Employee, with the help of coworker who was on the bed of a pickup truck, were unloading a 123 lbs. box from the pickup bed. Employee was holding the corner of the box with their right hand when the box was dropping off the truck. As the box was dropping to the ground, employee stopped the box from falling and was easing the box to the ground when felt pain in their right arm.

Conditions were normal, EE lifted a floor pan into place and felt a pop in EE's right side of chest

A forklift operator exited the forklift and attempted to disengage the catch lever manually. The employee placed left hand on the forks to stabilize the hopper skid plate. The employee disengaged the hopper's catch lever causing a weight transfer to the front. The hopper's rear skid lifted crushing the employee's index and middle fingers between the fork and skid.

Dozer(D10) operator was pushing material. Operator was preparing to push a ramp to get off from a shot. Operator observed material falling off from the highwall. Operator tried to back up but material hit the Dozer. The material shattered the cab door window and minor damage to the loader engine area. The operator drove the Dozer to a safe location before notifying EE' team leader.

Employee of 6 months was working on crusher feeder motor the employee removed the guard, and there was more guarding around it so, they had to stretch. Employee was on their knees reaching arm-length away to diagnose and repair the motor. They stayed in that position for approximately 20 minutes. The pain increased gradually, it turned mild after they stood up.

Loading a shot, pulling on a hose, hurt shoulder (1st day of treatment 2/22/24).

On Thursday Jan 11th, 2024, a mill employee of 2 years and 11 months of experience went to check on CBE 74 by the tunnels at 03:00am. When the employee entered the door to the drier building, a gush of high wind pushed the door getting their fourth finger on the right hand slammed between the door frame and the door.

A technician was performing EE's lube routes Sunday, 17-Dec-23 at approximately 2 p.m.. EE was carrying a bucket with EE's lube gun. While walking on the transition point, EE slipped and fell to EE's knees. EE attempted to break the fall with the arm carrying the bucket. EE felt a "pop" in EE's right shoulder.

While removing a product line on a railcar the ee pulled the line hitting a tooth, causing it to chip. Temp. agency reported this incident as a first aid. It was not until EHS manager spoke to the ee on the week of 2/5. EE mentioned the chipped tooth was repaired at the dentist. Since it was a temp employee from an agency, EHS contacted MSHA for advice on behalf of SMI.

On Wednesday October 4th, 2023. A swing shift employee of 2 years and 3 months in the company suffered a back sprain while descending from the G-loader at the beginning of their break at 22:30. As the employee stepped down the ladder, their left foot slipped and went through the last and 2nd to last rungs making them loose grip of their left hand from the handrail.

Two maintenance technician were replacing a rotary lock motor. As the motor was stuck a sledge hammer was used to tap loose the motor. However when one of the technicians struck the motor with the sledge hammer not realizing the other technician apparently had placed left hand under the motor to support it when left thumb was struck.

While operator was pushing material, the dozer began to slide sideways away from the highwall. Eventually, leading the vehicle to slide down and roll over once and back upright. Immediate drug and alcohol testing was performed. Operator was seen by a physician. Physician did not find any injuries. Results returned positive for alcohol.

A temp EE finished loading a 1000 lb. super sack when the EE noticed the super sack started to slide off the forklift forks. The EE thought they tilted the forks back. The EE then grabbed onto the super sack and felt pain in their lower back. On 2-15-22 temp EE was evaluated and given a prescription and a back belt for a lower lumbar strain.

EE reported on 12/6/2021 they felt a sharp pain in their upper back while carrying a bucket of tools up to Pebble Mill #2 blower. On 12/7/2021 EE requested to be evaluated and was diagnosed with a cervical strain; EE was given a prescription and put on modified duty.

EE reported to management on 10-12-21 that during their break they found a bump on their body. EE said they are not in pain but felt the bump and was concerned as they did not recall one there prior to 10-12-21. On 10-21-21 EE was diagnosed with an inguinal hernia.

Employee was manually packaging 50lb. bags onto a pallet for 7 hours. Next day EE's right elbow was sore. After a few weeks, employee felt more pain. Employee reported the incident to the manager on 7/12/2021.

