Injured worker was working on hopper underneath the 6x16 screen. The job was welding patches. EE had EE's back leaning against the wall with one leg extended and the other one bent while welding. When the job was done, EE extended EE's legs, and EE's knee was numb. And started feeling like EE pulled something on EE's back.

While patching holes on wet screen chute. Employee slipped with knife cutting left finger.

The pin on the rental excavator was walking out the wishbone, operator was holding the pin in place while maintenance guy was trying to realign the bearings. The mechanic asked the operator to pull the pin out a little and they pulled it all the way and that caused fingers to stick out of the hole causing the very tip of the maintenance man's finger to get cut.

When knocking the wedge tooth off with a hammer, the corner the tooth broke off shooting through employee's overalls into EE's abdomen skin.

Employee was using hammer on conclave liner to remove it. EE placed EE's foot under the liner and was injured.

Employee was walking out of office and twisted ankle on threshold in doorway.

Audiometric test shows Severe loss

The audiometric test shows moderate loss in both ears.

Audiometric test result says moderate loss.

Employee was sent to the clinic on March 27th 2018 because of ankle pain. The report from the clinic came back "unresolved pain causation."

The employee went to personal doctor for a pain in the right elbow and the doctor told the employee to report to employee's supervisor.

Audiometric testing showed a hearing loss - unknown cause.

The employee was cleaning the under flow pipe while working in the mud tank. Allegedly developed rashes on hands 16 days after contact with the mud. Employee did not lose any time until 03/02/2015 when we received a letter from Arrowhead Orthopedics advising us about his restrictions.

A standard threshold shift was reported on employee's annual hearing conservation test.

When cleaning a shovel tooth from telsmith jaw ee asked for a shackle. Another ee dropped one down in the feeder ee tried to catch it and cut his finger.

Stepping down off of drill onto uneven rocky ground.

Cleaning electric shaft with brake cleaner. Lighting the torch causing the fumes to flash. The flash caused minor burns to EE's face. (Looks like sunburn).

Employee stated that he hurt his shoulder in the water truck. He hit rock and jerked the wheel causing a shoulder injury.

Standing over top of conveyor - bending over to remove troughing rollers - twisted back wrong. Reported as incident only on 8/15/12 (1st aid). Continued working until 8/22/12 - back hurting more.

The adjusting bar on the gate on the transfer box 15A caused a pinch point to the box - Pinched employees forth (ring) finger right hand.

Repetitive fabricating of equipment guards.

EE was attempting to remove a keeper pin on the side of the bucket with a chipping hammer when a sliver of metal came off and lodged in the upper part of his right hand.

Conveyor man put arms & shovel inside truss to clean under conveyor. While shoveling he stuck the shovel between the return roller and the belt. His left hand was bruised.

When removing manhole cover on shovel boom, grip was lost on one of the handles. The cover was too heavy to be held with other hand.

Cause of injury is unknown. Employee alledges back muscle spasms. Employee alledges he was changing screens.

Pit conveyor #2-removing rail pulley guards

EE was running skiploader moving dust, it was windy and dust flew into EE's left eye.

Replacing Lippmann jaw die - climbing out of the feeder hopper, slipped.

Attached 1 1/2 ton comealong to flanged area of standard crusher feed box-pulled box closed-tightened flange bolts come along hook slipped off of flange.

Hauling super sacks of baghouse dust with a pallet jack up a slight grade, injured lower back.

Repairing feeder belt on FC-2 cut finger on guard.

Trying to pull torn feeder belt out from hopper-rocks were jammed in head pulley-pulled v-belts to free up.

Failed to recognize (identify) hazard - overhead jack.

Torch cutting screen when a bolt popped sending hot piece of metal into left ear.

Operating 5160 Bobcat, dumping bucket of fines on N/W side of screen 8, when loose hyraulic hose fitting sprayed oil in his face and on his chest.

Pinched finger with wrench while working on drill bit.

