Tehachapi Cement, LLC. Metal/Non-Metal

The employee was simply walking through the Maintenance Shop holding a box cutter they had just got out of the vending machine. Upon inspection of the box cutter, they noticed a foreign object (possibly glue) on the blade. They proceeded to wipe the blade off on their left sleeve, when they cut/punctured their wrist area.

EE was working on a machine when a cylinder press that was suspended in the air swung towards EE and pinched EE's right middle finger against the side of the buckets machine.

Carrying empty container of regent acid, stepped down and out of shed, landed on ankle incorrectly.

Employee tripped over object on floor, fell against cylinders and attempted to catch themselves.

Employee notified us 1 month after last day worked that it may be work related. Company received letter of determination on 11/15/24.

Someone had previously overloaded the conveyance, causing it to fault out. 3 total contractors were trapped, for approximately 3.5 hours. Communication was quickly established and maintained.

EE collapsed in the office prior to starting work. EE was diagnosed with a heart condition.

EE was lifting a 30 lb. bucket from ground level to the back of EE's service truck, when EE felt a sudden pain in left, lower back.

The EE was attempting to get an air slide flowing at the truck load out. As EE was in process of opening a door to check for flow, material spewed out and got in both of EE's eyes. EE went home on EE's own accord a bit later. Once home, EE talked to EE's wife, and decided to go to the Doctor, where EE was prescribed pain medication.

Employee was getting into the cab of the crane. Lost EE's grip with the hand on the rail and fell backwards on to the ground. Hitting with EE's right foot first and then landed on EE's back

EE began feeling ill while driving EE's haul truck. Quarry Manager was notified and took EE to the hospital in Manager's truck, where EE was observed and then released later the same day. Results are inconclusive.

Employee was in the process of cleaning the laydown yard. Part of the clean up involved removing some steel plate to take to the scrap pile. As the employee was setting the piece of plate into the bucket of a skid steer, EE felt a sudden pain in the left bicep.

A fire started at the Screening Tower at Quarry. First responders were called immediately. The fire burned for approx. 90-120 minutes. Investigation and repairs are ongoing. Damage is currently unknown.

While cutting the zip tie off a new band-saw blade, employee cut EE's left thumb approximately 1/2 deep (to the bone) across the top knuckle.

Co-worker was repositioning a wear liner plate when it came loose and fell on employee's right foot causing a crushing injury of the right foot. Contusion of the right foot.

Employee was cutting pipe with pipe cutter, cast off piece fell and struck thumb, causing laceration.

Employee was drilling holes in steel plate with a 1/2" shank drill. The bit got hung up and twisted the drill out of hand. The cord for the drill became wrapped around the trigger causing the drill to continue to twist and it caught right pinky finger.

Employee stepped in loose cement, and material splashed up into left eye, causing distress.

Employee (was wearing safety glasses and face shield) was grinding metal when shard found its way into the employees right eye. Weather conditions was very windy and dusty in the afternoon that could have possibly aided to the accident.

Employee reported to work Monday morning with numbness in right hand, attributed to heavy welding and bolt torquing the previous week. He continued to work his normal job until 5/15/15, when he was transferred to a new position to accommodate healing. This created a restricted duty case and the reporting.

Employee working to remove a loader tooth, struck by flying metal chip. Required physician to remove due to placement/size of object.

EE ASSEMBLED SCAFFOLD LADDER TO LEAN AGAINST I-BEAM AND CLIMBED DOWN. THE LADDER STARTED TO BEND - EE GRABBED ONTO I-BEAM JUST AS LADDER COLLAPSED. EE HUNG FROM I-BEAM THEN FELL FROM APPROX. 15 FEET TO LOWER LEVEL. EE LANDED STRIKING LEFT HAND AGAINST LADDER RESULTING IN DISLOCATED JOINTS.

Employee squatted to perform safe back lift, and felt pop in left knee, and subsequent pain.

Employee was using a pry bar to clean screens, and caught his right pinkie tip between the bar and a handrail.

Driver injured when seat of loader broke, pushing her forward in the cab and held her in an awkward position. The welds holding the seat had broken in two spots, allowing the scissor mechanism to force it forward. The driver requested no medical help or treatment at the time of the injury.

