Sebastian Mine Coal

While flipping the belt over on the beltline, slipped on tail roller, fell and landed on thumb.

Bad roof conditions. We had installed several different supplemental supports. The roof fall partially covered the front ATRS of a Fletcher roof bolter.

Mine power had been off for several days due to lightning strike, no fan also. After fan was restored the mine was being firebossed and a roof fall was discovered at 10:05 am in the intersection of #6 entry @ x-cut # 76 on the Main West.

Was shutting the double doors at cross cut 3 on intake and the air blew the doors shut. Injured employee was trying to hold the door shut so it didn't hit another employee in the head.

EE was bolting roof when his starter stick fell out of the hole and landed on his right hand, pinching his hand between the starter stick and pusher, causing a puncture/laceration right hand between thumb and index finger.

While operating the scoop turned body to look behind while backing up, had left hand outside of scoop and backed into a timber which fell and hit hand.

Installing supplemental roof. Entire crew was rolling an I-Beam along the mine floor on the count of three. Beam rolled on top of right foot.

INDIVIDUAL DID NOT REPORT INJURY UNTIL Feb 18, 2015. 6 to 8 men were lifting 8" X 25' I-Beams to support the roof. Individual was located at the end of the beam and while lifting it above their heads he felt something "pop" down low in stomach/groin.

Examiner found the fall in the belt entry (#4)@ # 52 cross cut on the early exam for 3rd shift. Initially the fall was approx. 30' by 30' by 12' high. It appeared that the slickened sided slips along with some verticals lined up in intersection/crosscut.

Fall started with an unbolted face falling in and then propagating into the intersection in the #2 entry @ #52 cross cut of the West Mains. It appears that slickened side slips were present and lined up in the intersection. Fall was approx. 40' long and 30' wide through the intersection and approx. 12' high. No personal injury associated with the fall.

While moving continuous miner to new location, a rock fell from the roof and struck him on the left side of head, and neck.

While bolting the top, was holding pin wrench attached to drill steel, under pressure wrench kicked out and jerked shoulder.

While lifting/hanging miner cable, felt a pull in lower abdomen.

While cutting conveyor belt box knife struck right leg causing a laceration to right thigh

While fixing ventilation curtain underground, pulled on curtain and injured left shoulder.

While driving a 6-wheeled dump truck, loaded with dirt to back fill a low spot in order to create a road, the ground gave way and the truck fell over on drivers side off a 6 foot bank.

While escorting MSHA on E01 Inspection (Operating fan/airlock) opened airlock door, the inside door blew open and caused the first door to slam shut and smash middle finger of left hand causing a laceration.

While changing batteries in Fairchild Scoop, lifted battery lids and latched doors, door got jarred causing lid latch to slip and fall on right hand resulting in a fracture.

Putting a bolt in the roof. When the bolter met the roof it broke loose a rock that fell on the drill pot then bounced off and hit left forearm causing a laceration.

While pulling cable, underground, twisted left knee.

While hooking up a chain to the continuous miner (CM) lost footing and right leg slipped under the bumper as the CM was let down causing the bottom part of right leg to get caught between the bumper and ground. Was transported to the ER, x-rayed and released to duty with bruising injury, was re-examined on 4/14/2014 and his right ankle showed a fracture.

Moving equipment through a curtain, stepped off sled (6" high), to pogo a curtain down and twisted right ankle.

While changing steering jack pin, hammered on pin to remove and piece of metal flew off of punch and punctured left arm

9:58 am found a roof fall in an abandoned area, 3 panel, #5 entry, xcut 17, fell @ anchor zone @ 6' at the intersection, no one around, non working area, timbering & dangering off

While walking in a muddy area, employee slipped and fell to knees.

While roof bolting, loose piece of top/rib fell striking employee on left shoulder and arm.

While putting up 8 foot bolts and straps, employee was moving strap over to line up next bolt when a 2'4" L X 1'4" W X 1-1/4" H shale rock slid down strap and hit his right wrist

While watching shuttle car cable slack get reeled up, ready to signal when to stop, the cable flipped over got behind employee grabbed his feet and back and slammed him face first into the rib.

Piece of sandstone fell out of the top, employee moved slightly and the sandstone grazed his neck, shoulder and back.

Area of fall was in old works used for a sump area.The area was just outby another fall we had 12/29/2010. Fall was approx. 5' high 16'-18' wide and 20'-25'long. It was approx. 10'-15' inby the #7 Entry, East Panel of the 1st So. Panel. Seasonal change/humidity probably a factor. No damage or injury.

