Employee was releasing a pressurized line and it exploded and cut employee's hand.

Employee was climbing a ladder when they slipped and fell 8 feet to the ground. They sustained a fracture in their right hand, a contusion on their right hip and a contusion on their left foot.

Employee was using ladder, slipped and fell approximately 12 feet.

Employee was staging grease drums, one pulled away and they stopped it with their right arm and their elbow popped an pain ensued.

Employee opened a poke hole on the hot air slide. It immediately went positive. The force blew off EE's hood and burned EE's neck and ear.

Employee was moving a cart down a ramp when the items on the cart slid and lacerated left hand.

Was lifting a hoist on a platform in the tower and EE reinjured a hernia.

Finger was pinched by a roller on the tire system.

Smashed finger when lance slipped inside poke hole over refractory.

Operating the drill over a period of six months caused an injury on the right palm.

Walking down stairs and somehow rolled foot outward.

Laceration to right hand caused by pipe stand striking employee while turning and wrapping shaft assembly.

The control room monitored a variation in temperature within a bin which was handled by normal procedures, consistent with design capacity. There was no injury or illness resulting from this condition. This pressure excursion does not constitute an "accident" as set out at 30 C.F.R §50.2(h). The "blast gate" which is designed to relieve pressure in this type of situation did open.

Employee was climbing into North side of Pantleg to see if the welds were cut on a seal plate, not realizing it was under pressure and got hit by plate piece.

The employee was holding a piece of wood to brace the metal while another employee was hitting in place. The other employee went to swing the sledgehammer getting it caught in the chain fall chains knocking EE off balance causing EE to hit the EE's hand.

Employee was picking up a case of 6 hams and 2 turkeys. While loading hams and turkeys into the tailgate of the truck, EE strained back.

Unplugging tire chute, strained back.

Employee was Shoveling Material and twisted body they felt a pinch in lower back Note: Employee returned to work the following day on Modified Duty 7/14/2020

Chemical Burn blisters to both ankles. Employee is reporting "Working in a very dusty environment the product we were cleaning got into my boot and by the end of the day they were rubbed raw"

Employee was mucking out the cone of 4cy10 vessel and was struck in the right shoulder by piece of falling debris.

Employee was using a crowbar to break rocks stuck in the vibrating screen. one rock broke easier than expected, causing to reach farther then intended and fingers were smashed between bar and screens.

Burn to hands and knees due to hot material

Shoveling into a skid steer bucket and felt a pop in back, experienced pain in lower back.

Smashed thumb in between a pinch point.

Employee was attempting to close the isolation valve on 5AC1 and felt a pop in EE's right shoulder

Left rear wheel came off of cart and lost control of vehicle, braced for impact when cart fish-tailed. Initially, a first aid incident, follow up Dr. appt July 1, placed employee on restricted duty.

Walking up stairs between f-2 and f-4 foot got caught between grates causing strain on left foot

Was pushing a loaded wheel barrow on an uneven surface and hit a bump causing pain in left lower back.

During cleanup, pinched hand between chipping gun gat and equipment housing.

EE States Exposure to dust in workplace water system with silica has caused EE to be diagnosed with Lupus in 2016

Walked down the staircase rolling ankle.

Laceration to right hand finger.

Strained back when turning to dump shovel.

Employee Stepped over an I-Beam, Clinker on the other side of I-beam, sprained left knee

Lost grip of Hydraulic ram

Mill Ball fell from above and struck employee on hand, bounced off and hit knee

3 workers were welding on a flat 5' X 5' steel door for a hammer mill (4BR1), the door was turned upright resting on the ground & leaned against a forklift for the final welding position. The door became unbalanced while the workers were welding, & fell over striking EE on the leg. EE was taken to the hospital by ambulance & was admitted for a broken leg.

Employee placed a metal rake against a post while waiting for a bucket to fill. Metal rake fell and hit employee in the back. Employee had a personal motorcycle accident 6/28/16.

Employee was cleaning under the kiln feed and dust hopper when shove bumped the chute and material came out covering ee. Dust got in eye causing an irritation and burn to face and right eye

Employee was lifting a lock and load weighing about 20lbs. EE set the lock and load in the CAT bucket, stood up and felt a pinch in the back resulting in a strained back.

