Employee was shoveling material at the secondary screen when they felt pain on left forearm and noticed swelling. The initial injury occurred on 06/20/2025 but EE declined medical treatment. On 06/23/2025 asked to see a doctor and was taken to a minor emergency clinic for treatment. Diagnosed with strain of muscle left forearm and place on restrictions.

Operator of a Caterpillar D6 Dozer was repositioning on a narrow strip and was looking behind them while travelling forward and ran the left track over a berm causing the dozer to fall off of a 15 foot tall highwall. The operator struck their head on something in the cab and was taken to Urgent Care. The physician asked that they be off work for two days.

Ratliff Ready Mix was on site to pour a new pad for a screen. The truck was leaning slightly to one side. A chute started to swing toward an employee. EE did not see the chute moving in their direction. The driver said "Look Out" but it was too late. The employee was struck in the knee by the metal chute. EE's knee cap was dislocated to the side of leg. EE was taken to hospital.

Employee was using a cutting wheel on a hand held grinder to cut a piece of metal. When cutting the metal, the grinder jumped forward resulting in contact with the employee's hand. The employee was wearing all proper PPE for the task being done.

Employee was using a pipe wrench to turn a belt. The pipe wrench slipped causing him to lose balance & footing. While attempting to regain footing he hit his head (the back of head) on a ladder. This forced his hard hat up & cut the rear of his head. The cut was about 2 inches in length requiring staples at the doctor's office.