Employee is working as a bolter, he finished his shift on 3/20/2012 then went to the ER on 3/21/2012 complaining of shoulder pain. He was diagnosed with tendonitis of the right shoulder. Employee has stated that his right shoulder has been hurting for months.

IE and another EE were cutting roof bolts that were hanging down over #3 belt. IE handed other EE a lit torch as he was getting in position to cut the next bolt. The torch hose caught on the belt structure and turned the flame toward IE, he put his right arm up to block the flame from his face an received a burn to his right forearm.

Was down in hole servicing equipment, started to pick up and load tools and supplies on mantrip, when he picked up a 5 gallon bucket of oil, he felt a pop in his left elbow and pain shot down his arm.

Employee was moving timber from travelway, when the timber started to slip he put his hand under it to catch it and smashed his finger between the timber and the ground.

While walking Belts employee stepped in a rut and on a piece of coal/rock causing his weight to come down on his left knee and twisted it inward.

While cleaning diesel scoop, putting cover back on slipped and turned knee.

Employee was shoveling #5 belt when he slipped and over extended his knee.

Scoopman was tramming around a corner bucket first, where scoop was at it was high and where employee was at it was low so scoop driver did not see employee kneeling down cutting tie wire and employee also had ear plugs in and didn't hear the scoop and the bucket came down and hit the employee in left leg.

Employee was headed to check on#5 drive when he turned and slipped on some loose coal on the floor.

Employee was swinging a sledge hammer breaking up a rib roll when the sledge hammer got caught up in some tie wire hanging from the roof and struck himself in the head.

Employee had pulled the covers off of the diesel scoop to wash it in the pit, he had put degreaser on it and slipped and fell against the cab canopy.

Mantrip was having trouble making it up a slick hill so men got off to help lighten the load, the mantrip slide and hit employee on the ankle

Employee was pulling rock off the head of the miner and caught his right hand pinky finger between the rock and the pan on the miner.

Employee was kneeling down to put a nail in the curtain while hanging a back check when the rib from the roof to about 21 inches toward the floor rolled out and struck him in the back of the head.

Employee was in the process of installing a buggy anchor when a section of the rib rolled off striking him in the back and pushing him forward and pushed his face into the bolt tray and busting his lip and chipping a tooth.

C.M cutting #2 entry, they backed miner out to clean scrubber in #2 entry x-c9, while cleaning scrubber the top edges of along rib line started to dribble, men vacated the area and used the miner remote started backing miner out when middle of intersection fell on miner then gradually the entire intersection fell in on most of miner. Fall area 20' x 20' x 8 - 10' thick

East panel #2 entry, approx 20' inby center line of #7 Xcut, top had fell out apporx 12-14" semi-domed, employee ran drill steel up against roof. Pressure on steel cause a piece of rock to break off causing drill steel to shift/slide sideways striking index finger on left hand. Employee had moderate bruising/swelling & pain. Doctors results showed a dislocation.

Employee was walking to the power center in the #5 entry @9-1/2 xcut south to turn power on to shuttle car when he stepped on a ledge/hole and twisted his ankle.

Employee's foot was caught between the mini-trac & the bantam duster.

Employee was swinging a sledge hammer, busting sand stone when he felt a pain in the groin area.

Employee was hanging curtain when he felt pain in upper left shoulder.

Employee was cutting belt with belt knife toward himself when he cut himself requiring 5 stitches.

While Employee was bolting, slider roof cracked and a piece of rock fell out between bolts and hit him on his right arm then another one fell and hit his right leg.

Shuttle car was pulling away from continuous miner when employee's leg slipped under the shuttle car's frame and the shuttle car ran over his left leg.

Employee saw a slab of coal hanging on the rib. He got up to get a bar to pry it down, but the piece fell and fit him striking him on the ear requiring 4 stitches.

Roof bolter cable was smashed under the bolter. When employee was trying to remove it, the breaker was accidentally reset, causing an electrical burn to the employees arm.

While roof bolting the employee was changing the bit insert when a piece of rock fell from the roof striking him in the back.

EE was picking up 6' timbers and putting them in the scoop bucket and turned wrong. Did not miss any work until 7-5-08, making it reportable.

EE was bent down nailing a spad to hold down a curtain, when a rock fell from the roof and struck him, knocking him to the ground.

Walking toward the power center stumbled on rock and fell, put arms down to catch himself and strained left shoulder.

Stepping up onto the top of the bucket of the scoop he felt a sensation in his left knee.

Employee stepped up on to the travel road and felt his knee pop.

Employee lost his footing and fell on his thumb. No fracture, but required a splint. No restrictions.

Employee felt wrist pain after shoveling out the portal canpoy (to be painted).