Santa Ana winds were blowing. We were loading shot with windy conditions. Loaders were digging below us creating more dust - got dust in eye.

Employee was doing normal duties of lowering hose into hole. As he was doing this, he felt pain in his lower back.

Employee was moving portable light plant with #270445 service truck (light plant had broken jack that would not retract or extend)When lifting tongue of light plant off of truck hitch, light plant shifted towards employee smashing left hand thumb on bumper of truck.

Employee was moving a suction hose & pump in the pit area with a forklift. Hit a low spot in the pit area and the forklift leaned over onto two wheels. He jumped from forklift spraining his ankle.

loading drill steel into steel rack-when it was handled-it slipped & smashed fingers. Originally was 1st aid, 1 finger sore & infected. Sent to clinic 10/5/06.

Left work on the 17th-(ok) woke up on the 18th a.m. @ home with eye problem-came to work @ 6 am-problem kept getting worse-went to eye dr. foreign body rt. eye

EE was working and all of a sudden couldn't feel his left pinky ring fingers, left arm & leg went numb.

EE was helping hookup klankity klank water pump when he slipped and fell injured left knee as he was leaving sand plant.

employee walked on a motormount catwalk, that was being built. He stepped in the hole for a pump shaft. Sprained right knee.

Repairmen were changing out a crusher. The hand rail on the work platform had been removed so the the crane could lift out the crusher. EE stepped off the platform while the temporary rail was being prepared. He was hurt while trying to break his fall.

While using a cutting torch on steel of primary crusher (wearing goggles), slag flew up under goggles and got into left eye.

Repairing crusher, he jumped from conveyor to catwalk causing left knee strain and contusion.

Swinging sledge hammer to break concrete surrounding 10' pipe, choked up on handle - finger got caught between handle and concrete. Laceration left index finger.

PACKING UP SHAFT WITH BEARING TO MOVE STACK ON A PALLET. LEFT KNEE GAVE OUT CAUSING HIM TO FALL AND THE BEARING FELL ON TOP OF LEFT LEG.

WHEN EMPLOYEE STARTED TO FALL, WHICH BOTH EMPLOYEES WERE CARRYING OVER TO A PALLET (SHAFT & BEARING) INJURED TRIED TO HELP HIM FROM GETTING HURT. WHEN HE DID SO, HE HURT HIS BACK.

While cleaning splitters in tower 3 chipping a rock. Rock rolled down landing on top of left foot.

Employee was hit by fly-rock while looking for a truck to load. He was walking on the driveway of the parking lot east of heavy equipment shop. Incident occured on company owned property.

WALKING ON OILY ROCKS TO GET TO TANK (TO REPLACE) LOCATION, SLIPPED AND CUT ARM WHILE TRYING TO BREAK FALL.

BREATHING DUST INSIDE LOADER.

LIFTING LADDER ON SHOVEL TO LET IT DOWN. FELT SHARP PAIN IN LOWER BACK.

REPLACING BEARING ON DRIVE SIDE OF 7X24 SHACKER FROM THE INSIDE. PUT IN GUIDE BOLT FOR BEARING HOUSING.

WORKING OFF OF THE BACK OF A TRUCK WITH A VISE, RECEIVED MULTIPLE LACERATIONS TO RIGHT HAND.

ADDING STEEL RODS TO DRILLING MACHINE WHEN RIGHTMIDDLE FINGER GOT SMASHED BETWEEN TWO RODS.

OVER CRANE HOISTING UP. THREEWAY SNAGGED ON CRUSHER. POPPED OFF AND HIT RIGHT ARMPIT AND ARM.

WELDING ON DRILL WHEN PIECE OF SLAG GOT ON EYELID AND PART OF EYE. (LEFT)

HURT LEFT KNEE WALKING UP PIT CONVEYOR CATWALK. WENT TO PRIVATE DR WHO NOTIFIED HIM HE THOUGHT IT WAS WORK RELATED. REPORTED TO SUPERVISOR 3/24/00.
