line. A piece of coal fell from the face striking the employee. Employee was standing under shield #123 on the Longwall face at the time of the accident.

The miner was in progress of storing electrical cable into a cable tray. The south bracket some how came loose. When the south side of the cable tray came down it struck the ladder on the east side causing the ladder to fall towards the west. The miner tried to compensate for the fall by leaning to the west. He then fell from the ladder hitting the ground.

A shuttle car driver was tramming through a crosscut from Entry # 5 to Entry # 6. The car driver stopped spoke to the victim. The victim proceeded past the car, traveled through a curtain into the intersection of Entry # 6. The victim told the car driver he was clear. The car proceeded through the curtain and struck the victim between the shuttle car and the left rib.

The victim was operating a battery-powered, rubber tired, personnel carrier on the 2nd 48 supply road. As he traveled outby, for a presently undetermined reason, the golf cart veered suddenly to the left before overturning on top of him. Co-workers initiated CPR and continued to the surface. Attempts to revive him were unsuccessful and he was pronounced dead at 1:59pm.

While in the process of pulling top chain onto the longwall conveyor panline, the chain became fouled. While examining the area, an apparent failure of the sheave anchor point may have occurred allowing the rope and sheave to be propelled forward. An unknown portion of the assembly apparently struck the injured causing fatal injuries.

EE was cutting grass with 7300 compact track loader w/attached brush hog. For some unknown reason the machine lost traction and slid down small embankment into water. Cause of incident unknown at this time. Compliance with rules/reg not a factor, mining equip/system unknown to be a factor, but was used, PPE not a factor, job skills/miner proficiency/training/attitude not a factor.

Employee was fatally injured when he was struck by a battery powered coal hauler in Unit 2.It is believed that the employee walked through the last open crosscut to #4 entry and sat down near the inby corner of #4 entry to eat his lunch. A coal hauler rounded the corner and pinned the employee to the rib.

Miner was attempting to bring disabled truck down from the upper most harvest area. As he was dumping load, breaks let loose & truck went down uncontrollably. As it came to the next lower level, it hit a berm causing the miner to be ejected through the right hand window & was presumed to have been struck by a part of truck.

Employee walked up to the 7-2C header to inspect wipers. The victim was on an elevated catwalk at discharge of the 7-2B conveyor belt to examine the belt wiper. He fell off the end of catwalk on 7-2B belt. Employee was caught between the skirt and the top belt resulting in a fatal injury.

Employee was cleaning a jammed rock from a return roller on a track mounted CEC Crusher. The engine was still running and the belt was engaged. Employee "assumed" belt was turned off because the head pulley was not spinning skipping on the belt; the belt was actually stalled. He did not lock out/ and tag the crusher before performing the work to de-energize all power.

The individual was struck by an unknown object when replacing guide rope bushings on the north skip in the loading pocket at the bottom of the 16 foot shaft. The victim was working on top of a skip, performing maintenance with 2 coworkers, when a piece of salt fell and struck him.

The crew was attempting to align a section of pipe, which is 40 feet long, when the 2 pipes disconnected and 1 end of the pipe that the crew was trying to install slipped out of the joint and struck the employee. An excavator was being used to position a 36" pipe diameter by 40' long section pipe to connect it to another section of pipe.

EE was fatally injured while installing a rib bolt in the 8 Right Crosscut on Unit 1 when a slick-n-side formation fell from the mine roof inby the ATRS of the bolt machine, struck the mine floor and protruded into the employees work area. The employee was struck against the back with the rock and caught between the drill pot/mast area of the Fletcher DDR-13 WT he was operating.

The shuttle car operator was shoveling the coal rib in the 5-6 crosscut on #3 section at #18 break. This was the 2nd open crosscut outby the face of #6 entry. The scoop operator turned from #6 entry battery first into the crosscut and struck the shuttle car operator. The shuttle car operator sustained fatal injuries.

Scoop operator placed the scoop bucket over the edge of the elevator/hoist to eject a trash insert. Inadvertent movement of the elevator/hoist raised the scoop upward and the scoop operator ended up under the scoop deck suffering fatal injuries.

EE was a self-employed individual who was subcontracted by T & T Energy to perform welding work. EE was performing welding work on the T & T Energy, Begley #1 jobsite and while working experienced fatal blunt force trauma to his head. There were no direct witnesses to the accident and the accident is currently under investigation.

Employee worked as lead maintenance mechanic. Believed on morning of accident, Employee went to area Kiln 2 Preheater, Ram 4 to respond to work order. Employee had two-way radio. No witness to accident. Unknown whether employee was inspecting work or performing work on the No 4 ram. Initial examination suggests Employee was injured when ram activated. Ram was not locked out.