Employee's hand slipped off a wrench and impacted the bolter head, causing a cut. It was later discovered that a sliver of metal was embedded in his finger, which required removal and stitches on April 1, making this accident reportable at that time.

Employee was bolting. His tool pouch accidentally initiated rotation while his hand was still on the steel.

EE jammed his finger while passing rock dust bags at Xcut 13 between entries 7 & 8. This was not a reportable injury until he had surgery on his finger on 10/14/08.

*** Employee notified Office Personnel on 05/21/08 of his condition, he had worked and not went to the doctor underworkers comp until this time*****EE pulled groin area while trying to move and lift a hydraulic pump in XCut 3

Employee was using his foot to push on a wrench, when he felt pain in his calf.

Employee was struck on the hard hat while roof bolting.

Employee was hanging cable and felt a pain in his back. Employee received a full medical release and no treatment. However, employee went to a chiroprator on 2/20/2008 making this now medically reportable.

Miner operator was trimming bottom when he noticed small blue flames along a portion of the floor. Extinguished flames with water. NOTE: This report was filed electronically on Sunday 12/16/07 from an airport location while traveling, but in checking the records today there is no evidence it was ever received by MSHA, so it is therefore being refiled.

Continuous miner on MMU-002 was trimming bottom in 4 entry when an instantaneous ignition occurred. A large sandstone roll was present in the face.

A small instantaneous flash occurred when the miner was cutting in 2W entry face.

Employee appeared to momentarily lose conciousness when sitting in the lunch area. He was taken outside, where he refused medical attention. Work had not yet commenced. There was no injury.

Employee's nose was caught by a nylon cable hanger hook and was cut, requiring stitches.

The continuous miner began cutting the face of #5 entry when a significant sandstone roll was encountered on the initial sump. The miner head contacting the sandstone created a spark, which in turn caused an instantaneous flash, presumably due to methane.

Employee strained a pectoral muscle while lifting.

Miner operator accidentally lowered the head against sandstone chunk and created a spark leading to a small ignition. This is the second electronic filing for this incident. The original was filed July 31, 2007 but does not show up on the MSHA database.

Employee slipped and dislocated his finger.

Employee was making a cable splice, when the hammer/anvil in use, broke and sent a chip of steel into his finger.

Employee was helping remove a steel bar from a roll of belt. Employee lost his grip and caught his finger between the bar and a stationary object. This resulted in a laceration, requiring stitches.

Employee slipped while walking past a piece of parked equipment and jammed his thumb. Required a rigid support installed on the thumb.

Employee smashed finger replacing cover on continous miner, stitches were required.

Continous Miner was 10 feet into a cut when the operator backed up to trim roof. The sandstone roof created a spark that apparently ignited a small methane bleeder. No flash was observed, but a small air wave was felt. There were no injuries or damages.

Employee was operating a roof bolter, the wrench slipped out of the chuck, the bolt fell onto his arm causing a fracture.

Employee was roof bolting and a speck of material got into his eye.

Employee was moving a miner cable and pulled an abdominal muscle.

Employee developed a skin rash after getting into water on the section.

Employee caught his finger between a pipe and the mine roof.

The continuous miner was cutting in number 1 face when a small instantaneous flash occurred.

Employee was trimming a belt with a knife, cut his leg, and required stitches.

****Employee had not missed any work with this injury unitl the scheduled operation on 5/15/08, he was on vacation until 5/27/08 when he returned with a light duty release.***** Moving an electrical cable employee slipped and fell twisting his left knee.

Miner was cleaning up in the face of 8 entry when a small instantaneous flash occurred.

Employee was stacking crib blocks to block up the head of the continous miner. One of the cribs fell on his foot, resulting in a fracture.

The continuous miner was pushing cleanup matrial after completing a cut when a flame appeared at the miner head. There was no damage to personnel or equipment.

An ignition occurred in the face, confined to below the cutter head, while cutting through a small sandstone roll. There were no injuries.

Employee twisted his knee in muddy conditions when walking.

Miner was cutting in number 7 entry face. The bits hit the sandstone roof, creating a spark that resulted in an instaneous flash of light believed to be an ignition. There was no noise, air blast, smoke, or damage.

Employee was running a roof bolter, the steel bowed, caught his hand. The hand was cut, requiring stitches.

This is the second electronic filing on this incident. Apparently the first filing did not go through. A small instantaneous ignition occurred in the roof of #9 entry while cutting the face.

This is the 2nd electronic filing on this incident. Apparently the first filing did not go through the syste. A small flash occurred while the continuous miner operator was grading bottom in #8 entry. The flash was instantaneous.

Employee reports pain in back while rockdusting the belt line.

