Maintenance supervisor was disconnecting an electric forklift battery power connection. Employee came in contact with battery frame and received shock on the right wrist. Electric shock ran through their upper body to left hand. Employee was transported to the ER for evaluation and was released.

Miner stacked pallets 20 high for use in packaging supersacks. When they reached for the top one a foot got caught under the pallet stack and when they went to step back was unable to. They fell back on to the roller conveyor and sustained a back contusion and sprain of ligaments of thoracic spine.

Employee worked for about 30 mins packaging product in super sacks then started to clean for 60 minutes. EE felt light headed & went to get a drink of water. Went back to work. EE felt light headed still so EE stood in front of a fan. EE was then helped to the break room. EE felt overheated. The temperature outside was high of 100. EE had hiked the previous day. EE was dehydrated.

During a bulk truck loading and packaging Temporary worker was kneeling on the top of the truck closing the truck hatch after finishing bulk load out. When the employee stood up EE felt a sharp pain in the right knee. Kneeling to close hatches and add seals is a normal practice. The temp worker was driven to the hospital and it was determined that EE had a right knee strain.

Employee was lifting a pallet to place it under a loading spout when ee lost grip causing the pallet to fall onto the left foot above the steel protector resulting in a bruise.

Employee was pulling a vacuum hose when it got stuck between crossbeams. He yanked the hose which "slingshotted" towards him striking and chipping his front tooth.

Employee was sweeping the packaging/warehouse floor when he bent down to pick up an empty bay he felt a sharp pain in his back.

Employee was on top of a bulk truck trailer trying to align a loading spout with the trailer opening, when he tripped and fell through the space between the bottom rail of the gangway and the trailer, landing on the ground.

Employee was lifting slipsheets when he felt sharp pain in his lower back.

Employee was removing 300.20.04 screw conveyor when he stepped on the edge of a section of floor grading that had been removed and placed on the floor.

The employee was stepping down from the forklift, lost his footing, and injured his left hand and wrist falling to the ground. Employee is on restricted work duty.

EMPLOYEE WAS STEPPING DOWN FROM THE DFA ROBOT PALLET DISCHARGE CONVEYOR AND TWISTED HIS LEFT ANKLE. THIS WAS AN UNAUTHORIZED WALKING ROUTE AND HE WILL BE RETRAINED. HE IS ON RESTRICTED WORK ACTIVITY

EE WAS PERFORMING LAB TESTS. AFTER DIGESTION OF THE BONE MEAL SAMPLES, EE WAS RINSING THE LID OF THE VESSEL USING A 2:1 NITRIC ACID SOLUTION WITH A SQUIRT BOTTLE. THE BONE MEAL AND NITRIC ACID SPLASHED UP, CAUSING SEVERAL SMALL ACID BURN SPOTS ON HER FACE. THE BURNS WERE CLASSIFIED AS SECOND DEGREE BURNS.

Employee was attempting to lube throttle linkages with a can of WD40, when retracting his hand he came in contact with a fan blade. He cut his left palm area and the tip of his left thumb, requiring 17 stitches.

EE WAS UNLOADING DRUMS FROM A PICKUP TO BE USED TO CATCH OIL BEING DRAINED FROM THE ROLLER MILL.AS HE SET THE DRUM DOWN, & TURNED IT, HE RECEIVED A 2CM LONG LACERATON TO HIS RIGHT THUMB

EE WAS SHOVELING CALCIUM CARBONATE INTO A HOPPER AND HE FELT A SHARP PAIN IN HIS LOWER BACK. EE WAS PLACED ON RESTRICTED WORK ACTIVITY.

EE ALLEGES HE INJURED HIS BACK AT WORK WHILE WORKING ON A ROTARY ASSEMBLY. HE DID NOT REPORT THIS TO THE COMPANY UNTIL 12/16/03 AFTER HE WENT TO HIS OWN PHYSICIAN. CLAIM IS BEING QUESTIONED SINCE HE DID NOT REPORT UNTIL ONE WEEK LATER AND AFTER HE WENT HUNTING ON HIS OWN TIME. SEE ATTACHED NOTE. HE IS ON MODIFIED WORK.