Employee (mobile equipment mechanic) was loosening bolts on a loader wear plate. He used his left hand to push the bolt into the recessed sleeve, and his right hand on an impact wrench to loosen the bolt. When he activated the wrench, the bolt spun, cutting his thumb.

Employee cut on forehead by 1/2 inch plastic sampling line, required stitches. EE was working on a kiln sampler probe line. It was a small flexible plastic tube. He left area and when he returned to re-attach it something bumped the line and caused it to move, he was struck by the brass fitting above the eyebrow as he was reaching for it.

Dusty working environment.

EE was working to remove compacted dirt from a pile, when a piece sloughed off, striking him in the lower left leg, pinning him to the deck and twisting his body. EE broke both bones in lower left leg.

Employee is now seeing a physical therapist for soreness and tightness of right knee. pain has been off and on for several months. Seen by doctor in past with no medical treatment provided, however, patient is now seeing a physical therapist for strengthening exercises.

Payroll accountant felt ill, faint, pale, and did not improve upon rest. Ambulance called for transport to hospital. Early tests inconclusive, possible blood pressure medication issue? Upon release from hospital, the patient complained of some pain and sweating, and was subsequently diagnosed with blocked arteries, and remitted for surgery to repair.

Employee approached a haul truck parked at the ready line. He lifted his left foot to step onto the lower step of the access ladder, and felt a `pop' in his right knee. He immediately stopped on the ladder, and waited for the situation to feel better. After a few moments, he continued on up the ladder, and completed the work on the door, and then exited the truck.

Employee was rebuilding cardox tubes, and escaping gas released and blew into his face and eyes, Diagnosis: corneal conjuctivitis. EE given prescription for potential infection. EE was sent back to finish shift with no further care.

Employee was cutting a piece of 4 inch drill steel when the piece fell off the support and struck the employee's left foot, fracturing the second metatarsal bone.

Employee walking down stairs, felt left knee pop.

Employee complained of left wrist/elbow pain and soreness.

Ee is undergoing surgery on this date for an umbilical hernia. The hernia occurred October 21st. EE received NO medical treatment and NO work restrictions. The employee returned to work the same day as the hernia occurred, and has been working ever since, until 12/14. On 12/15, the employee was laid off due to lack of production, and is having surgery.

EE lifting steel plates/parts when he felt a hot burning pain in left inguinal area. Over the next few days, the pain increased and the employee reported it and sought medical attention. Employee underwent a layoff (planned production shutdown) during this time and was not eligible to work until 1/4/10, when recalled. and he was unable to due to medical restrictions.

Unknown. Employee reported his left arm/elbow was tingling and painful. Stated he had this condition before, but never this bad.

While loading clinker dust with a skidsteer, dust blew into EE's eyes, causing some light irritation and redness. By end of shift, EE reported his eyes were itchy/watery/sore/red, but felt okay to drive home. Next morning, EE reported eyes were still sore, inflammed/red, and was sent to doctor for check.

While working at the undergrate deck, ee got clinker dust in eyes. He raised up suddenly from a crouched position, knocking his hardhat off and striking head on overhead beam. He continued raising up to get out of the area, and struck his head on a steel beam on the ceiling. He received a 1.2 inch laceration to the forward central skull that required 4 sutures.

EE was cutting plastic sheeting with a knife when he slipped, cut himself slightly in the left knee.

Employee was walking towards plant storeroom, and slipped on ice, twisting right knee.

Employee was cleaning the spitzer in the finish mill when a spec of cement blew past his PPE and got caught under his eyelid. He was seen by a doctor who irrigated the eye and removed the dust spec. A mild corneal abrasion was noted from the effects of the cement and irrigation.

While attempting to clean material under the A silos, the employee got cement dust in his eyes. He immediately sought water to wash the dust out, and then came to the plant for more rinsing. He was sent to the hospital for further decon and evaluation.

Employee was exiting a loader, and lost his balance on the last step. As he fell, he caught himself sharply with his right hand on the handrail, and his right arm and shoulder took the shock of his fall.

EE received a foreign object in his right eye when he was walking through the plant, and was caught in a dust devil/windstorm. He was wearing safety glasses, but the wind blew a fragment foreign object into his right eye.

EE was using a knife to open a cardboard box containing filters. His hand slipped, while cutting the box, and he cut across his left index finger, between the 2nd and 3rd knuckles.