While employee was removing a large piece of Rebond from Cyclone opening, EE pinched EE's finger between the rebond and the side of the opening. The result was a minor bruising of the finger.

Employee was trying to loosen a hydraulic line on a unit and sprained lower back due to excess tightened fitting.

High winds, Blowing dust, Paved uneven surface with coating of clinker dust. Employee was walking to break and his foot slipped out from under him, causing him to wrench his back while falling.

While trying to install a blank to block the flow of material, the section in which the blank is installed began to eject dry lime striking the employee in the chest and rebounding into his face.

Employee was cleaning down-comer when he smashed his pinky finger between the door frame and air-lance

While on top of a silo cleaning silo 5556 at mine site, silo whip hit handrail and hit miner on the nose causing it to bleed.

Employee was working on a moisture analyzer which had a small amount of water that overheated and sprayed out a blast of steam and hit him causing burn to his abdomen.

Hearing test levels reduced from 2014 to 2015

Driver was attempting to open lids on top of pneumatic trailers when the rack had become jammed and the lid had wedged in the rack. When he tried to free the lid, the rack jumped up causing the employee to fall 12 feet.

While working inside Dust Collector baghouse, blowing off buildup dust got inside goggles and irritated miner's left eye.

While Miner was assisting inspection of Baghouse, dust got in miner's left eye. He was wearing proper eye protection. (Goggles). The dust entered the goggles and irritated his eye.

While testing the Stowage of the jib on the boom, it fell off the locking mechanism dropping two feet to the top of the Stabilizer guard. It hit employee on the shoulder bruising it, The sheave mount scratched him on the back, strained his shoulder muscles. The Jib pin locking was not properly installed or double checked. There was no failure of equipment.

Employee states while removing bad thermocouple hot material exited hole and struck his face, causing 1st and 2nd degree burns to ears, eyes and head.

While manually loading bricks in the bucket of a bobcat with 3 other employees, a brick being tossed into the bucket hit employees right hand and smashed his index finger against the brick he was tossing. The impact broke his right index finger. Employee was wearing gloves.

Employee was walking to get a bucket for an oil leak on a loader when he stepped wrong and twisted ankle

Employee was organizing top shelf in the tool room and while stepping down from the step stool, the stool slipped out from under him.

While trying to break up a chunk of rock with a jackhammer, the jackhammer slipped off the rock puncturing his toe.

Employee injured his left hand while doing repairs to a sledgehammer in the tool room. Employee was hammering a wedge into the top of sledgehammer to tighten the handle to the head, the sledgehammer was clamped in a vice, and he hit the base of his left thumb with a glancing blow while trying to drive the wedge in with a hammer. No broken bones, released for modified duty.

Employee was relieving the pressure on the lime slurry injection system the discharge was plugged of the relief valve. Employee thought the pressure was gone and opened the line when it hot lime water shot out and went up his shirt sleeve burning his left arm.

While Poking down in the tower employee states he was sweating, going into his left eye causing irritation and blurred vision.

Employee started walking towards 48 forklift when he slipped, tripped and fell over a pipe that was sticking out in the dark. He hit the ground cutting and splitting his left hand.

While closing the knife gate, the Porta Power slipped pinching the employees hand between the frame.

(Weather - dry/calm) Accessed fall protection rack then activated air operated gangway to lower it to top of tank trailer. Operator then walked out across gangway to trailer to open lid. The gangway w/midrail started up on its own, exposing operator as he tried to grab it - lost balance and fell to ground.

Employee was inspecting walls between bag houses standing on electrical raceway, weld broke and he fell to deck below landing on feet. (8 feet) Not wearing Harness, nor following procedure of scaffolding the area. He received minor cuts and a sore back in the fall.

Employee was pushing an oversize tire over a gate as two coworkers were pulling on tire when it rolled backwards and tweaked his right wrist.

While moving dust bags on shoulder, materiel shot from bags into face and eyes. Employee was wearing eye protection and dust got into left eye around the safety glasses. Employee rinsed eye and didn't have a problem rest of shift. Next day (Sun) Called Super and reported. Emp. went to Doctor and had foreign body removed.

While installing a 160 double roller chain around a set of sprockets, EE left index fingertip was cut off at base of the fingernail.

While changing bags in bag house, with system disabled, cement dust flew into eyes and face from above, causing irritation to the eyes.