A Scalehouse employee reported on EE's shift at 6:45am. The employee decided to take a break outside the scalehouse area, and leaned up against a metal chain connected to a trailer building and scalehouse structure. As EE leaned on the chain, the wood trailer structure gave way, causing the employee to fall on EE's back with the chain wrapped around EE's lower left leg and ankle.

A temporary haul truck driver (#54) was climbing up the steps to enter the drivers seat, when then opened the door (drivers side) to enter. The operator was holding the door handle, and as they opened the door, they struck the adjacent railing, and smashed right index finger into the railing, causing a non-displaced fracture.

Employee was working in the Packing Center, specifically at the pallet re-stack corner. Employee was going to repair a damaged pallet of 50 lb bags. Employee pulled out a bag to dump it in the hopper when they felt pain in their back.

A Temporary employee was in process of loading an incoming truck, when the mast chain drive became loose by hitting a bump on the roadway. They decided to try and put the chain drive back in place by placing their left hand inside the chain pulley, when the pulley snapped back in place, catching the left finger, causing a fracture.

The Haul Truck Operator driving a 777D unit was traveling with a load to the Quarry Crusher, when the operator veered from roadway, & traveled through an approximate 5'7" berm. The truck rolled down an approximate 15-20 embankment & landed upright at the next roadway area. The operator allegedly nodded off & lost control of the haul truck.

Truck driver having truck positioned onto above ground scale to load product into tank trailer. As driver exited truck cab and stepping onto scale railing (flat surface) EE held a 10 foot aluminum pole to position spout, lost footing and fell onto railing. Result was fractured rib. Doctor has reduced EE's duties to modified duty at desk after one day off.

Employee was using a forklift and noticed that there was slack in one of the chains which raise and lower the forks and decided to adjust the chains. The Operator tried to realign the chain onto the pulley, when the chain moved and caught the right hand between the chain and the pulley. This resulted in avulsion to the right index finger

Mechanic was in the process of removing the sump lid to gain access to the sump by using an electric winch to move the 400 pound lid out of the way. Upon freeing the lid, it moved and caused left hand to be caught between the edge of the lid and an electrical panel mounted to the wall, resulting in a contusion type injury requiring stitches

Temporary worker alleges ee was injured while driving a haul truck through a switchback turn.

Employee was driving a haul truck when EE suffered a back injury.

Conflicting story, not sure this is a worker's compensation.

Employee reported discomfort and knot in groin area at end of shift.

A worker had climbed on top of CBE14B in order to inspect the back side of the middle feed chute. As he was descending, his foot caught on the safety pull cord. This caused him to lose his balance and he jumped to the floor from a height of 33". He wound up on his feet on the tunnel floor but the jar resulted in a muscle spasm in his upper back.

Two electricians were installing new fluorescent lighting into suspended ceiling gridwork. The injured worker was standing on the floor after handing a light assembly to the second electrician who was on a stepladder. In trying to maneuver the assembly, the light fell. A corner of the light struck the injured employee and caused a laceration of the right, inside forearm.

Sprained lower back muscles Demoing access platform. While picking up and moving grating felt pain lower back. Reported injury refused medical. Continued rest shift. Not reportable. Only worked plant on 4/22/2013. 4/25/2013, at work in shop, not at mine, requested to have back checked. Sent to Doctor ,placed on restricted duty 4/25/2013. Now reportable.

Worker noticed discomfort in groin area and thought it should be checked out.

Temporary staffing employee was using a razor knife and cutting excess tape from a vacuum hose when he accidentally cut his left thumb. He sustained a superficial laceration to the left thumb.

Worker has been complaining of left problem. He was placed on restricted duty on 11/07/2011.

Employee was adjusting carousel brake on an overburden drill. He was leaning against the carousel with his left hand as he applied an impact wrench to the brake adjuster. The carousel moved enough to pinch his left hand. He jerked his hand out and a rough surface caused a partial skin avulsion to his palm. The doctor decided to apply stitches.