IE severed left index finger while assisting in the removal of hatch door in the pre-heater tower.

Failure to watch path, poor housekeeping left objects in path, failure to be aware of surroundings

Doing demo to a chute - cut a piece loose and it flipped the wrong way and hit him in the hard hat.

Employee was using a rivetbuster, when the tooth got caught in the material. As he tried to remove it, the rivetbuster lunged forward catching his right ring finger and pinky finger between the rivet buster and material causing a 1 cm laceration to his 5th (pinky) finger.

Wind blown door to office closed on employee's left ring finger tip.

A slip and fall on stairs.

EE stumbled on catwalk, struck middle finger right hand reaching for hand hold, breaking finger tip.

EE was disconnecting coupler between electric motor and water pump. When coupler was disconnected, water pressure jarred impeller of water pump backwards and EE was struck by a coupler half.

Employee was attempting to patch leaving vessel with epoxy mix. Free lime solution was in vessel, some type of chemical reaction took place creating pressure in vessel, releasing liquid. Liquid splashed on employee.

Miner was cutting on feed end of ball finish mill with a torch. A flamable material flashed from cutting slot. The material was unknown. Miner received first degree burn and 2 small 2nd degee burns of left side of face.

Mild back strain from operating bobcat. Reported next day, visit to Dr on day reported.

EE was "sucked" by super sucker hose in crotch. Has suction bruise and mild back strain.

Dust in right eye. Antibiotics in eye.

Wind-blown dust in right eye. Antibiotic in eye.

Wires fell out of cardox. EE removed PPE to reattach wires. Hot material spilled onto unprotected sleeve. Sleeve acted as catch area trapping hot material on arm. EE misconduct caused injury.

Hatch fell on Lt pinky finger, laceration.

Standing/kneeling on uneven ground, possible left knee injury. Being treated as first aid but will become reportable with MRI exam.

Heat stress. Employee did not eat for 24 hours and no fluids for 14 hours prior to starting work.

Water spray leaked into hot material. EE tried to open door. Hot material flushed into employee's overalls. Hot material burned his back(blisters). Employee had a poor position to open the door, contributing to injury.

Cut left forearm, cutting rope, pulling knife towards the body, not cutting away.

Wind blown dust into right eye.

A 30# steel ring fell on thumb.

EE energized a powered hacksaw with his finger against drive wheel. EE tested positive for marijuana; suspended; further investigation for level of drug; return TBA.

Closed door on hand; miner not paying attention to what he was doing.

Wind blown cement dust.

DUST IN LEFT EYE.

FK pump arm was being checked, Arm dropped and pinched finger finger, right hand. Small laceration -1 stitches.

7000-1 FILED AS PRECAUTION. EMPLOYEE HAS A RASH THAT THE SOURCE OF HAS NOT BEEN IDENTIFIED. CONSULTED WITH LOCAL MSHA OFFICE AND WAS ADVISED TO FILE 7000-1.

FOREIGN BODY IN EMPLOYEE'S EYE. BEING REPORTED AS REPORTABLE WITH UNKNOWN PRESCRIPTION CHARGE RECEIVED IN MAIL YESTERDAY.

EE WAS KNOCKED OFF A LADDER BY A FALLING CEMENT CHUNK. (EE WAS ON FREE-STANDING LADDER UNDER SILO, CLEANING DISCHARGE CHUTE. EE FELL APPROX. 2' AND HIT HIS SHOULDER AGAINST SUPPORT WALL OF SILO, CAUSING INJURY. FIRST AID ONLY - X RAY NEGATIVE; NO PRESCRIPTION DRUGS GIVEN.)

EE OPENED DUST COLLECTOR, HIGH WIND PRESENT, DUST INTO EYES.

EE & COWORKER WERE WORKING USING A CUTTING TORCH TO REMOVE SUPPORT PIPE. PIPE WAS HEAVIER THAN ANTICIPATED, SLIPPING THROUGH COWORKER'S GLOVES AND HITTING EE ON THE FOOT. (EE WAS HOLDING THE CUTTING TORCH. PIPE WEIGHED APPROX. 12 LBS.)