Employee was starting a diesel coal hauler, the air tank ruptured accompanied by a loud noise. Employee suffers from headache, dizziness, possible loss of hearing. As of 6/23/2006 employee is off work due to inavailability of medical care (ENT specialist) needed to continue diagnosing nature of the injury.

Employee slipped on the mine floor, landing on his knee. No immediate medical treatment was given, and no job restrictions given. On 6/21/2006 employee underwent additional diagnostic treatment and damaged meniscus was reported, making this a medically reportable accident to MSHA. There have to-date been no lost or restricted duty days: this will be updated if necessary.

The ee was helping lift a 12 ft timber for setting along the belt line. During the lifting process he felt a pop in his knee. Employee did not seek medical attention for several day and cont'd to work. Initial diagnosis is a miniscus tear and surgery is likely. Report will be amended when/if restricted or lost days occurs.

WHILE WORKING TO RESET A SUMP PUMT, THE INJURED SLIPPED AND TURNED WRONG CAUSING A STRAIN TO HIS RIBS.

WHILE INSTALLING ROOF BOLTS INTO THE ROOF, THE INJURED'S SHOULDER POPPED OUT OF PLACE AND HAD TO BE POPPED BACK INTO PLACE. HE RECEIVED PAIN & ANTI-INFLAMATORY PRESCRIPTIONS & WAS SENT BACK TO WORK THE NEXT MORNING BY A DOCTOR.

A CONVEYOR BELT ON THE WASH PLANT WAS NOT RUNNING, THE INJURED TURNED OFF THE POWER AND WENT TO TEST THE DRIVE BELTS AND PULLEY ON THE DRIVE MOTOR. AS HE PULLED ON THE DRIVE BELTS TO CHECK WHE THER THEY WERE LOCKED UP, HE ROLLED HIS FINGERS INTO THE PULLEY CAUSING LACERATIONS AND/OR TEARING ON TWO OF HIS FINGERS AND A CUT ON THE WRIST. ONE FINGER AND THE WRIST RECEIVED STITCHES AND

CONDITIONS ARE WET & SLIPPERY MINE FLOOR. EE WAS TRAVELING ON FOOT IN THE RETURN ENTRY APPROX 350 FT. INBY THE SURFACE PORTALS WHEN HE SLIPPED ON THE MINE FLOOR AND FELL, TWISTING HIS RIGHT KN EE. THIS DID NOT BECOME A LOST TIME ON RESTRICTED DUTY ACCIDENT UNTIL 10-10-02.

EE STOPPED AT J.F.&H. EQ SERVICES IN FT. SMITH AR. TO PICK UP A COMMERICAL WELDER MOUNTED TO A RUBBER TIRED TOW TRAILER THAT MID AMERCIAN HAD PURCHASED FROM J.F. & H. WHEN EE ATTEMPTED TO LIFT THE TONGUE OF THE TRAILER UP TO HITCH IT TO THECOM. TRUCK, HE FELT SHARP PAIN IN THE LOWER PART OF HIS BACK.

EE STARTED DRILLING A BOLT HOLE WHEN A PIECE OF COAL/SHALE SLID DOWN DRILL STEEL HITTING HIS WRIST. PIECE MEASURED APPROX 2' X 2' X 16" THICK.

EE WAS NOT PAYING ATTENTION WHEN STARTING A BOLT IN A HOLE C/W RESIN CARTRIDGE. PROBABLY DID NOT HAVE DRIVER ON BOLT HEAD BEFORE APPLYING THRUST CAUSING THE TWO TO THRUST APART AND PINCH HIS T HUMB.

PRESHIFT EXAMINER FOUDN ROOF FALL IN #6 XCUT/#1 ENTRY INTERSECTION. CAUSED BY PRESENCE OF 'HORSEBACKS' & WATER IN ROOF. FALL 20' WIDE IN #1 ENTRY & 30' LONG. CARRIED 20' OUT INTO XCUT #1 TO #2 ENTRY. STEEL BEAMS SET AT BOTH BROWS.

EMPLOYEE WAS STRUCK A GLANCING BLOW WITH A LARGE PIECE OF FALLING ROOF ROCK WHILE STANDING ADJACENT TO A 10SC SHUTTLE CAR, ALONG AN EMPLOYEE WHO WAS WATCHING THE MINER TRAILING CABLE.

EE WAS REMOVING DAMAGED GATHERING ARM RACE. HITTING SAME CAUSED METAL CHIP TO FLY OFF RACE & HIT EE IN EYE SOCKET.

EE JUMP STARTED DOZER WHEN BATTERY BLEW OFF CAUSING CONTENTS TO HIT HIS FACE AND HEAD.