Employee was helping shovel rock off of a conveyor when he felt a pop in his right shoulder.

Employee was lifting a water bottle onto the water dispenser in the Production lunchroom when he strained his back.

Employee had entered the cab of a haul truck and was sitting in the seat. A wind gust caught the door as he was closing it and jerked his arm and shoulder.

Worker was driving a haul truck when he noticed pain in his shoulder. He had been experiencing this pain for over a year, but it was bothering him enough that he wanted to see a doctor. We have not determined yet if this is work related.

Worker was operating overburden drill near a highwall, when three (approximately 2-3") rocks struck side windows on cab, causing fractures in the glass. The fractures released very small glass particles into the cab which entered the worker's eyes, resulting in corneal abrasion.

Two employees were moving a refrigerator down a stairway. They were using a dolly but the injured employee lost his footing. The refrigerator got away and caused the employee to strike a wall with the back of his head and shoulders.

Worker was on walkway when a gust of wind knocked him off balance. In grabbing a hand rail, he 'tweaked' his back. Incident occurred on 12/22/2008 but worker did not seek medical treatment until 01/05/2009.

Worker was pushing vacuum hose further onto spout when hand was pulled into vacuum hose.

Employee was shoveling material when he experienced pain in lower back.

Employee was walking up an inclined walkway when he felt a pop in his right calf.

Employee was cleaning a photocell on our automated packaging center, when a push arm overextended and caught his finger.

Employee was positioning a drill steel onto a flatbed truck. The steel rolled and pinched his finger.

Employee was handing parts up over his head when he felt a rib strain in his right ribcage.

Worker was removing a 3/8" pipe fitting from a solenoid valve when he felt a pop in his shoulder.

A hydraulic chain wrench was being used to loosen the hammer on an overburden drill. The threaded portion of the end of the chain wrench broke, swung around, and struck the employee in left shin, causing a small puncture wound.

Worker was trying to operate a railroad switch to divert a railcar to a scales for weighing. The switch was hard to operate. Worker tried to kick the switch into place when he pulled ligaments in his knee.

Employee was pushing large metal box into an MCC room when he experienced a strain in calf of left leg.

Slipped on rolling scrap of conduit; strained groin.

Employee was shoveling dry light material. When changed to wet heavy materiel, he felt pain in left thumb.

Reconnecting Guzzler Pipe glove caught pulled into and pinched finger in connector.

Worker was helping mechanics remove screen panel from SCH10 when right index finger was caught between screen and framework.

Employee was bagging 50 lb bags.

Employee was operating a chain jack which was attached to the underside of the floor above where the employee was working. He was sitting on a gate approximately 3' below the point of attachment, not giving him much room to work.

Crew was bull rigging a 10' piece of pipe up two flights of stairs when the jack stand rolled from under the pipe. Employee was holding the pipe with his hand when jack stand started to roll. Employee let go and put his hand around the front of the pipe where his (L) hand was pinched requiring stitches to his 3rd finger.

Employee was walking to the tool room to return some equipment. She had to pass through the fabrication area. She tried to shield her eyes from the grinding, but felt something in her eye. A foreign body was removed; she was treated for conjunctival infection and given prescription medications as treatment.

Employee felt pain in his lower back after pushing on a wrench to loosen an electrical termination.

Employee was placing an empty pallet on a roll table that presents pallets to a palletizer when his finger became pinched between pallet and pallet alignment bar.

Packaging at R8, stacking bags on pallet. Packer stopped filling and bag was falling off saddle. EE turned back around to packer and attempted to catch falling bag (50#). Felt pain in middle of back. He was unable to finish shift due to increasing pain.

Employee was shoveling out an elevator when he felt a pain in lower back.

Employee was placing bags on a pallet when she felt pain in her right shoulder.

Employee was tossing a 50 lb bag to the top of a pallet when he felt a sharp pain in his right wrist.

Threshhold shift hearing loss.

Employee was getting off of forklift when he felt pain in foot.