EE WAS WORKING INSIDE RAW MILL WHEN COWORKER HIT CERAMIC STONE WITH HAMMER AND A PIECE OF THE CERAMIC STONE BROKE OFF AND HIT EE IN HIS FACE (CHEEK). (EE WAS WEARING SAFETY GLASSES. FIRST AID TREATMENT ONLY; BUTTERFLY BANDAGE ONLY - NO STITCHES.)

While descending the moving stairs of the stacker reclaimer, a hidden rock moved forward, ee stepped on it, twisted his ankle and fell.

WHILE COMING DOWN THE LADDER OF A 50T EUCLID HAUL TRUCK, THE EE MISSED THE LAST STEP, FELL AND TWISTED HIS BACK. ACUTE LUMBAR STRAIN.

WHILE CLEANING UP A COAL SPILL, THE EMPLOYEE WAS CLEANING THE RAILS & WHEN APPROACHING THE UNLOADER THE BUCKET HIT A RAISED PART OF THE GRIZZLY. THIS THREW THE EE INTO THE SAFETY BAR OF THE BOBCAT HITTING HIS CHEST, WHERE HIS RADIO MIKE WAS.

EE WAS LEAVING THE CAB OF HIS LOADER AND CLOSED THE DOOR ON HIS HAND.

EMPLOYEE LIFTED A 5 GALLON BUCKET OF OIL AND FELT PAIN IN THE LOWER BACK AND DOWN RIGHT LEG.

Employee was removing large pieces of hardened cement. After removing a large piece, fine powdered cement flushed into ee's face and eyes. The right eye received an abrasion.

EE WAS REMOVING A FILTER COVER TO REPLACE A FILTER. WHILE DOING THIS, HE FELT A POP IN HIS LEFT SHOULDER. THIS INJURY WAS NOT REPORTED TO HIS SUPERVISOR UNTIL 3/4/04 AND EE WAS NOT SENT TO A DOCTOR UNTIL 3/9/04. AT PRESENT, EE IS ON MODIFIED DUTY, BUT MAY BECOME AN LTA.

WHILE MANUALLY ACTIVATING AN AIR CANNON, A CARDOX PORT CAME OFF AND HOT KILN FEED CONTACTEDEE'S RIGHT HAND.

WORKER ON LOWER PORTION OF CATWALKS, LOOKED UP TO ANSWER A CALL AND MATERIAL FELL FROM GRATING ABOVE TO THE FACE OF WORKER. DIRT GOT UNDER SAFETY GLASSES INTO EYE (RIGHT).

EE RECEIVED A WELDING FLASH BURN TO BOTH EYES WHILE OBSERVING RAW MILL TABLE WELDING OPERATIONS.

EE STEPPED OUT OF GOLF CART AND FELT PAIN IN HIS LOWER BACK AND LEGS.

EE WAS GOING TO HIS VEHICLE AND TRIPPED OVER A PARKING BLOCK, INJURING HIS LEFT ANKLE.

EE WAS CLIMBING THE LADDER OF HIS 777 HAUL TRUCK WHEN HE FELT A PAIN IN HIS LOWER BACK.

THIS EE INJURED HIS KNEE ON 8/10/01. HE HAD A TOTAL OF 168 DAYS OFF AND WAS TRANSFERRED TO A TRUCK DRIVING POSITION ON 4/3/02. HE STATED THAT HIS KNEE STARTED SWELLING SOMETIME THE PREVIOUS WE EK. HIS DR. HAS PLACED HIM ON LIGHT DUTY.

EE WAS DIAGNOSED WITH CARPAL TUNNEL SYNDROME ON 11-17-02. HE HAS BEEN WORKING WITH A FULL DUTY RELEASE UNTIL HIS SURGERY. EE STATES HIS SYMPOTOMS BEGAN IN 1997.

EE WAS CUTTING A PLASTIC COVER ON A FUEL NOZZLE WHEN KNIFE SLIPPED, CUTTING HIS FINGER.

EE WAS OPERATING A 992G LOADER. WHILE DIGGING, BUCKET CAME IN CONTACT WITH A LARGE ROCK CAUSING REAR OF LOADER TO COME OFF THE GROUND. WHEN REAR OF LOADER RE-CONTACTED THE GROUND, EE WAS BOUNC ED IN THE SEAT CAUSING LOW BACK PAIN. EE IS SHORT & LIGHTWEIGHT & MUST ADJUST SEAT TO LOWEST POSITION TO REACH PEDALS. BY DOING THIS CAUSES THE SEAT TO LOSE ITS SHOCK ABSORBING CAPABILITIES.