Foreign body in right eye, not initially felt. Began to bother miner next morning. Physician removed item from eye.

Employee was working on 4CY10 plug up. While walking pass the tipping valve, he hit the end of the tipping valve arm at his nose and teeth.

Miner's arm contacted edge of a heat shield causing laceration

Truck driver from customer (Double Eagle) fell on flat ground after checking his trailer while filling. Driver stooped under his trailer to check butter fly valve. when he turned to stand up he bumped his hard hat on a support beam and fell down breaking his lower right leg.

Backed over bolder with loader and jarred his back.

While trying to realign the ram with the hole in the plate on the slide wrench for 013 drill the operator was holding the plate with his left and operating the ram with his right hand, The ram jerked forward pinching the tip of his finger between the plate and the slide wrench severing the tip of his left index finger.

A staging platform to open/close truck lids for loading cement powder had a step on the loading dock of the platform which needed resecuring. One step was lifted to expose the repair area of the step needing repair. While working, the lifted step fell and hit the left hand pinky finger of employee.

Shoveling a pile of mud at counterweight of 2BC2 belt conveyer, and strained lower back

Climbing up a scaffold ladder, our employee IE's hand slipped out of his glove. While compensating, foot slipped, He fell backwards and down 5.5 feet hitting another EE who caught him and broke his fall. IE hit his hip on the floor.

While walking on a slope, employeed slipped and fell.

After removing 7DC8 Dust collector roof hatch using proper lifting techniques, EE stood up and felt a pinch in his lower back which continued to his left side. He refused to go to the doctor and worked to the end of his shift. The next day he had increased pain and then requested to go for medical attention.

On December 8, 2009 EE reported to his supervisor that his Rt knee was bothering him when he was kneeling to change dust collector bags, He refused to go to the doctor at that time (Not Reportable). On 4/8/2010 He requested to go to the doctor to get the knee checked saying it still bothered him and He was put on restricted duty (Now Reportable).

Using a porta power to remove a bearing from a shaft, EE put his hand on the bearing to see if there was any movement. The bearing broke free and a spacer popped off the shaft and hit EE in the left index finger.

Felt sharp pain in left lower abdomen while lifting 60# Actuator onto bed of Rig truck 30 inches fm gnd. Doctors first report says Employee has abdominal muscle strain.

IE was using an air lance to clean build-up from inside of feed hood. As he pulled the air lance out of the poke hole, he slipped backward, grabbed onto handrail(gate) bumping his right knee and left elbow causing a contusion of right knee and left arm.

Picked up 5 gallon water bottle, approx 50 lbs, and turned to place upside down on top of fountain, employee felt sharp pain in lower back. Twisting motion on back under load caused problem.

Wood chips/dust burned on the conveying area of the wood chip 'Wood Hog" system. The fire lasted approximately five to eight minutes. The extent of the damage was a piece of burnt rubber skirting. There were no injuries related to this incident. Three ABC fire extinguishers were used extinguish the fire.

Employee Layed off 3/31/2009, Dr's First Report of Injury Received 5/12/2009, I was notified and received copy of Dr's first report on 5/21/2009. Employee did not lose time or see Doctor during employment for his claim that while pulling a wheel barrow he felt immediate pain.

Driving pickup to job, stopping, seat slid forward unexpectedly, wrenching lower back. Seat was not latched in driving position. Driver new to pickup and unaware seat was not latched. Seat belt in use but attached to seat frame which moved with seat.

Miner was pulling a conveyer belt into place. A C-clamp grasped the belt, being pulled via a choker by a forklift. The clamp pulled loose and sprung back, striking the miner in the face. He had a lip laceration and minor contusion to his face.

EE was descending an A-frame ladder from an elevated (5') work area. His foot slipped off the 3rd or 4th rung of the ladder, causing him to fall 3-4 feet, landing on his left leg and tail bone. Back-tender over L1, Leg-tender over lateral. No dislocation or bruising.

Assisting coworker lifting and sliding sideways a 250 lb pipe onto steel table, pipe coated with nonslip material creating more resistance, Employee felt sharp pain in lower back. Employees should have used mechanical lift but were in "hurry" was the reason for not following their training and company policy on proper lifting.

