Employee was injured by a small metal shard that entered his right forearm while he was attempting to remove ground engagement (teeth) from the bucket of the Komatsu WA900 loader. He was using a 20lb sledge hammer to knock the "teeth" off of the shank when a piece of metal chipped off of the tooth and lodged into his arm.

Employee was removing a bolt from the jaw crusher when the bolt struck him in the mouth causing a laceration that required stitches and a chipped left front tooth. Employee was transported to the clinic immediately and was released back to work with no modifications.

EE came into the office on 4/24/07 to fill out a incident report for a sore knee. He indicated that while coming down the staircase at the #1 Crusher he slipped on the stairs about 4 to 5 steps from the top and was able to catch himself with the handrails. He did not fall but said he twisted his left knee and did not need to go to the clinic.

Employee was moving the screen down by the crush plant with the forklift. He got out of the forklift to check on the forks when he tripped over the screen and fell and cut open his left knee.

EE WAS WALKING WITH A PIECE OF PLYWOOD IN WHICH COVERS THE PIT. HE DID NOT SEE THE RIGHT HALF OF THE PIT WHEN HE SLIPPED AND FELL IN AND STRUCK HIS LEFT SHIN AGAINST A METAL EDGE OPENING.
EE WAS USING A HAMMER TO REMOVE FROZEN BOLT FROM RADIATOR MOUNT. HE WAS USING A SMALL HAMMER THAT WAS BEING USED AS A PUNCH WITH 2 LB SHOP HAMMER. A SMALL PIECE OF THE HAMMER CHIPPED OFF AND S TRUCK HIM IN HIS LOWER ABDOMEN. THE SKIN WAS PENETRATED AND THE METAL CAME TO REST APPROX 1" DEEP.
EE WAS REMOVING A TRANSMISSION FILTER HOUSING THAT WAS WEDGED IN POSITION. AS HE PULLED THE LOWER END TO GET IT LOOSE THE WEIGHT SHIFTED AT THE BOTTOM OF THE FILTER HOUSING CATCHING HIS RIGHT LITTLE FINGER BETWEEN HOUSING.
EE STARTED SHOVELING DIRT IN OUR TUNNEL WITH COWORKER ABOUT 0745 AND ABOUT 10 MINUTES INTO SHOVLING HE HIT STEEL WITH HIS SHOVEL, SO HE REACKED DOWN TO PICK IT UP AND WHEN HE WAS TURNING TO PU T THE CHANNEL OFF TO THE SIDE OUT OF HIS WAY HE FELT SHARP PAIN IN HIS LOWER BACK ON HIS LEFT SIDE.
EE WAS SHOVELING MATERIAL OFF OF P-5 BELT AND FIGURES THAT HE PULLED SOME MUSCLES IN HIS BACK WHILE DOING THAT. HE DIDN'T FEEL ANY PAIN RIGHT AWAY, UNTIL FRIDAY (2-21-03) MORNING.

EE WAS DUCKING UNDERNEATH A CHUTE & MOVING FORWARD WHEN HE DIDN'T CLEAR THE CHUTE, HIS HARD HAT STOPPED HIM AND AS A RESULT HE HAS A SORE NECK. DR INITIALLY RETURNED EE TO WORK WITHOUT RESTRIC TIONS, BUT ON FOLLOW-UP, DR RESTRICTED EE TO NOT LIFTING UP TO 50 LBS UNTIL 11 NOV 2002.
EMPLOYEE WAS HELPING TO CARRY A 7'X16"X1/4" METAL PLATE INTO THE TUNNEL TO REPAIR SIDE SKIRTS ON FEEDER BOX. EMPLOYEE SLIPPED ON LOOSE GRAVEL AND CAUGHT RIGHT MIDDLE FINGER BETWEEN THE METAL P LATE AND GRAVEL. FINGER WAS LACERATED, REQUIRING SUTURES. EMPLOYEE WEARING GLOVES AT THE TIME OF THE INCIDENT.
WHILE REPLACING A MOTOR IN #2 CRUSHER, EE WAS USING A PRY BAR TO MOVE MOTOR SO THAT THE CRANE COULD PICK IT UP. THE MOTOR SPUN, CAUSING THE PRY BAR TO STRIKE THE HANDRAIL, CATCHING EE'S HAND BETWEEN THE PRY BAR AND THE HANDRAIL.