EMPLOYEE WAS CLIMBING HAUL TRUCK LADDER. EMPLOYEE SLIPPED AND FELL CAUSING A SPRAIN TO THE LEFT KNEE.

EE WAS TROUBLESHOOTING PLUGGED DUST COLLECTOR. HE THEN OPENED THE INSPECTION DOOR AND STUCK HIS HAND IN WHILE ROTARY AIRLOCK WAS STILL RUNNING. ROTARY AIRLOCK CONNECTED WITH THE TIP OF HIS RIG HT MIDDLE FINGER CAUSING AN AVULSION.

RE-OCCURRENCE OF AN INJURY REPORTED ORIGINALLY AS 2/27/00. EMPLOYEE HAS HAD CONTINUAL PROBLEMS. ON 7/18/02 HE FELT HIS LEFT KNEE POP. HE RETURNED TO HIS DOCTOR AND HAS BEEN OFF WORK SINCE.

EMPLOYEE WAS SHOVELING OFF MATERIAL FROM AN OVERLOADED BELT CAUSING LOWER BACK STRAIN.

EE WAS ASSIGNED DUTY OF HARD FACING CRUSHER ROTOR BARS. ROTOR WOULD NOT TURN. EE PULLED UP ON CRUSHED ROTOR AND FELT A POP IN LEFT KNEE. ORIGINAL DIAGNOSIS - KNEE SPRAIN. SURGERY REQUIRED.

EE WAS PUSHING AND PULLING AN ANALYZER PROBE CAUSING A LOWER BACK STRAIN, WHILE TRYING TO REMOVE PROBE.

EE WAS CALLED OUT TO RECALIFBRATE A CONVEYOR BELT THAT HAD BEEN REPLACED. EE SLIPPED ON SPILLED CLINKER FROM BROKEN BELT THAT HAD FALLEN ONTO CATWALK.

EMPLOYEE WA CONDUCTING AN INSPECTION OF THE B4 MILL BUCKET ELEVATOR. HE WAS LOOKING UP WHEN CEMENT DUST PARTICLES DROPPED INTO HIS RIGHT EYE.

EE WAS PLACING A HYDRAULIC JACK UNDER A PLATE OF STEEL AND FELT PAIN IN HIS LOWER BACK.

EE WAS ENTERING CAB OF 992 LOADER, STEPPED ON SEAT BELT LATCH AND TWISTED HIS RIGHT KNEE.

EE JUMPED DOWN OFF TRACK MOBILE AND TWISTED LT. ANKLE.

EMPLOYEE OPERATING LOADER WAS MOVING STOCKPILED MATERIAL FROM ONE AREA AND THEN DUMPING IT OVER THE EDGE ON AN ESTABLISHED STOCKPILE. BEFORE BE-GINNING TO DUMP, THE OPERATOR PUSHED OFF THE EDGE AND THEN BEGAN DUMPING MATERIAL OVER THE EDGE(PART ON TOP AND THE REST GOING OVER THE SIDE). THE BANK UNDER THE RIGHT TIRE GAVE WAY. THE LOADER SANK AND THEN ROLLED OVER. NO INJURIES.

EE TRIPPED OVER PIECE OF ANGLE IRON LEFT ON CATWALK, CAUSING A LACERATION AND BRUISE TO RIGHT KNEE.

EMPLOYEE WAS CHANGING A DRILL BIT ON AN INGERAOLL RAND DM-45E DRILL AND FELT PAIN IN TOWER BACK AND RIGHT LEG.

EMPLOYEE WAS DUMPING MATERIAL INTO A SMALL DUMP TRUCK WITH A BOBCAT. WHILE RAISING THE BUCKET AND MOVING FORWARD TO DUMP, THE BOBCAT STARTED TO BUCK (ROCK BACK & FORTH), CAUSING THE BOBCAT TO TIP FORWARD. IT APPEARS THAT THE EMPLOYEE LET GO OF THE CONTROLS AND REACHED OUT, PLACING HIS HANDS ON THE SIDE OF THE DUMP BED. THE CAB OF THE BOBCAT THEN CAME DOWN ON THE EMPLOYEE'S HANDS.