Employee experienced a sharp pain in his left shoulder on about Jul.2,2008 while installing conduit at the Roll Press and again on Aug. 29 while performing similar work at the Blend Silos. The pain continued to worsen and it started impairing usage of his arm. On Sep.28 he reported to a supervisor his condition and arrangements were made to get medical advice.

Loading 20 foot piece of 8 inch wide channel iron, hand on end of channel to steady it. Material slid sideways 4 inches, finger pinched between metal and ground and cut on end requiring stitches. No glove on hand, employees in hurry.

Employee was assisting in opening a rail car bottom dump door by attaching a come-along to the door to try to pull it open. The day after the event, he felt discomfort in the left side of his back. On Aug.20th the employee reported that his back was still hurting, the company arranged for a Dr's. appointment that day. The extent of the impairment is not yet known.

While jack hammering on clean-up project, jack hammer slipped off of rock causing left ring finger to be pinched between jackhammer and frame of T-Bird crusher.

There were no witnesses to accident. Injured appeared to have been closing the lids on truck trailer and fell to ground between platform and trailer while attempting to traverse the opening between top of trailer and the steps of the platform. The truck appeared to have been positioned approximately 3 feet further away from the platform than normal.

Employee was pushing upward with a long metal rod trying to dislodge a tire that was jammed up on the tire rollers (conveying system). In doing so, employee strained his lower back.

WHILE EE WAS ADJUSTING THE PULLER, HE PULLED HIS BACK MUSCLES CAUSING SPASM TO HIS MID BACK.

Rubber tires were moving along a roller conveyor. A tire became jammed against a steel beam, other tires continued to tighted up the row of tires. The injured employee attempted to 'unjam' the tires by pulling up on the first tire in the row. As he pulled on the tire with a long hooked bar, he strained a muscle in his lower back.

Employee bent down to pick up box and aggravated prior injury to left knee. He properly lifted the box.

Installing gauges on bottle on top of rig truck bed. Turned & slipped off edge falling to ground-broke fall w/arms on ground. Did not go to doctor until 3/20/08. Dr deemed he was able to work w/no limitations. This made the injury "not reportable". On 4/29/08 he went to his personal orthopedic surgeon & was put on limited use of left hand, making it reportable on 4/29.

Climbing down from access hatch approx 10' above landing and 50" above walkway, employee slipped, scraped right lower back on metal step. Reacting to the injury, grabbed bar, swung down to walkway stepping on the railing horizontals, rolling to deck on his back. He was wearing double lanyard and harness but failed to use it in accordance with company policy.

Employee was working in 4cy5. At 10:30 pm a large piece of coating fell from the upper walls of 4cy5. The coating fell on his head and left leg.

Employee was walking from her car to her office when she stepped on a small rock in the roadway, causing her to slip and fall. There was a slight down hill angle and it was not yet daylight although there are street lights in the area. Her knee was cut when it made contact with the rough surface of the pavement and required stitches.

Climbing ladder to dust collector, when at top of landing reached up 4 feet and felt pull in right elbow.

Miner had a respiratory distress situation. Miner had problems breathing at beginning of shift, but insisted she was fit for duty. During work activity cleaning up a raw material spill she had further difficulty breathing and sought treatment.

While assisting another employee with unplugging a rock feeder, a rock, approximately 4" in diameter, came out of the poke hole, ricocheted and struck employee in the pelvic area causing a bruise.

Using sledge hammer, swung and missed piece of angle welded to shaft on screw for 4SC29. Pinched right middle finger between handle of sledge and the angle iron that was sticking up.

Employee was sitting in crusher control room assumed to be sleeping. when awakened he said that he forgot to take his insulin shot for diabetes. He was talking and acting like he just woke up.

Dusty floor.

When reporting the incident, employee stated that his foot was hurting when he woke up. After starting work, the condition was aggravated when he stepped down from a step in the pre-heater tower. He had pain in his ankle/foot and it appeared to be swollen. Although it was never diagnosed as gout, the Dr. suggested that was what it might be.

Miner diagnosed with mesiothelioma which may be work-related.

This is a claim of repeditive motion injury to the left shoulder. The minor did not report this claim until after a non-work related vehicle accident, where the physician indicated that a clai med shoulder injury may be work-related.

