Investigator narrative
Employee was cleaning a jammed rock from a return roller on a track mounted CEC Crusher. The engine was still running and the belt was engaged. Employee ""assumed"" belt was turned off because the head pulley was not spinning skipping on the belt; the belt was actually stalled. He did not lock out/ and tag the crusher before performing the work to de-energize all power.