Employee was lifting and stacking sacked material on Ranger 8 when he experienced back pain.

Employee was rising from a prone position on the floor when he experienced a minor discomfort in his lower back.

Employee had not been performing any lifting when he started experiencing back discomfort. The pain worsened during the remainder of shift. Specific cause is unknown.

Carpal Tunnel Syndrome

Employee was bending over while trying to position a 40-lb saddle on #5 Packer when he felt a needle-like pain in his back. He finished the shift without problem but his back was causing a lot of pain by the time he came back to work on July 13. Doctor diagnosed as an acute lumbar spasm.

Employee was holding a Guzzler hose to remove material from the bottom of a product elevator. The material on the sides sloughed in on the hose. The employee was trying to pull back on the hose to prevent clogging when he experienced a pain in his back. Doctor indicated thoracic spasm.

Employee stepped on uneven floor surface, causing ankle sprain.

EE WAS CLEANING UP A SPILL WITH A SHOVEL WHEN HE EXPERIENCED PAIN IN MIDDLE OF BACK.

Employee was operating packer, filling bags.

EE WAS ONE OF AN 8 MAN CREW WORKING TO REPLACE IMPACTOR BARS. EE HAD CRANE PENDANT IN RT. HAND,LOWERING ROCKBUSTER IMPACTOR ROTOR ONTO SPACERS.EE WAS HOLDING SPACERS IN PLACE WITH HIS LT.HAND AS HE LOWERED THE ROTOR. IN DOING SO, HE PINCHED THE SKIN ON THE BOTTOM SIDE OF HIS LT. THUMB. EE WAS IMMED. TAKEN TO HOSPITAL E.R. X-RAYS SHOWED NO BONE DAMAGE, THUMB CLEANED & SUTURED.

ON 10/12/03, BETWEEN 3:00 & 11:30 P.M., EMPLOYEE WAS STACKING 100 LB. BAGS OF GROUND CALCIUM CARBONATE (LIMESTONE). HE AWOKE ON 10/13 WITH A PROTRUTION IN THE NAVEL AREA OF HIS STOMACH AND CAL LED HIS SUPERVISOR TO REPORT THE PROBLEM. EMPLOYEE STATES THAT HE DIDN'T STRAIN HIMSELF OR FEEL ANY PAIN ON 10/12. HE WAS SENT TO DOCTOR WHO DIAGNOSED AN UMBILICAL HERNIA.

EE WAS ATTEMPTING TO CLOSE A STEP LADDER WHEN HIS LT. INDEX FINGER WAS CAUGHT IN ONE OF THE STABILIZING BRACKETS ON THE SIDE OF THE LADDER. EE SUFFERED A LACERATION THROUGH THE FINGERNAIL JUS T ABOVE THE CUTICLE AND FRACTURED THE TIP OF THE FINGER. EE RECEIVED SUTURES TO THE LT. INDEX FINGER AND WAS PLACED ON RESTRICTED DUTY, LIGHT USE OF LEFT HAND & NO CLIMBING.

EMPLOYEE WAS RE-STACKING A PALLET OF 100 LBS BAGS FULL OF CRUSHED LIMESTONE ON 07/18/03. EMPLOYEE REPORTED TO SUPERVISOR VIA VOICE MAIN AT 10:00 AM ON 7/21/03 THAT HE HAD STRAINED STOMACH MUSC LES STACKING BAGS ON 07/18/03. EMPLOYEE WAS SENT TO COMPANY MD AT 8:00 AM ON 07/22/03 FOR TREATMENT. EMPLOYEE WAS RELEASED TO RESTRICTED DUTY - 10-12 LBS PUSHING, PULLING, LIFTING. STRAINED S

EE REPORTED TO SUPERVISOR AT END OF SHIFT THAT HE HAD INJURED HIS TESTICLES. EE BELIEVES THAT HIS ROUGH-RIDING FORKLIFT HAS CAUSED HIS INJURY. EE SENT TO ER AT 11 PM ON 6/25/03. EE REFERRED TO UROLOGIST AT 9:30 AM ON 6/26/03. UROLOGIST BELIEVES THAT EE'S TESTICLES HAD BECOME TWISTED. TESTICLES WERE DETORSED BY ER PERSONNEL. EE RELEASED TO LIGHT DUTY UNTIL 7/1/03. FOLLOW-UP VISIT TO