EMPLOYEE WAS WORKING INSIDE RAWMILL, ADJUSTING TYRE. EMPLOYEE SLIPPED ON SAND & OIL CAUSING EMPLOYEE TO SLIP AND TWISTED RIGHT KNEE.

EE HAD JUST COMPLETED THE POSITIONING OF A SCAFFOLD PLANK & BEGUN TO RAISE & TURN TO PREPARE TO RECEIVE NEXT PLANK FROM CO-WORKER. THE CO-WORKERHAD PREPARED THE RISING PLANK TOO CLOSE TO EE BACK WHICH RESULTED IN THE EE'S LOWER LIP TO STRIKE AGAINST THE PLANK AS HE TURNED

EMPLOYEE WAS SHOVELING MATERIAL OFF A CATWALK. EMPLOYEE TURNED WHILE LIFTING SHOVEL AND FELT A POP IN LOWER BACK CAUSING A LOWER BACK STRAIN.

EE WAS BREAKING A BUILD-UP OF MATERIAL WITH A PICK. WHILE BREAKING MATERIAL EE HIT A SCREEN GUARD, CAUSING THE PICK TO BOUNCE BACK STRIKING HIM IN THE BACK.

EE WAS USING A PRY BAR TO REMOVE DEBRIS FROM SK VALVE. BAR SLIPPED, CAUSING EE'S RIGHT KNEE TO STRIKE FK PIPE. NOTE: AT TIME THSI WAS CONSIDERED A FIRST AID INCIDENT, IN FACT EE PALYED RAQUET BALL THAT EVENING. WE QUESTION VALIDITY, BUT ARE REPORTING SINCE SURGERY IS REQUIRED.

EE WAS CLEANING UP SPILLED MATERIAL W/BOBCAT. MATERIAL CONTAINED OIL/GREASE. EE DISMOUNTED FROM BOBCAT AND STEPPED ON BUCKET WHICH HAD GREASE/OIL/DIRT CAUSING HIM TO SLIP AND TWISTED HIS RIGHT ANKLE.

THE DAY WAS WINDY, EMPLOYEE WAS LEAVING FOR THE DAY. HIS HAT BLEW OFF IN PARKING LOT. EMPLOYEE FELL TRYINGTO RETRIEVE HIS HAT, LANDING ON RIGHT ELBOW. THIS INJURY WAS NOT REPORTED UNTIL 1-22-0 1.

THERE IS NO EXACT INJURY DATE. THIS EMPLOYEE HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROME. HE IS A MECHANIC IN OUR SHOP. HE HAS BEEN HAVING TROUBLE SINCE 1982. HE HAD SURGERY FOR THE LEFT HA ND AT THAT TIME. HE NOW HAS HAD SURGERY ON THE RIGHT HAND.

LIFTING SUPER SUCKER HOSE, FELL INJURING EMPLOYEE'S LOWER BACK. HOSE WAS PLUGGED, EMPLOYEE WAS ATTEMPTING TO LIFT HOSE AND SHAKE OUT MATERIAL.

MOVING PIECES OF BROKEN CONCRETE, EMPLOYEE SUFFERED STRAIN TO THE RIGHT SHOULDER.

PULLING SUPER SUCKER HOSE OUT OF A PORT. LIFTED 6" P8PE, FELT A POP IN THE CENTER OF HIS BACK.

ADJUSTING BELT SKIRTING. STRAINED LEFT HAND WHILE ADJUSTING SKIRTING. HAND WAS IN AN AWKWARD POSITION.

A CREW WAS DIRECTED TO FOLD THE JIB OUT ON AN 80 TON CRANE. THEY BEGAN TO PULL THE PINS ON THE STOWAWAY JIB WITHOUT INSTALLING THE HINGE PINS THAT PIVOT THE JIB OUT. AS A RESULT, THE JIB FELL OFF THE SUPPORT AND HIT THE EMPLOYEE ON THE WAY DOWN. IT CAME TO REST ON THE OUTRIGGER.

EE WAS WORKING ON A DUST COLLECTOR ON TOP OF THE CEMENT STORAGE SILOS. HE STEPPED ON A PILE OF CEMENT DUST, SLIPPED AND TWISTED HIS ANKLE. HE WILL BE ON MODIFIED DUTY FOR 2-3 WEEKS.