Employee stated he was on one knee removeing a cover on a poke hole (a metal cover approximately 1/4" X 4") on a 4" round pipe. As he went to put the pin (1/4" X 1" X 6" steel pin) back in he bent over. That's when he felt the pain in his lower back. Strain - low back.

EMPLOYEE WAS CHIPPING SLAG INSIDE THE KILN. SLAG ENTERED EMPLOYEES RIGHT EYE. HE DID NOT REPORT IT UNTIL THE NEXT MORNING WHEN HE SOUGHT MEDICAL TREATMENT.

Employee installing air slide in place when his hand got pulled between the flanges. EE rec'd 3 stitches on middle finger.

Employee removing top of airslide when his arm slipped back causing him to hit his elbow on the handrail. He verbally refused medical treatment. On 8/18/06 he complained of swelling and pain in his elbow and requested medical treatment. He was placed on modified duty until 8/23/06.

Lost grip on handhold while stepping up on bearing housing to take measurements. Fell onto pipe and uneven footing causing sprain of left ankle and bruise to left shoulder. Sprain of ankle necessitated limited walking for the first few days after incident.

While checking links and bucket on the bucket elevator, a steel pipe used to secure the chain and bucket flipped hitting employee on the head. The employee received 9 stitches on the head.

Employee manually unloading tires from semi-trailer reached up to pull down tire and legs collapsed pulling tire down with him while falling.

Hitting side of chute, smashed ring finger between sledge handle and fin on ball hopper chute. Employee not watching clearances.

Forklift operator lifting k-rail to release lock pin, Laborer reached for pin when it moved before given clearance by operator to remove it. Finger pinched in between rails.

EMPLOYEE WAS STACKING 94 LB. BAGS OF POOL SAND & HE FELT PAIN IN HIS SHOULDER.

Employee was leaning on shelf in the warehouse with his head resting on his arm. He lifted his head up, spun around and fell, striking his head on the shelf, resulting in a cut over the right eye requiring stitches.

Employee was cleaning above area of stairs. Walked down stairs, stepped on clinker pile made from cleaning, slipped and sprained right knee. Darkness in area contributed.

In darkness walking on east side of mill/kiln, he slipped on thin coating of mud on concrete slab. Mud was same color as slab, so not easily seen in the darkness.

Employee tripped on the edge on a floor matt in his work area and fell to his knee. He has a strained (or sprained) knee and was released to modified duty with the use of a knee brace. He will perform modified work in his regular classification until released for full duty.

Driving down hill Sweeper gained speed attempting to brake, brakes failed. Used Ebrake, operator's adjustment of handle being too loose. When fail to stop, drove into hillside. Seatbelt in use. Driver hit forehead on steering wheel, knee on column. Minor front end damage to sweeper.

Employee complained of stress caused by his job. He stated that he is on medication for high blood pressure. He stated that he can not work any overtime and whenever that subject comes up, it stresses him and his blood pressure goes up. We don't yet know if this is indeed work related and are awaiting evaluation and determination. We may want to contest this claim.

EE was passenger sitting in rear seat of 4-door truck. While driving from Service Yard, driver fell asleep and the vehicle idled into a tree on the left side of the road. Impact with back of front passenger seat bruised EE's knee (only reportable injury). Damage to left front of vehicle & windshield. Minor tree damage sustained.

While unloading Bauxite from a train car EE slipped on chunks of white bauxite that had accumulated on the grizzly grating during the process of clearing a clogged chute on the train car..

While entering elevator car, outside door closed on right hand as injured reached to close the door smashing right thumb between door and jamb.

Employee was opening a heavy door on a preheater vessel (kiln was down). A pin was missing from the door hinge that allowed it to fall, striking the employee on the top of the right foot. Employee was taken to hospital for ex-rays and evaluation and it was determined that he suffered contusion/herniation/abrasion to the top of the right foot.

Employee stated he was stacking bags with no energy (very tired that day) & thought he had pulled a muscle. EE HAD HERNIA REPAIR SURGERY ON 6/1/05.

Air Lancing Hopper, Air pressure caused loss of balance. When stepping back to brace, his right foot slipped past edge of catwalk. Frame edge poked inner side of his right knee.

Cut finger while using a knife to cut lunchmeat. Finger was cut to the extent that it was necessary to seek medical assistance. Stitches were required..

Employee, not paying attention to safety precautions, bruised his hand. No impairment or broken bones.