EE WAS PUTTING AWAY SUPPLIES IN A TWO-DOOR METAL SUPPLY CABINET. EE CUT THE MIDDLE FINGER OF HIS RT. HAND ON A SHARP EDGE ABOVE THE TOP SHELF. EE WAS SEEN BY MD ON 5/2/03 & RECEIVED 6 SUTURES TO HIS HAND. EE WAS PLACED ON RESTRICTED DUTY. NO LIFTING, PUSHING, PULLING MORE THAN 10 LBS. WITH HIS RT. HAND.

EE WAS TRAVELING ACROSS PLANT RAILROAD TRACKS ON A 4500# CAPACITY FORKLIFT WITH TIPS APPROXIMATELY 2" ABOVE ASPHALT & BOOM TILTED FORWARD. RIGHT FORK TIP JUST HIT TOP EDGE OF RAIL CAUSING FORK TO BURY AGAINST RAIL IN ASPHALT. EE HAD SEAT BACK. THE FORCE OF IMPACT CAUSED SEAT TO DETACH FROM FORKLIFT. EE HIT STEERING COLUMN WITH CHEST & KNEES. HE WAS SEEN BY MD WHO GAVE RESTRICTIONS.

EE REPORTED TO HIS SUPERVISOR THAT HIS LEFT SHOULDER WAS SORE WHEN ASKED TO MANUALLY FILL 50 LB. BAGS OF CRUSHED LIMESTONE. HE DOESN'T KNOW WHEN HOW OR WHERE HE INJURED HIMSELF. HE MAINTAINED THAT HE WAS NOT INJURED AT HOME. HE WENT TO HIS OWN MD WHO SAID INJURY WAS WORK RELATED. SENT EE TO COMPANY MD ON 3/25. MD IS TREATING INJURY AS WORK-RELATED AT THIS TIME. UNDER INVESTIGATION.

EMPLOYEE WAS SHOVELING 1/4" PRODUCT ON FLOOR OF DRIER BUILDING. EMPLOYEE STRUCK SHOVEL BLADE INTO CONCRETE CURB WHILE SCOOPING PRODUCT. EMPLOYEE FELT PAIN IN HIS LEFT FOREARM AND WAS RELEASED TO MODIFIED DUTY WITH NO USE OF LEFT ARM AND UPPER LEFT EXTREMITIES. EMPLOYEE RETURNED TO MD ON 10/17/02 AND WAS RELEASED TO FULL DUTY.

EE INJURED RIGHT HAND USING POWER WRENCH TO CLOSE DOOR ON RAIL CAR. EE WAS TREATED BY MD AND WAS RELEASED TO MODIFIED DUTY NO USE OF RIGH HAND. EE WAS GIVEN A PRESCRIPTION FOR ANTIBIOTIC WHICH HE DID NOT HAVE FILLED. UPON FOLLOW UP MD PLACED EE OFF OF WORK DUE TO INFECTION.

EE FILLED 50 LB BAGS ON 7/13/02. HE ASKED TO FILL BAGS AGAIN ON 7/18 BUT SAID LEFT SHOULDER HURTFROM 7/13. HE REFUSED MEDICAL TREATMENT. HE ASKED TO FILL BAGS IN FEB. 2003 BUT SAID LEFT SHOULD ER HURT WITH PAIN STILL FROM 7/13. REQUIRED HIM TO SEE MD ON 2/28/03. INJURY DETERMINED TO BE WORK RELATED ON 3/13/03. INJURY UNDER INVESTIGATION BY WORKER'S COMP. CARRIER.

