As EE pulled on the wrench, it slipped and the wrench struck the tip of EE's finger smashing it between the wrench and the bumper of the truck. EE was taken to the clinic where it was determined that EE suffered from a laceration to the tip of the finger which required 3 stitches and a fracture to the bone as well

It had just rained and EE was walking the crusher at gray Mtn when ee slipped and pulled a muscle in left leg

Employee was was on the cat walk of the cone crusher checking for bowl float when the cone over flowed and an 8" rock fell down cutting ee left little finger that required three stitches

Bottom of stairs going to office there is a rubber mat and with all the rain received at locations scale operator coming out of scale office and slipped on rubber mat twisting her knee causing knee injury.

EMPLOYEE WAS LEANING AGAINST RAILING ON CATWALK WHEN ANCHOR POST BROKE CAUSING HIM TO FALL BACKWAYRS APPROX. 2 1/2 FT., LANDING ON HIS BACK AND CAUSING A COMPRESSION FRACTURE TO LUMBAR #1.