TIGHTENING BOLTS IN OSEPA INLET LOUVER AND STRAINED SHOULDER. *EMPLOYEE DID NOT SEEK MEDICAL ATTENTION AT TIME OF INCIDENT NOT REPORTED UNTIL RECENTLY.

While Air Lancing the Bauxite feeder with a stretched out position, employee felt a pull in the lower back while making a twisting movement.

EMPLOYEE WAS HELPING A BAG ALONG A CONVEYOR AND FELT A POP IN HIS ARM.

Employee was involved int he process of are gouging out cracks when object flew into his right eye causing them to be embedded.

Employee fell through the grizzly, a grid was missing.

Ee was part of a team relaying 67# plates from the floor around a corner & up stairs to a silo. Ee was at the past of the stairs passing plate from one person to another. He felt a pop on the right side of his chest after the 7th piece. It was at the end of twisting with the plate. He dropped the plate & was given 1st aid & transported to ER.

REPAIRING PRIMARY CRUSHER, A LIFTING TASK. A PIECE GOT CAUGHT & JAMMED. USING A HOIST & COME-ALONG TO AID IN LIFTING OPERATION, THE PIECE LOOSENED UP & SLID INTO PLACE & THE WORKER HAD HIS FOOT IN THE "SOCKET". HE RECOGNIZED HE WAS IN ERROR AFTER ALREADY BEEN INSTRUCTED.

A CREW WAS WORKING INSIDE OF CYCLONE, ONE WORKER WAS RECEIVING A 10'SCAFFOLD LEG TO PLACE INSIDE THE CYCLONE WHEN IT CAUGHT ON THE INSIDE RING. AS HE PULLED IT AWAY FROM THE RING A LARGE PIECE OF REFRACTORY BROKE LOOSE. INJURED EE WAS WORKING BELOW THIS WORKER AT THE TIME & WAS STRUCK BY THE FALLING REFRACTORY ON HIS RT. THUMB.

EE STATES HE GOT OUT OF CAB AND LEANED ON AIR CUTTING TOOL AND IT RELEASED AIR, CAUSING LACERATION TO LEFT ARM REAR ELBOW.

EMPLOYEE GOT HIS GLOVE CAUGHT IN A MOVING CONVEYOR.

USING BREAKER BAR TO UNCLOG FEEDER - RIGHT MIDDLE FINGER WAS SMASHED BETWEEN BAR AND RAIL.

EE HAD A DISPUTE WITH HIS SUPERVISOR ABOUT MISSING ITEMS. EE HIT THE DOOR WITH HIS HAND.

WHILE JACKHAMMERING FOR A PERIOD OF TIME - HANDS WENT NUMB

WHILE REPLACING LINERS IN RAW MILL PRIMARY COMPARTMENT, TURNED LEFT ANKLE. STEPPED ON MILL BALL.

EE JUMPED ONTO SCAFFOLDING FROM THE NOSE RING CASTING, CAUSING INJURY TO RT ANKLE (SPRAIN).

EE GETTING A TOOL FROM OUT TOOL CRIB, TOOL BUMPED A PIECE OF THICK WALL 4"PIPE, WHICH SUBSEQUENTLY FELL OVER, STRIKING EE ON TOP BONES OF RIGHT FOOT.

EE USED POOR LIFTING TECHINQUES AND CAUSED STRAIN TO MUSCLES OF LUMBAR SPINE.

EE WAS WIRING UP 4FA13 AT MOTOR JUNCTION BOX. WHILE CLIMBING OUT, HE LOST HIS BALANCE AND FELL BACK INTO THE HOLE, TWISTING HIS RIGHT KNEE.

SMALL CUT BACK OF HEAD AND A LARGE GOOSE EGG, CHECKED THOROUGHLY, TREATED AND RELEASED FROM MED CENTER AND RETURNED TO LIMITED DUTY 5-27-03.

EMPLOYEE STEPPED DOWN FROM ONE STEP TO ANOTHER AND TWISTED HIS RIGHT ANKLE.