EMPLOYEE COMPLAINED TO SUPERVISOR THAT HIS LEFT ELBOW WAS SWOLLEN. DID NOT KNOW WHAT HAPPENED BUT RELATED IT TO AN INJURY HE INCURRED IN 2001. THE INCIDENT WAS TREATED AS A REOCCURRING INJURY. EMPLOYEE WAS SENT TO MD WHO DIAGNOSED A BONE SPUR. COMP CARRIER HAS DETERMINED ON THIS DATE THAT CONDITION IS AS INDUSTRIAL ILLNESS AND NOT RELATED TO PREVIOUS INJURY. EMPLOYEE RESTRICTED TO

EMPLOYEE INJURED RIGHT HAND ON PALLET IN 1990. EE REPORTS ON 5-29 THAT OVER THE YEARS THE HAND WOULD SWELL AND CAUSE PAIN. ON 5-29, NOTICED EE HAD A BRACE ON RT HAND. QUESTIONED EE ABOUT BRACE AND DISCOVERED HIS HAND WAS VERY SWOLLEN. SENT EE TO MD WHO TREATED EE FOR RT WRIST STRAIN. RELEASED EE TO MODIFIED DUTY WITH 15 LBS LIFTING, PUSHING, PULLING RESTRICTION TO RT ARM.

EE STEPPED FROM TOP RUNG OF LADDER (5'4"HIGH) TO20"X 24"PLATFORM TO GET SAMPLE FROM A FINES FEEDER. EE TRIPPED ON EMERGENCY STOP PULL CORD THAT IS LOCATED 4"ABOVE PLATFORM. EE FELL, CATCHING HIS LT. HAND ON A HANDRAIL IN THE IMMEDIATE AREAEE TREATED BY MD FOR SPRAIN/STRAIN OF LT. THUMB.EE RELEASED TO MODIFIED DUTY. NO PUSHING,PULLINGLIFTING WITH HIS LT. HAND.

EE USING HAND DRILL TO DRILL HOLE IN STEEL. DRILL BIT CAUGHT ON WELD AFTER MAKING HOLE. DRILL TWISTED OUT OF EE'S HAND AND STRAINED MIDDLE FINGER OF RIGHT HAND MIDDLE FINGER BECAME SLIGHTLY SW OLLEN BY NEXT DAY. SENT EE TO MD WHERE HAND WAS WAS EXAMINED. EE RELEASED TO MODIFIED DUTY LIMITED USE OF RIGHT HAND FOR ONE WEEK.

EMPLOYEE INJURED RIGHT FOREARM WHILE LIFTING 100 LBS BAG OF PRODUCT ON 4-3-02. EE REPORTED INJURY ON 4-15 BUT REFUSED MEDICAL TREATMENT. INJURY WAS TREATED AS A FIRST AID CASE. ON 5-24-02, EE STATED INJURY WAS NOT GETTING BETTER. SENT EE TOMD WHO TREATED EE FOR RT FOREARM STRAIN. RELEASED EE TO MODIFIED DUTY WITH 20 LBS LIFTING RESTRICTION TO RIGHT ARM.

EMPLOYEE WALKED THROUGH A PRODUCT SPILL THAT LED TO THE ENTRY TO A CONVEYER BELT WHICH CONCEALED A LARGE STONE. HE STEPPED ON THE STONE AND TWISTED HIS RIGHT ANKLE AS HIS FOOT ROLLED OFF THE S TONE. EE HAD HIS RIGHT KNEE ON THE GROUND CAUSING A CONTUSION OF THE KNEE. EMPLOYEE WAS TREATED BY A MD AND RELEASED TO RESTRICTED DUTY.

EMPLOYEE FELL WHILE BOARDING FORKLIFT. EE HAD RIGHT FOOT IN STEP AND LEFT FOOT ON PLATFORM AND LOST FOOTING OF RIGHT FOOT FALLING BACKWARDS ONTOLEFT BUTTOCKS. EE COMPLAINED OF PAIN IN LEFT SHO ULDER BLADE AND LEFT BUTTOCK. EE WAS TREATED BY MD AND RELEASED TO RESTRICTED DUTY. LIMITED USE OF LEFT ARM.