THE EE WAS USING CAUTION TAPE TO TAP OFF THE NORTH #1 BULK LOADOUT SCALE BECAUSE THE LOADOUT SPOUT WOULD NOT RETRACT. AS THE EE STEPPED OFF OF THE SCALE, HE STEPPED ON THE SCALE FOOTING CAUSIN G HIS FOOT TO ROLL SIDEWAYS AND HE FELL TO THE UGROUND. EE FRACTURED HIS FIBULA BUT BOTH THE INITIAL PHYSICIAN AND THE ORTHOPEDIC SPECALIST RETURNED HIM TO MODIFIED WORK. HE DID NOT LOSE ANY

EMPLOYEE CLOCKED IN AND WAS CLEANING HIS POCKET KNIFE. HE ALLEGES HIS HAND SLIPPED AND HE LACERATED HIS RIGHT INDEX FINGER, WHICH REQUIRED 10 STITCHES. CLEANING HIS OWN PERSONAL POCKET KNIFE I S NOT ONE OF HIS ASSIGNED WORK DUTIES.

EE WAS UNFOLDING A LADDER AND PINCHED HIS THUMB BETWEEN TOP STEP AND RAIL.

TWO EMPLOYEES WERE PLACING A WALK GRATE. ONE DROPPED HIS END SO THE OTHER EMPLOYEE'S END SLIPPED FROM HIS HAND AND ONTO HIS RIGHT INSTEP. INJURY WAS A CONTUSION.

EE WAS WALKING DOWN THE WEST STEPS TO THE BULK LOADOUT AREA AND TRIPPED. HE TWISTED HIS LEFT KNEE. EE WAS SEEN BY A PHYSICIAN AND PLACED ON MODIFIED WORK.

EE'S LEFT KNEE BEGAN TO SWELL IN THE MIDDLE OF HIS SHIFT. HE ALLEGES IT WAS CAUSED BY KNEELING WHILE WORKING ON THE CRUSHER SYSTEM. IT IS QUESTIONABLE AS TO WHETHER OR NOT IT IS WORK RELATED B ECAUSE OF THE DIAGNOSIS.

THE EE WAS INSTALLING A MAGNET HOUSING. HE HAD THE MAGNET LAYING ON THE FLOOR WHERE HE WAS WORKING. HE STEPPED BACK ONTO THE MAGNET AND TWISTED HIS LEFT ANKLE.

EE WAS TAKING INFORMATION FROM A CLASSIFIER MOTOR TAG PLATE WHEN DUST PARTICLES FELL FROM ABOVE.SOME OF THIS MATERIAL GOT INTO HIS LEFT EYE. A SMALL PIECE OF METAL BECAME INBEDDED IN HIS EYE.

EE WAS WALKING INTO THE SPARE PART WAREHOUSE WHEN A METAL DOOR THAT WAS PROPPED UP FELL ON HER CAUSING HER TO FALL TO THE GROUND. EE WAS NOT SEEN BY A DOCTOR UNTIL 2 DAYS LATER.

AFTER COMPLETING JANITORIAL DUTIES, CONTRACTOR WHITE GLOVE PRO CLEANING SVCS EE LOCKED THE DOORTO THE ADMIN BLD. SHE TURNED QUICKLY, STUBBED HER TOE & FELL TO THE GROUND ON HER RIGHT SIDE. FI RST AID WAS GIVEN UNTIL AMBULANCE ARRIVED. SHE WAS TRANSPORTED TO HOSPITAL FOR TESTS; SHE SUSTAINED 2 FRACTURED RIBS & FRACTURED STERNUM. TREATMENT CONSISTED OF MEDICATION ONLY.

SCREEN CONVEYOR WAS LODGED. EE TRIED TO DISLODGE IT MANUALLY. HE TOLD CO-WORKER TO TURN CONVEYOR. CO-WORKER MISUNDERSTOOD, AND TURNED BELT ON, CAUSING EE'S LITTLE FINGER TO BE ROTATED BETWEEN BELT AND ROLLER, AND CAUSING CONTUSIONS ON ALL FINGERS OF RIGHT HAND.