EMPLOYEE WAS A PASSENGER IN A MANLIFT CUTTING BOLTS OFF THE RAW MILL. THE OPERATOR MOVED THE MANLIFT. THE BASKET CONTACTED BOTTLES CAUSING THE BASKET TO JERK CAUSING THE EMPLOYEE TO HIT THE RA IL. INITIAL DOCTOR REPORT WAS VERY UNCLEAR AS TO THE STATUS OF EMPLOYEE RELEASE. THE EMPLOYEE LEFT PROJECT RETURNING HOME TO DALLAS. DOCTORS RELEASE WAS UNCLEAR/FULL RELEASE OBTAINED IN DALLA

DRIVER WAS ACCESSING TOP OF TRAILER TO OPEN COVERS. HE FELL FROM TRAILER WHEN HE ATTEMPTED TO JUMP TO THE TRAILER PARKED 3 FEET AWAY FROM THE END OF THE EXTENDED ACCESS STAIRS.

EE FELT A SHARP PAIN ON RIGHT GROIN AREA WHILE PICKING UP HIS TOOL BOX.

DRIVING ON ROUGH ROADS ON THE EXPANSION NEAR THE5080 ELEVATION CAUSED JARRING OF THE NECK.

EMPLOYEE WAS CLIMBING UP THE STAIRS TO TOP OF 4SS4 WHEN HE FELT SOMETHING HITHIM IN THE RIGHT EYE.

UNKNOWN AT THIS HIME SUPPOSED HE WAS REPORTED BACK TO HIS PRIMARY CARE PHYSICIAN BY THE ER. DR WAS FAXED EE HE HAD A ASHMASED BACK MUSCLE LOW TORN MUSCLE AND THAT HIS DR WAS RELEASED HIM BACK FOR WORK PREMATRELY EE ON DISABILITY FOR SKIN RASH RASH FROM THE AIFALY FROM THE MINE FOR TWO OR THREE MONTHS.

EE WAS TRIMMING SCREEN PANEL TO FIT. KNIFE SLIPPED AND CUT LEG ABOVE RIGHT KNEE.

EE WAS REMOVING THE TAIL PULLEY FROM 7BC1Q. WHEN HE REACHED OVER AND PULLED ONT HE TAIL PULLEY EXPERIENCING A SHARP PAIN IN HIS LOWER BACK.

BURNED RIGHT HAND WHILE PERFORMING POKEDOWN OPERATION. HOT MATERIAL BLEW BACK AT HIM FROM VESSEL AND BURNED HIS HAND THROUGH HIS GLOVE.

THROWING SAFETY CHOCKS OFF OF ENGINE COMPARTMENT, LOST BALANCE, FELL 10' TO THE GROUND. LANDED ON HIS FEET AND ROLLED.

EMPLOYEE DROPPED TOP TRAY OF TOOL BOX(FULL OF TOOLS) INTO BOX AND HAND GOT PINCHED IN BETWEEN CUTTING HAND AT JOINT OF MIDDLE FINGER AND PALM OF HAND.

EE WAS REMOVING A LOOSE TIRE FROM EXTERIOR OF THE TIRE CHUTE. HIS ARM ACCIDENTLY CAME IN CONTACT WITH THE EXTERIOR WALL OF THE TIRE CHUTE CAUSING A BURN TO HIS LEFT FOREARM.

INJURED WAS SETTING UP ACTUARIES ON FAN DECK AT DUST COLLECTION BAGHOUSES. FINISHED ONE AND TURNED AROUND TO CHANGE ANOTHER WHEN HE STEPPED INTO A VOID LEFT BY A PIECE OF GRATING THAT HAD BEEN REMOVED AND NOT REPLACED. INJURED SUFFERED A STRAIN TO THE RIGHT SHOULDER WHEN HE CAUGHT HIS FALL.

EE WAS POKING DOWN WHEN DUST CAME THROUGH THE SAFETY EQUIPMENT INTO LEFT EYE.

EE WAS CARRYING ONE END OF EXTENSION LADDER DOWN STAIRS AND AROUND CORNER BY #1 TIRE OF 4RK1 KILN. LADDER GOT HUNG UP ON PIPING BY #1 TRUNION. EE PICKED UP END OF LADDER THATHAD BEEN RESTING O N HANDRAIL, AS HE DI, LADDER SUDDENLY MOVED. PINCHING FINGER BETWEEN LADDER AND HANDRAIL/

EE WAS CLIMBING DOWN A LADDER, SLIPPED ANDHIT HIS LEFT GROIN AREA.