EMPLOYEE PHONED SAFETY ENGINEER IN A.M. OF 03/04/02 AND SAID THAT HE COULD NOT COME TO WORK DUE TO SEVERE STRESS. HE ALLEGES STRESS IS WORK-RELATED. UPON BEING INTERVIEWED HE COULD NOT SPECIFI CALLY IDENTIFY ANY REASON WHY ILLNESS IS WORK-RELATED OTHER THAN IT STEMS FROM HIS EVERYDAY DUTIES AS UTILITY OPERATOR. EMPLOYEE WAS REFERRED TO A PSYCHOLOGIST. HE WAS NOT RELEASED TO WORK.

EE WAS USING A LARGE WRENCH TO TIGHTEN A NUT. EE FELT A PAIN IN HIS LEFT SHOULDER. EE WAS SENT TO MD FOR TREATMENT & WAS RELEASED TO RESTRICTED DUTY. LIGHT USE OF LEFT ARM & HAND. EE WAS GIVEN PRESCRIPTION DRUG FOR PAIN.

EE WAS CARRYING A 5 GAL BUCKET UP A FLIGHT OF STAIRS WHEN HE SLIPPED AND A STEP. EE DID NOT FALL BUT CAUGHT HIMSELF. EE FELT PAIN IN HIS LEFT BUTTOCKS. EE WAS USING A VACUUM TRUCK TO CLEAN PRODUCT FROMT HE FLOOR UNDERNEATH A PRODUCT SILO, AS EE BENT OVER TO MOVE A 6" VACUUM HOSE, EE FELT MUSLCE PULL IN HIS BACK. EE WAS SENT TO DR AND WAS TREA TED FOR A SACROILIAC STRAIN. EE WAS RELEASED TO RESTRICTED DUTY. HOWEVER THE RESTRICTION WERE SUCH THAT NO VIABLE WORK IS AVAILABEL TO EE LTA.

EE WAS RELEASE WITH RESTRICTION OF NO LIFTING PUSHING PULLING.

EE WAS DROPPING BOTTOM PADS OF PACKER TO CLEAN PACK BIN OUT FOR PRODUCT CHANGE-OVER. HE HIS HISLEFT ELBOW ON METAL STRUCTURE OF PACKER. HE WENTTO DOCTOR & WAS RESTRICTED TO LIGHT USE OF ARM.

AS EE DESCENDED RAMP IN REVERSE, FORKLIFT LEANED IN DIRECTION OF FALLING BAG & ROLLED OVER. STRUCK SHOULDER

EE HAD A SENSITIVE BACK (WHICH WENT UNREPORTED FROM PACKING FOR 12 HRS STRAIGHT EARLIER IN WEEK. EE WAS PACKING 50 LB BAGS ON 8-11-01 WHEN HE TURNED WITH A BAG AND HEARD A LOUD CRACKING NOISE IN HIS UPPER BACK. EE IMMEDIATELY SENT TO MD. EETOLD TO BED REST FOR 3 DAYS AND THE REVISIT MD ON 8-13-01 @ 1045 HOURS.

EE WAS WORKING ON TURN HEAD IN MILL BUILDING WHEN LARGE PLUME OF DUST ROSE UPWARD THRU BUILDING CAUSING POOR VISIBILITY PLUME WAS CAUSED BY HIGH WINDS OUTSIDE BUILDING EE THOUGHT PLUME WAS CAU SED BY AN EARTHQUAKE AND BECAME VERY EXCITED. EETRIED TO VACATE AREA IN A HURRY WHEN HE SLIPPED AND FELL.

REPLACING PAD ON PUSHER BAR IN THE AUTOMATIC PACK CENTER. PAD IS MOUNTED ON RETRACTABLE FLAPPER. WHEN EE WENT TO REMOVE OLD PAD AND IN THE PROCESS OF TIGHTENING UP THE NEW PAD, THE ELECTRIC EY E WAS ACTIVATED. WHEN SAW MOVEMENT(HAND) AND ACTIVATED THE FLAPPER AND PINCHED EE HAND.
