A ROOF FALL OCCURRED ON THE 001 SECTION CAUSING INSTANT DEATH TO I NAME JACKSETTER THE ACCIDENT OCCURRED AS THE WILCOX MINER WAS BEING POSITIONED IN THE FACE OF THE NO. 2 ENTRY WHILE THE VICTI M WAS RESETTING TIMBERS WHICH HAD BEEN REMOVE FROM THE CROSSCUT.
